Vessels that operate in salt water must undergo two drydock exams within a 5 year period. No more than 3 years may elapse between any two examinations. Vessel must undergo a drydock exam before the carriage of passengers.

All engine installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Generator installation was found to be leaking diesel fuel and it was unable to do automation tests such as overspeed and low lube oil. Repair leak and demonstrate tests prior to the carriage of passengers.

The minimum instrumentation, alarms, and safety controls required for specific types of systems are listed in Table 62.35-50. Vessel could not perform low lube oil shutdowns on either the main propulsion engines or the generators. Vessel is required to perform these tests to the satisfaction of USCG MI.

When only the main steering gear is power-driven, two separate and independent systems for full followup control must be provided in the pilothouse; except that the steering wheel or lever need not be duplicated. Vessel reported loss of steering pump 2. Affect permitted repairs to the satisfaction of the USCG.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Lifebuoy light on starboard side main deck was found to be inoperable. Rectify deficiency prior to getting underway after sunset or within 14 days.

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Inspectors found multiple emergency lighting fixtures with inoperable lights installed. Vessel is to replace bulbs in each faulty emergency lighting fixture. 46 CFR 111.75-15(c)(2) 60c

Gears, couplings, flywheels and all machinery capable of injuring personnel shall be provided with adequate covers or guards. Rotating machinery in the emergency generator room was found to not have a guard over it’s right side, exposing a rotating shaft capable of causing injury. 46 CFR 58.01-20 16c

The respective piping systems installed on ships shall have the necessary valves and flanges for safe and efficient operation of the vessel. Bow thruster directional control valve is leaking significantly. Valve shall be repaired, be leak free, and have all hydraulic fluid removed from the bilge to the satisfaction of attending Coast Guard Marine Inspector.

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. On systems in which the CO2 cylinders are not within the protected space, these instructions must also include a schematic diagram of the system and instructions detailing alternate methods of discharging the system should the manual release or stop valve controls fail to operate. Each control valve to branch lines must be marked to indicate the related space served. Marine inspector witnessed labeling for the CO2 fire suppression system was incorrectly labeled on the system when comparing to detailed operating instructions and system schematic diagram was missing.

Fixed fire extinguishing systems must be checked as noted in table 71.25–20(a)(2). In addition all parts of the fixed fire extinguishing systems must be examined for excessive corrosion and general conditions. Marine inspector witnessed documentation for CO2 fire suppression system was missing requirements in the CFR cite.

Central control locations must (i) Be arranged to allow the operator to safely and efficiently communicate, , control, and monitor the vital systems under normal and emergency conditions, with a minimum of operator confusion and distraction. Marine Inspector witnessed the M/E low oil pressure indicator lights on the bridge were inop.

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering-gear compartment. The rudder-angle indicator must be independent of control systems for steering gear. The pilothouse and engine room rudder angle indicators did not reflect the same rudder angled. Repair/adjust indicators so they match and reflect the correct rudder angle.

Onboard training, musters, and drills: held in accordance with Subchapter W (Lifesaving Appliances and Arrangements) of this chapter. During abandon ship drill MI observed the crew and during questioning crew was unfamiliar with duties during drill. MI asked master if there were any training manuals or training procedures that were being utilized to train and instruct crew on proper procedures, there are no training manuals or aids other than the training received for crew licensing. Vessel must develop crew training manual in accordance with 46 CFR 199.180 parts A through H as applicable.

The outlet at each fire hydrant must be provided with a cock or valve fitted in such a position that the firehose may be removed while the fire main is under pressure. Marine Inspector (MI) identified a broken valve for the fire hydrant on the port side of the top deck, which must be replaced.

Each item of lifesaving equipment required by this part to be carried on board the vessel must be approved. Marine Inspector (MI) observed that the lifefloat onboard does not have an approval placard or any information indicating that it is approved. 46 CFR 199.40(a)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. MI observed one self-igniting waterlight attached to a ring life-buoy did not work. 46 CFR 199.190(a)

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. MI observed six emergency lights that had inoperable bulbs. 46 CFR 111.75-15(c)(2)

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. MI observed the access panel of an electrical distribution box in the machinery space was not secure allowing potential contact by a person. 46 CFR 111.01(a)(2)

Each self-propelled vessel must have duplicate light sources for the side, masthead, and stern lights. MI observed that the secondary masthead light alarm was alerting that the light was not working. 111.75-17(c)

Means shall be provided for closing all openings to the space protected from outside such space. Vessel reported allision and subsequent damage sustained to starboard side ventilation cover leading to engine room. Renew/replace closure to satisfaction of attending Marine Inspector. CITE: 46 CFR 76.15 35(c)

All passenger vessels shall have efficient guard rails or bulwarks on decks and bridges. As result of allision, vessel sustained damage to railings on starboard side. Renew railings to satisfaction of attending Marine Inspector. CITE: 46 CFR 72.40 5(a)

All vessels shall be provided with a satisfactory bilge pumping plant capable of pumping from and draining any watertight compartment. Marine Inspector (MI) observed corrosion on the rudder room bilge suction pipe. After exploring the corrosion utilizing an inspection hammer, MI punctured a ½” hole in the pipe. Renew pipe to the satisfaction of the USCG MI.

Cables manufactured to IEC 60092-353 must be installed in accordance with IEC 60092-352 (both incorporated by reference; see 46 CFR 110.10-1), including clause 8. MI observed metallic support bands for the horizontal electrical cable run on the Galveston deck are all corroded away and need to be renewed.

MI’s observed fuel leaking from inside of the port valve cover on the port main diesel engine indicative of a failure/leak of the fuel system within the valve cover. Vessel shall identify and correct all fuel leaks to the satisfaction of attending Coast Guard Marine Inspector.

46 CFR 58.10-5 Engine design. All installations shall be or marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. MI’s observed fuel leaking from inside of the starboard valve cover on the starboard main diesel engine indicative of a failure/leak of the fuel system within the valve cover. Vessel shall identify and correct all fuel leaks to the satisfaction of attending Coast Guard Marine Inspector.

46 CFR 58.10-5 Engine design. All installations shall be or marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. MI’s observed fuel leaking from three fuel injectors on the Port Ship's service generator engine. Vessel shall identify and correct all fuel leaks to the satisfaction of attending Coast Guard Marine Inspector.

46 CFR 56.01-1 The respective piping systems installed on ships shall have the necessary valves and flanges for safe and efficienct operation of the vessel. Bow thruster directional control valve is leaking significantly, vessel is restricted from operating until valve is repaired and leak free to the satisfaction of attending Coast Guard Marine Inspector.

46 CFR 111.81-1 (f) Each outlet must be constructed to the appropriate NEMA or IEC standard. MI’s observed several electrical outlets on the Galveston deck up forward in the dining area on the columns as well as on the Texas deck in the aft portion of the bar area that were broken and presented an electrical hazard. Vessel is to replace all broken outlets/covers to the satisfaction of attending Coast Guard Marine Inspector.

46 CFR 56.20-9 Each valve must close with a right-hand (clockwise) motion of the hand wheel or operating lever as seen by one facing the end of the valve stem. MI's observed that the bilge system overboard valve was not able to be operated by hand. Vessel shall repair/replace valve to the satisfaction of attending Coast Guard Marine Inspector.

46 CFR 78.47-40 Illuminated exit signs are required and must be installed in accordance with subchapter J. MI's observed that the emergency exit for forward auxiliary space did not have an illuminated exit sign. Vessel shall install an illuminated exit sign to the satisfaction of attending Coast Guard Marine Inspector.

33 CFR 159.7 (c) When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. MI's observed that the sewage overboard valve was open. Vessel's crew closed the valved and secured it in the closed position with a chain and padlock.

The activation of any detector or manual pull station must cause an audible and visual fire detection alarm signal at the control panel. Marine Inspectors observed that the fire control panel in the navigation bridge did not alarm when the smoke detector in the generator room was tested, which must be corrected to the satisfaction of the attending Marine Inspector.

Each vessel must carry lifejackets approved under approval series 160.155, 160.176 or 160.177 and each lifejacket must be marked With Type I retro-reflective material approved under approval series 164.018. The arrangement of the retro-reflective material must meet IMO Resolution A.658(16). 06 Lifejackets found in unsatisfactory condition with bouyant material degraded. 01 lifejacket requires found with degraded reflective tape. Replace 06 lifejackets and renew reflective material on 01 lifejacket.

Each vessel must carry lifebuoys approved under approval series 160.150. The lifebuoys shall be fitted with retro-reflective material (tape) with a minimum of 5 cm (2 inches) width around the lifebuoy at four locations - spaced equally around the circumference of the lifebuoy. Retro-reflective material shall be Type II material that is approved under 46 CFR 164.018. Ring buoys on Main Deck, 01 Deck, and Stern deck found with degraded retro-reflective material. 04 ring buoys require tape renewal. Renew reflective material.

Electric installations on vessels must ensure maintenance of services necessary for safety under normal and emergency conditions. Battery in pilothouse found with presence of moisture and corrosion at terminals. Replace battery.

Each navigation light must be installed so that its location and its angle of visibility meet the applicable navigation rules. The second anchor light is inoperable.

On vessels required to have a final emergency power source by § 112.05-5(a) of this chapter, the following emergency lighting and power loads must be arranged so that they can be energized from the final emergency power source: Lighting for survival craft launching, including muster stations, embarkation stations, the survival craft, its launching appliances and the area of the water where it is to be launched. Life float and IBA locations do not have functional lighting. Lighting must be brought into operational condition.

Emergency means of communication required by this subpart must provide common talking means of two-way voice communication and calling among the navigating bridge, emergency control stations, must be a reliable means of voice communication and must be independent of the vessel's electrical system. Marine Inspectors witnessed inability among crew communications between engine room and bridge. Provide evidence of reliable means of communication.

Be arranged to allow the operator to safely and efficiently communicate, control, and monitor the vital systems under normal and emergency conditions, with a minimum of operator confusion and distraction. Several alarm indicators in the navigation bridge are unreadable.

The respective piping systems installed on ships shall have the necessary valves and flanges for safe and efficient operation of the vessel. Bow thruster directional control valve is leaking significantly, vessel is restricted from operating until valve is repaired and leak free to the satisfaction of USCG Inspector.

Battery operated lanterns must be capable of providing light for at least 3 hours. STBD side FWD Galveston Deck Emergency Light illuminated for approximately 5 minutes while on emergency power.

Nothing in this subpart shall be construed as limiting the inspector from making such test or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. Vessel broke free from its moorings during heavy weather and became hard aground in muddy bottom approx 200 feet from pier. Vessel reported no hull damage or flooding during interior inspection by crew. Vessel stated they will have divers check the bottom once re-floated. Provide sea trial report and dive report to USCG Marine Inspectors for approval prior to carrying passengers.

Each firehouse on each hydrant must have a combination solid stream and water spray firehose nozzle that meets the requirements of 46 CFR 162.027. Firehose nozzle at fire station #10 was spraying water through fittings and attachments due to appreciable corrosion.

Each EAP education program must include at least the following elements: Display and distribution of a community service hot-line telephone number for crewmember assistance. Vessel was unable to identify a location or provide evidence of a community service hot-line number being posted or displayed for crewmember assistance.

Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of the circuit. Twist-on type connectors were observed being used to facilitate a splice in the air compressor room.

Repair/replace check valve on fire pump.

Provide machinery guards for #1 generator.

Repair emergency light forward port side of E/R.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Marine Inspectors identified 04 unserviceable adult life jackets from overhead lockers located on the 01 deck and 22 unserviceable children’s life jacket from 03 lockers located on the main deck and 02 deck. In addition, the overhead lifejacket stowage lockers were inoperative. Replace the unserviceable life jackets with the same type and quantity IAW with the COI requirements and service inoperative stowage locker latches.

Each link certified by the manufacture to meet the requirements of this subpart must have a corrosion resistance, waterproof tag attached to it. Marine Inspectors observed that the 25 person life float (aft of the bridge) had a weak link without a tag, nor could the tag be located on the vessel. Replace the existing weak link with the correctly rated weak link for the life float on the vessel. 46 CFR 160.073-20

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Inspectors found an emergency light on the main deck that wasn’t illuminating properly. Vessel to repair light to the satisfaction of a USCG Marine Inspector. 46 CFR 111.75-15(c)(2)

Each cable installation must meet sections 25, except 25.11 of IEEE 45-2002. Cable retention devices should be installed not less than 2.5 meters on horizontal runs. Vessel had wasted cable supports on the upper deck below the bridge space. Vessel to replace the cable supports to the satisfaction of a USCG Marine Inspector. 46 CFR 111.60-5

All passenger vessels shall have efficient guard rails or bulwarks on decks and bridges as follows: The decks and bridges as follows: The height of rails or bulwarks shall be at least 39 ½ inches from the deck. Deck rails around the bridge were found wasted completely through. Vessel to replace and repair the wasted railing to the satisfaction of a USCG Marine Inspector. 46 CFR 72.40-5(a)

All portable fire extinguishers and semi-portable fire extinguishing systems must be maintained in accordance with NFPA 10. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Each lifesaving appliance, including lifeboat equipment, must be inspected monthly using the checklists under paragraph (b)(1)(i) of this section to make sure the appliance and the equipment are complete and in good working order. A report of the inspection, including a statement as to the condition of the equipment, must be recorded in the vessel’s official log book. Vessel is not performing monthly service inspections on firefighting or lifesaving equipment. Vessel is to start monthly inspections of both the firefighting and live saving equipment and provide a record of inspections. Verification of the monthly inspections and logs will be made by a USCG Marine Inspector at the next quarterly. 46 CFR 199.190(e)

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the stands established by the ABS Steel Vessel Rules MI observed exhaust leak on the #1 STBD Generator. 46 CFR 58.05-1(a)

Electrical installations on vessels must ensure protection of passengers, crew, other persons, ad the vessel from electrical hazards. An extension cord (draped over the sink) and power strip were found supplying power to multiple appliances at the snack bar on the 2nd deck. Extension cords are intended for temporary use, and not permanent installations. A permanent electrical installation shall be used to power appliances in the snack bar to the satisfaction of the attending Marine Inspector.

The location of equipment (portable fire extinguishers) shall be to the satisfaction of the Officer in Charge, Marine Inspection. The fire extinguisher in the snack bar on the 2nd deck was hidden behind the sink and ice machine and not located in the open so as to be readily seen. The fire extinguisher shall be moved to where it is readily visible in case of emergency to the satisfaction of the attending Marine Inspector.

Each entrance to a space storing carbon dioxide cylinders or a space protected by carbon dioxide must be conspicuously marked as follows: Spaces storing carbon dioxide - "CARBON DIOXIDE CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATON." Spaces protected by carbon dioxide - "CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHE ALARM OPERATES OR WINTERGREEN SCET IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING." The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. The carbon dioxide storage space and EDG space (CO2 protected space) were not marked properly. Ensure proper marking are on both spaces to the satisfaction of the attending Marine Inspectors.

Vents extending above the freeboard deck from fuel oil tanks must be at least Schedule 40 in wall thickness. A fuel tank vent on the fwd port side of the vsl was found to have significant wastage where the vent piping is welded to the deck. Permanent repairs shall be made to the satisfaction of the attending Marine Inspector at the next dry dock.

Remove red and green holiday lights that interfere with navigational lights.

Before vessel leaves port and all timees during voayge, each lifesaving appliance must be in working order and ready for immediate use. Repair or replace lifefloat straps for grab lines to the satisfaction of the attending Marine Inspector. 46 CFR 199.190(a)

Vessel added 20 lifejackets for a total of 650. Vessel is restricted to 650 total persons on board.

Provide information for ECDIS chart stard and method for updating.

Provide 69 life jackets. Vessel is restricted to 630 total persons on board.

Crop and renew fire pump piping IWO hole/leak.

The top ladder rung servicing the emergency exit from the pump room is broken. Re-weld connection.

Lifefloat improperly stowed. Debris obstructing float free arrangement. Remove all loose gear IVO lifefloat station.

WTD leading from the engine room to the pump room is not properly sealing. Adjust door as needed. Chalk test.

Fire hose station #5 and #6 are obstructed by the life jacket boxes. Make accessible and re-mark for all to see.

Improper spanner wrench attached to fire hose station #2. Replace with correct type of spanner wrench.

Machinery spaces contained excessive amount of uncollected oil rags, wood material and improperly stowed equipment which is blocking access to vital equipment.
