Pre-engineered gas system not meeting the following requirement: -Must shut-down power ventilation systems upon discharge. Must make repairs to satisfaction of USCG Marine Inspector.

Port shaft sheared causing a loss of the port propeller. Vessel operator must make necessary repairs to port propulsion system to satisfaction of USCG.

Must replace gasket on watertight hatch to lazeretted space.

Vessels on limited coastwise routes are required to carry six (06) red and six (06) orange flares or twelve (12) red flares. Three (03) red flares are expired by two months at the time of the inspection. Must replace expired flares with current approved flares.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Hydrostatic release for EPIRB expired. Install new hydrostatic release.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent extent necessary to determine that they are in suitable condition and fit for the service intended.

Machinery and fuel tank spaces must be separated by boundaries that prevent the passage of vapors. The portside has a hole in the bulkhead to the outside that is not a ventilation openings, the penetration is not vapor tight. Provide a proper seal to prevent loss of FM 200 upon future activation.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. 15 life jackets had faded retro tape. New approved retro tape must be installed to life jackets. Reduced passenger count by 15.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The port side engine had one rusted, bursting, cracking fuel hose. Must install new hose with fitting in accordance with 46 CFR 182.720

All vessels must have operable navigation lights in accordance with the Navigation Rules. No stern light. Provide navigation lights that are in compliance with the Navigation Rules. Due no later than 27 July 2022.

Fuel tanks and fittings must be so installed as to permit examination or removal for cleaning with minimum disturbance to the hull structure and fuel shutoff valves installed at the tank connection. Shut off valve at the tank shall be operable from outside the tank compartment, preferably from an accessible position on the weather deck. Fuel tank is not accessible for inspection and no fuel shutoff valve installed on the tank.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. The vessel's starboard side emergency fuel shut is inoperable. Please make permanent repairs to the starboard side emergency fuel shut.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The vessel's EPIRB has an expired hydrostatic release unit and needs to be replaced.

Connections must be made to battery terminals with permanent type connectors. Replace wing nuts on the battery terminals.

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. One of the three required child life jackets was removed due to the poor condition. The life jacket needs to be replaced.

A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds. The vessel allided with the pier and the impact broke the mount holding the steering ram. The vessel lost steering and had a ½ inch breech in the hull. The vessel needs to make permanent repairs to the steering mount and hull fracture.

Replace SOLAS tape on 03 adult and 02 child life jackets.

Provide ammonia inhalants in first aid kit.

Provide visual indicator on bilge high level alarm.
