All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] on port and starboard egress hatches from passenger accommodation include directions on how to properly operate the hatch in the event of an emergency. From inside the space it says "To Open Hatch Push Handle and Turn Clockwise." To open the hatch the handles must be turned counter-clockwise. Confirmed both labels are misleading with the owner and vessel master. Add proper verbiage clarifying counter clockwise operation as well as "EMERGENCY EXIT, KEEP CLEAR" as per regulation prior to overnight operations with passengers. 46 CFR 185.606

Each entrance to a space storing carbon dioxide cylinders, a space protected by CO2 systems, or any space into which CO2 might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. Machinery space entrance did not have CO2 warning signs. CO2 bottles are stored in the space and the space is protected by CO2. Place signs to satisfaction of Marine Inspector. 46 CFR 185.612(g)(1)&(2)

A vessel must have two independent means of controlling each propulsion engine. Control must be provided for the engine speed, direction of shaft rotation, and engine shutdown. This is an amended CG-835V stemming from Deficiency Item Number 2 issued 11/6/2021. Attended vessel to verify approved DVTP & QFA. Vessel was not able to successfully complete steps 3 & 4. When performing step #3, upon removing the actuator positive voltage cable from the ERIM, all systems respond as prescribed until trying to shift with the emergency back up controls. The back up system activates but the controls don't shift. Vessel can throttle but not shift. Vessel O/O has tested this with multiple ERIMs and they all respond exactly the same. Vessel provided successful underway testing for shifting manually at the gearboxes as back up. Vessel will need to coordinate with manufacturer and obtain correct DVTP/QFA for the installed system. A vessel must have two independent means of controlling each propulsion engine. Control must be provided for the engine speed, direction of shaft rotation, and engine shutdown. Immediately prior to the grounding on 05NOV21, the engines failed to go into gear when activated form the main pilothouse control and both wing control stations. Vessel underwent repower and installation of electronic engine controls in 2012, and QFA/DVTP verification was not conducted. In accordance with CVC Policy 17-07, submit the following for review to MSC: (1) A limited QFA of the vessel’s propulsion engine control system (throttle), documenting system compliance to CVC Policy 17-07. The limited QFA should only consider loss of power to the control system. (2) A propulsion control drawing should be submitted with an installation/operational test protocol. This test protocol is an abridged document intended to show compliance with 46 CFR.184.620. In addition, provide manufacturer's qualitative failure analysis showing failures which would lead to loss of control to engine transmission. Vessel may transit without passengers or cargo within the Sector LA/LB area to proceed to a repair facility.

A grease extraction hood required by 46 CFR 181.400 must meet UL 710 or other standard specified by the Commandant. A grease extraction hood must be equipped with a dry or wet chemical fire extinguishing system meeting the applicable sections of NFPA 17 or NFPA 17A or other standard specified by the Commandant. G-MSE-4 policy letter 1-99 allows for the use of the existing hood, but must meet the requirements: the vessel must establish a maintenance program for the existing hood, install a fire detection system for the galley, and add one additional 40-B:C fire extinguisher to the galley. Vessel's galley heat detector was not present and there is no maintenance program for the existing hood which was evident by the heavy accumulation of grease in the vent hood and in the vent above the ovens. Properly install heat detector and produce a maintenance program for the existing hood or properly install grease extraction hood to the satisfaction of an attending marine inspector prior to enrgizing galley heating equipment.

Repairs or alterations to the equipment that effect the safety of the vessel must not be made without the approval of the cognizant OCMI, to include electrical wiring and power supply systems. Vessel was found with numerous added electrical loads, including a NITROX generating system in the lazarette, food freezers in the lazarette, electric clothes dryer in the lazarette, a shower/bilge drain pump in the lazarette, a new electric double oven in the galley, additional engine room ventilation fans, a salon ice maker, additional salon refrigerator, water heater in shower room, reverse osmosis unit in engine room, and an electric grill on the main deck. Provide plans for approval showing electrical generating and distribution system as installed, to include materials used, over current protection details, and electrical load analysis.
