Safety orientation for crew members for their first voyage on a particular towing vessel and non-crew members must be completed and documented in the TVR in accordance with 46 CFR 140.410(d). The vessel did not have any records of a Safety Orientation for any crew members or non-crew members who recently embarked on the vessel. Properly document the safety orientation for all crew members and non-crew members in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021

Safety orientation for non-crew members who are getting underway with the vessel must be completed and documented in the TVR in accordance with 46 CFR 140.415. The vessel did not have any records of a Safety Orientation for non-crew members who recently embarked on the vessel. Properly document the safety orientation for all non-crew members getting underway on the vessel in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crew members are capable of performing the duties expected of them during emergencies and documented in the TVR in accordance with 46 CFR 140.420. The vessel did not have records of emergency drills and instructions properly documented in the TVR. Properly document emergency drills and crew instructions in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

Vessel is subject to bridge to bridge requirements per 33CFR26.03(a)3 and was unable to provide station logs as described by 47CFR80.409(f)(3). Ensure Radiotelephony record logs entries 47CFR80.409(f)(3) paragraphs (e)(1), (3), (5) (6), (7),(10), and (11) of this section. Properly maintain an FCC Radio Station Log to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

Each towing vessel must be fitted with a general alarm and installed in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light. Provide a supplemental flashing red light as part of the general alarm system suitable to alert crew members in the event of an emergency in the engine room in accordance with 46 CFR 143.235(b)(3) to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway in accordance with 46 CFR 140.635. The master was not properly documenting a navigational assessment that met all items as listed in the regulations. Ensure all elements of a Navigational Assessment are completed and the navigational assessment is documented in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021

Prior to assuming duties related to the safe operation of a towing vessel, each crew member must receive training to ensure that they are familiar with navigational safety terms and duties in accordance with 46 CFR 140.645. There is no record in the TVR that documents navigation safety training for the crew. Properly conduct and document the navigation safety training for each crew member and record in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to to May 28, 2021.

Each master must record the oil residue discharges and disposals from the vessel in accordance with 46 CFR 140.655. There is no record in the TVR of the discharge or disposal of any oil residue from the vessel. Ensure all oil disposals from the vessel and any accidental discharge from the vessel is captured in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

The owner, master, or operator of each vessel towing alongside or pushing ahead shall ensure that the face wires, spring lines, and push gear are frequently inspected and remain in serviceable condition In accordance with 33 CFR 164.76. The results of the towing gear inspection must be captured in the TVR in accordance with 46 CFR 140.820(a). Conduct an inspection of all towing and terminal gear and capture the results including the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations of that inspection in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021

All portable fire extinguishers, semi-portable fire-extinguishing systems, fire-detection systems, and fixed fire-extinguishing systems, including ventilation, machinery shutdowns, and fixed fire-extinguishing system pressure-operated dampers on board the vessel, must be inspected or tested at least once every 12 months in accordance with 46 CFR 142.240. Properly document all the tests and inspections for all firefighting appliances in the TVR to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021

All crew members must be provided with health and safety information and training in accordance with 46 CFR 140.515. Once the health and safety plan is developed, ensure all crew members are familiar with and have been provided training with the information in the health and safety plan and the training is documented in the company records to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021

The records in the TVR must be kept available for review by the Coast Guard upon request. Records, unless required to be maintained for a longer period by statute or other federal regulation, must be retained for at least 1 year after the date of the latest entry. The vessel was unable to produce records when requested. Provide Safety Orientation, Personnel Records, Examinations and Tests, Navigational Assessment, Towing an Terminal Gear Maintenance and Inspection, Fire Fighting Annual Instruction and Fire Fighting Inspection and Maintenance Records to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

Each towing vessel must have a fire-detection system installed to detect engine room fires and must identify the zone or zones of origin of the fire in accordance with 46 CFR 142.330. Identify and label all zones protected by the installed Fire Detection System so the person on watch can readily identify the location of the fire to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021

Electrical equipment and electrical cables must be suitably protected from wet and corrosive environments in accordance with 46 CFR 143.400(f). Bilge pump wire connections were attached by wire nuts and exposed to a wet environment. Repair or properly affix the deck lighting on the aft deck of the vessel to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

Each towing vessel of at least 26 feet, but less than 79 feet, in length must carry a minimum of two lifebuoys and each must be fitted with a floating electric water light approved under approval series 161.010 or 161.110 as required in accordance with 46 CFR 141.360(c)(2). Make repairs to the inoperable floating electric water light and demonstrate proper operation of both to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

All flammable or combustible products on board a towing vessel must be stored in a designated storage room or cabinet approved by either UL 1275 or FM approval standard 6050 when not in use. The vessel has a five gallon gas can deck that is used for fuel in way of the portable fire pump. Properly store all flammable and combustible products in a designated storage room or cabinet when not in use in accordance with 46 CFR 142.225. In addition provide one additional 40: B portable fire extinguisher located near the space all to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

Fuel systems for towing vessel main engines and generators must have a documented maintenance plan to ensure proper operation of the system in accordance with 46 CFR 143.255. Provide a documented maintenance plan for all fuel systems for the main engines and generators to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

Each Towing Vessel must have an installed or portable bilge pump for emergency dewatering in accordance with 46 CFR 143.275. The vessel has a an electric submersible pump that is not secured to the bilge in the machinery space. Properly secure the electric pump to the bilge to the satisfaction of an attending Coast Guard Marine Inspector prior to May 28, 2021.

Each towing vessel must have a fixed or portable fire pump. Crew was unable to start the portable pump. Prove proper operation of the fire pump.

Each towing vessel must have an alarm at the operating station to indicate low hydraulic steering fluid levels. No such alarm exists. Provide proper alarm at the operating station.

All lifejacket containers must be properly marked. The operating station lifejacket storage was not labeled. Properly label container.

Closure devices must be provided for hull penetrations. The wire run into the engine room on the forward side was not properly secured. Pack the cable penetrations.
