Operator installed a venrilation system with ~5FT of ~3" PVC piping on STBD side in fwd compartment without plan review. Fan was spliced into an extension cord to a 110VAC outlet. Vent fan was mounted in fwd bulkhead of Engineroom preventing it from being vapor tight. Remove all unapproved vent piping/fans; seal holes.
