On all passenger ships each lifejacket shall be fitted with a light complying with the requirements of paragraph 2.2.3 of the Code. PSCOs identified 9 lifejacket lights inoperable. 74 SOLAS 20 III/22.3

Where "A" class divisions are penetrated for the passage of electric cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made to ensure that the fire resistance is not impaired. PSCO observed non-approved material in the cable run penetration of the A-60 bulkhead above FSD 1.6.08 and above FSD 1.6.11. 74 SOLAS 01 II-2/18.1.1

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed an air compressor in the garbage room that had a portable fan permanently installed next to it for cooling as the air compressor commonly overheats and trips off, presenting an electrical hazard. 74 SOLAS 04 II-1/45.1.3

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces at least one of which shall give access to a stairway forming a vertical escape. PSCO observed a crew breakroom (space category 7) within the deck 9 pantry near fire station 9.2.6 (space category 13) without a safe escape route to a stairway, nor an acceptable means to notify crew normally employed in the space of a fire in the surrounding space. 74 SOLAS 01 II-2/28.1.2

Every ship of 100 gross tonnage shall carry a garbage management plan which the crew shall follow. The plan shall provide procedures of disposing of garbage including the use of the equipment on board. It was self-reported the shipboard incinerator required by the garbage management plan is inoperable due to failure of second chamber burner flange gasket.

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above hte deck at all points of the secape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the secape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. Category 3 corridor (lobby near service lifts 7 and 8, near maitre'd office) is missing low location lighting/tape. 74 SOLAS 01 II-2/28.1.10

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Assigned crew for lifeboat 8 unfamiliar with operation of emergency tiller. 74 SOLAS 20 III/19.2.1

Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: A door designed to re-open upon contacting an object in its path shall re-open sufficiently to allow a clear passage of at least 0.75m but no more than 1m. PSCO observed F.S.D.8.3.09 re-opened upon contacting the crash bar more than 1 meter. 74 SOLAS 01 II-2/30.4.11

All "A" class doors located in stairways, public spaces, and main vertical zone bulkheads in escape routes shall be equipped with a self-closing hose port of material, construction and fire resistance which is equivalent to the door into which it is fitted, and shall be a 150mm square clear opening with the door closed and shall be inset into the lower edge of the door, opposite the door hinges or, in the case of sliding doors, nearest the opening. PSCO's observed fire doors F.S.D.A.6.01 and F.S.D.A.4.20 with hose ports that did not fully close when the fire doors were at the closed position. 74 SOLAS 01 II-2/30.7

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. PSCO observed combustible material stored in Category (10) Spaces F.S.D.5.6.02 and F.S.D.4.6.02. 74 SOLAS 01 II-2/26.2.2

Within the perimeter of such stairway enclosures, only public toilets, lockers of non-combustible material providing storage for non-hazardous safety equipment, and open information counters are permitted. PSCO observed combustible storage underneath stairway leading outside the Pharaoh Theater. 74 SOLAS 01 II-2/29.2

Fire doors in main vertical zone bulkheads, galley boundaries, and stairway enclosures, double-leaf doors equipped with a latch necessary to their fire integrity shall have a latch that is automatically activated by the operation of the doors when released by the system. PSCO observed double leafed F.S.D.9.2.11 not properly close. 74 SOLAS 01 II-2/30.4.12

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO's observed in lifeboat #18 the starter battery was non-operational. 74 SOLAS 20 III/20.2

The means of control of any fixed gas fire-extinguishing system shall be readily accessible and simple to operate and shall be grouped together in as few locations as possible at positions not likely to be cut off by a fire in a protected space. At each location there shall be clear instructions relating to the operation of the system having regard to the safety of personnel. PSCO observed CO2 operating instructions in the main CO2 space not clearly correlating to the release components. 74 SOLAS (01) II-2/5.1.7

Within the perimeter of stairway enclosures, only public toilets, lockers of non-combustible material providing storage for safety equipment and open information counters are permitted. PSCO observed ATM cash machine fitted in stairway 160, deck 2 forward starboard side. 74 SOLAS (01) II-2/29.2

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement. PSCO observed stairway fire door B.4.04 close in 3 seconds. PSCO observed main vertical zone fire door 5.4.01 close in more than 40 seconds. 74 SOLAS (01) II-2/30.4.2

The Load Line ring, lines and letters shall be painted white or yellow on a dark background or in black on a light background. They shall also be permanently marked. PSCO observed deck line not painted in contrasting colors to the hull.

A self-contained emergency source of electrical power shall be provided to supply all those services that are essential for safety in an emergency, including emergency lighting at every muster and embarkation station. PSCO observed emergency light at launch station between lifeboats 6 and 8 not functioning. 74 SOLAS (04) II-1/42.2.1

Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: The doors shall be self-closing and be capable of closing against an angle of inclination of up to 35 degrees opposing closure. PSCO observed double leaf hinged fire door A.3.27 in MVZ boundary not latch completely closed. 74 SOLAS (01) II-2/30.4.1

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Crew assigned to lifeboat 6 was unfamiliar with boat starting arrangements. Crew assigned to lifeboat 16 was unfamiliar with location of fishing kit and radar reflector. SOLAS 74 (20) III/19.2.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on boars. PSCOs found storage of non-combustibles exceeding marked height restrictions in various spaces including main laundry, art auction locker, hospital space, and housekeeping stores. 74 SOLAS 29 I/11(a)

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Low location lighting was extinguished in MVZ 4, Deck 3 near FSD 3.4.07. Low location lighting on Deck 7 near FSSD 7.2.10 was cracked and exposed. 74 SOLAS 01 II-2/28.1.5.10

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Tender #3 could not secure power to the port engine. Crew used fuel shut off valve to stop diesel engine and lifeboat remained inoperable throughout the exam. 74 SOLAS 20 III/20.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCOs observed excessive water leak from AC drain line in overhead pipe in crew accommodation space Deck C, MVZ 5. 74 SOLAS 20 I/11(a)

Two means of escape shall be provided from each machinery space. Machinery space escape ladder was blocked by luggage carts and other miscellaneous material in the stbd tender embarkation area. 74 SOLAS 01 II-2/28.3.1

Fire doors in main vertical zone bulkheads, shall be self-closing, and a door designed to reopen upon contacting an object in its path shall reopen not more than one meter from the point of contact. PSCO observed FSD 3.4.01 open beyond one meter upon contact, and did not continue to close as designed. PSCO also observed FSD 2.5.09 continue closing upon contact with an object although it is designed to reopen closing upon contact with an object although it is designed to reopen closing upon contact with an object although it is designed to reopen upon contact. 74 SOLAS 01 II-2/30.4

The means of control of any fixed gas fire-extinguishing system shall be readily accessible. PSCO observed storage of galley items completely blocking fire door (No. 9.3.16) leading to CO2 controls for the aft galley. 74 SOLAS 01 II-2/5

Each power-operated sliding watertight door shall be provided with an audible alarm, distinct from any other alarm in the area, which will sound whenever the door is closed remotely by power. PSCO observed at Frame 166, WTD 5 does not alarm & doesn't fully seal. 74 SOLAS 04 II-1/15.7.1.6

The activation of any detector or manually operated call point shall initiate a visible and audible fire signal at the control panel indicating units. PSCO observed call point number 5617.1032, above hood #2, in the deck 10 aft galley did not initiate a visible or audible alarm. 74 SOLAS 01 II-2/13

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. PSCO observed emergency egress from cabins, missile hazards, and electrical shock and safety concerns. 74 SOLAS 20 IX/3.1 ISM Code 10.1

Before being assigned to shipboard duties, all persons employed or engaged on a seagoing ship shall receive approved familiarization training and be able to: communicate with other persons on board on elementary safety matters, know what to do if a person falls overboard, fire or smoke is detected, or the fire or anandon ship alarm is sounded; identify muster and embarkation stations and emergency escape routes. PSCO observed several stairwell guides and a hotel services crew member that were unable to communicate what to do in the event of fire or abandoning ship. STCW V/2.2, A-VI/I.1

Fire doors in galley boundaries shall be capable of remote release from the continuously manned central control station, and shall be capable of release from a position at both sides of the door. PSCO observed the magnetic hold back for door # 9.2.10 would not hold the door, preventing remote release of the door. 74 SOLAS 01 II-2/30

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazard will be ensured. PSCO observed dead-end wires in the electrical locker of the LIDO galley starboard side forward of the ship. 74 SOLAS 04 II-1/40.1.3

Where electrical cables are carried through watertight bulkheads, arrangements shall be made to ensure watertight integrity of bulkheads. PSCO observed piping improperly installed in electrical bulkhead penetration at central fresh water near chemical storage station in the engine room. 74 SOLAS (01) II-1/15.2.1

The bulkhead deck or a deck above it shall be weathertight. The PSCO observed latches on weathertight doors leading from passenger cabins to balconies were inoperable, preventing doors to be closed in cabins 7271, 7259, 7295, 6228, 6218, 6210, 6194, 6178, 6176, 6140. 74 SOLAS 04 II-1/20.1

Lifejackets shall be so placed as to be readily accessible and their position plainly indicated. PSCO noted that the placard to mark location of PFDs in passenger cabin 8123 was missing. 74 SOLAS 20/III.7.2.2

All "A" class doors located in stairways, public spaces, main vertical zone bulkheads in escape routes shall be equipped with a self-closing hose port. PSCO observed hose port on door #6404 could not close all the way. 74 SOLAS 01 II-2/30.7

Ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements. PSCO observed a soft patch to the grey water system in the starboard side forward overhead of the engine room. 74 SOLAS (04)/II-1.3-1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed numerous passenger cabins on multiple decks with excessive gear adrift (missinglight fixtures, shower heads not installed, spare glass railing stored in rooms, open ceiling tiles, exposed electrical sockets, furniture adrift); presenting unsafe conditions for carriage of passengers. PSCO noted storage closet with mattresses stacked to the ceiling in excess of storage limits. 74 SOLAS 20 I/11(a)

Handrails and other handholds shall be of such strength as to withhold a distributed horizontal and vertical load. PSCO observed the gate pins seized in the up position on the handrails of the embarkation deck, presenting an unsafe condition for crew and passengers. SOLAS 74 (01): II-2/13.7.3.1

Machinery spaces of category A shall be adequately ventilated so as to ensure that machinery is operating at full power in all weather conditions. PSCO observed the EDG louver system is not functioning as designed. The interior louver solenoid system is not functioning. 74 SOLAS (04)II-1/35

The bulkhead deck or a deck above it shall be weathertight. On decks 5, 6, & 7 multiple doors from passenger cabins to balconies were missing gaskets. 74 SOLAS 04 II-1/20.2

Electrical installations shall be such that the safety of the passengers, crew and ship from electrical hazards will be ensured. PSCO observed a portable electric vacuum pump (440V) hardwired into an electrical controller in the aft sewage space. 74 SOLAS 04 II-1/40.3

Indication shall be provided at the fire door indicator panel in the continuously manned central control station whether each of the remote-released doors are closed. Indications for double leaf doors 10.5.01 and 10.5.02 are incorrect at bridge. SOLAS 01 II-2/30.4.6

Fire doors designed to reopen upon contacting an object in its path shall reopen not more than 1m from the point of contact. Fire door 10.3.01 does not reopen upon contact. SOLAS 01 II-2/3.4.11

Within the perimeter of such stairway enclosures, only public toilets, lockers or non-combustible material providing storage for safety equipment and open information counters are permitted. Category 2 space in vicinity of FSD A.5.11 is being used for storage of unauthorized items. SOLAS 01 II-2/30.4.2

The approximate time of closure for hinged fire doors shall be no more than 40 seconds. Fire doors 6.3.08 and 10.3.02 failed to close within allotted time after contact with bumper. SOLAS 01 II-2/30.4.2

Vessel to make permanent repairs to damaged hull area in way of dry tank No. 1 at port side transom to the satisfaction of class and USCG. Temporary repairs approved under the following conditions: 1. Dry tank No. 1 sounded and logged by ship’s crew on a daily basis and damaged hull area externally inspected by ship's crew at each port call until external doubler plate installed see notes

#5 Lifeboat door window was found cracked during inspection. Repair window to the satisfaction of class surveyor within 30 days.

Main theater back stage portside exit stairs blocked (with props). Means of escape not available including FSD 2.6.01 portside (not normally manned) according to Master.

Hand phone use in an emergency on deck A around elevators portside, not working (No Dial Tone).

#18 lifeboat sustained minor hull damage on the port side. Repair damage to the satisfaction of class within 30 days.

Update Structural Fire Protection Plan to indicate all Category 10 spaces that were changed into Category 13 spaces.

Replace combustible partitions in Multi-purpose room on deck 10 and in Children's Playroom on deck 5 with non-combustible, non-locking partitions.

Properly label PFD storage lockers on embarkation deck and throughout to indicate number of lifejackets stored inside.
