Vessel's fresh water hose penetrated the lazarette/engine room bulkhead and the engine room/# 2 space compartment bulkhead on the port side and penetrations were not watertight. Various electrical cables and wiring penetrations were also found on both port and starboard side of the lazarette and engine room bulkhead that were not watertight. Vessel has old/unused generator piping that penetrates the lazarette/engine room bulkhead in the center of bulkhead that is not watertight. Make all penetrations watertight. 46 CFR 171.114

Life jackets are stowed in one centralized location in a forward compartment of the accommodation space located within the head. Passengers on weather deck would need to open three doors to access them. Prepare a proposal in order to have life jackets in an arrangement spread throughout the accommodation spaces and readily available and submit to USCG for review. 46 CFR 180.78.

Each hatch cover must have a securing device; and be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Engine room and laz space hatches do not have securing devices to maintain watertight integrity; hatches are not attached to the vessel. Alternatively, prove watertightness of current arrangement by a 30PSI hose test though a nozzle of not less than .5". 46 CFR 171.214.
