A periodic examination made in accordance with the requirements of this subpart shall include the underwater hull and appendages. propellers, shafting, stern bearings, rudders. through hull fittings, sea valves and strainers~ and shall be of sufficient scope to determine that these items are in a satisfactory condition for the service intended. As discussed with the vessel master present there are 5 keel, 2 stern, 6 starboard side and 7 port side hull sections to be repaired. 46 CFR 176.15-5

The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84), so as to be visible at the following minimum ranges: (b) In a vessel of 12 meters or more in length but less than 50 meters in length: (i) A masthead light, where the length of the vessel is less than 20 meters, 3 miles; (v) A white, all-round light, 2 miles; Vessel anchor light and mast head light were found not working. Management Information System (MIS) requirements - A consortium or other employer representative may submit data for a marine employer. Reports may contain data for more than one marine employer. Each report, however, must list the marine employers included in the report.(4) Marine employers must ensure that data submitted by a consortium or other employer representative under paragraph (b)(3) of this section is correct. MIS report was not available for review.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements:(1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Vessel Captain did not have a FCC Marine Radio Operator Permit. 47 CFR 80.159(e)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First Aid Kit missing required equipment.

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Container for distress flares not properly marked.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 1 inch numbers and letters at the entranceway to that deck. Passengers permitted on 2nd deck missing.

Exhaust gas must not leak from the piping or any connections. Inspector observed salt build-up on exhaust piping from water seeping through fiberglass piping for both engines in steering space.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. Fuel tank vent pipe flame screen missing.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Fuel shut off valves missing markings indicating the purpose of the valve and the direction of operation.

Waste receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles are constructed of combustible plastic material.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly on each container for life jackets. Letter and numbers must be 4" high. Life jacket compartment at operating station missing required markings.

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of §111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. No flexible cord may be used except as allowed under Sections 400-7 and 400-8 of NFPA NEC 2002. Unless specifically permitted in 400.10, flexible cables, flexible cord sets, and power supply cords shall not be used for the following: (1) As a substitute for the fixed wiring of a structure. Permanently installed extension cords are being used on second deck.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. All engine belts were found to be loose; accessories could be turned by hand.

Batteries... shall be effectively screened by a cage or similar structure to minimize the danger of accidental spark through dropping a metal object across terminals. Spare batteries are being stowed on the deck of the engine room with no protective covering. 46 CFR 183.05-20 OLD T

Vital Systems - (b) Mechanically propelled ''S" vessels which are ferry vessels, or which carry more than 49 passengers and mechanically propelled "L” vessels shall comply, with the provisions of' Subchapter F of this chapter. (ii) Where fire endurance is required in Appendix 4 of IMO Resolution A.753(18) the pipe must, at a minimum, be approved as meeting the fire endurance level required in Appendix 4. Ratings of “0” in Appendix 4 indicate that no fire endurance test is required. Ratings of “N/A” or “X” indicate that plastic pipe is not permitted. Inspector observed that a new portion of the fire main system was installed utilizing PVC pipe. 46 CFR 182.40-5 Old T

Gages to indicate cooling water discharge temperature and lubricating, oil pressure shall be provided t:or all propulsion engines and located so as to be in view of the operator. Oil pressure gage for port engine not operating. 46 CFR 182.15-5 Old T

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Not all crew are participating in drills every three months.

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. Log of drills do not provide a description of the drill scenario and training topics.

The construction and arrangement of vessel must allow for the safe operation of the vessel IAW terms of the COI giving consideration to provisions for ... necessary facilities for passengers and crew. The vessel reported damage to approximately three feet of the starboard quarter railing due to a allision with another vessels anchor. Complete repairs to the satisfaction of the attending Marine Inspector.

46 CFR 180.71: An adult life jacket must be provided for each person carried on board a vessel. Vessel is not equipped with the number required on the COI. Provide required approved type of life jackets

46 CFR 185.604(h) - Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). A large number of life jackets have wasted retro reflective markings. Repair.or replace as required.

46 CFR 185.604(b) Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Ring life buoys missing on name by operating station. Make with name as required.

46 CFR 185.604(I) - Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Ring life buoys by operating station missing retro reflective markings. Install as required.

46 CFR 185.420 (a) - The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by § 185.510 of this part and, when applicable, the duties listed in the station bill required by § 185.514 of this part. Crew has not documented required training for 9 months. Crew also failed to demonstrate fire drill. Conduct training and document as required.

46 CFR 120.200: Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment,electrical shock. Remove all extension cords, frayed wires to the satisfacton of CG Marine Inspector.

46 CFR § 185.524 (d)Fire fighting drills and training shall be logged orotherwise documented for review by the Coast Guard upon request. Master shall ensure drills are being logged IAW CFR.

46 CFR § 185.728 (f)Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. When any vessel is drydocked, examination shall be made of the propeller, stern bushing, sea connection, and fastenings if deemed necessary by the marine inspector... Replace rudder bearings and both cutlass bearings on the port side. 46 CFR § 61.20-5

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull... Repair FRP damage to the bow. Below the waterline, re-fiberglass the two areas on the port side forward and one area on the forward portion of the keel. 46 CFR § 177.100

The weather deck on a well deck vessel must be watertight. Topside on the main deck is not watertight, repair beam to beam the delaminated FRP by the bow to good material by the house and other soft spots on the deck. 46 CFR § 178.430 (a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull... Apply new metal support brackets on the forward overhead transverse longitudinal beams in void #1 (most forward compartment). Replace the cracked overhead transverse frame and apply new fasteners in void #1. Replace rotted/cracked frame in lazarette on the starboard side and re-tab where it has delaminated. 46 CFR § 177.100

This part contains requirements for the design, construction, installation, and operation of propulsion and auxiliary machinery, piping and pressure systems, steering apparatus, and associated safety systems. Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. replace the portion of the leaking pipe in void #2 (storage room) which has a leaking hydraulic winch line. 46 CFR § 182.100

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Reweld the cracked ladder from the main deck to second level. 46 CFR § 177.100

A suitable marine type strainer shall be fitted in the fuel supply line. Fit fuel filters of marine type. 46 CFR 182.20-40(b)(5)

The minimum capacity of the poower pump shall be a pressure of not less than 60 pounds per square inch at the pump outlet. Renew pump outlet fittings to correct loss of pressure due to fitting leak/rupture. 46 CFR 181.10(c)

Rails shall consist of evenly spaced courses... between courses shall not be greater than 12 in except as provided by paragraph (f). Renew stern railing. 46 CFR 177.35(b)

Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems. Generator water pump is leaking excessively. Repair or replace to the satisfaction of CG Marine Inspector.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail.

A vessel must undergo a drydock and internal structure examination at least once every two years. Last drydock and internal examination was conducted on 05FEB2020 and vessel is not permitted to operate until a drydock and internal structure inspection has been completed.

To maintain a valid Certificate of Inspection, you must complete your annual inspection within the periods specified in 176.500(b)(1) [annual inspection within 3 months before or after each anniversary date] and your Certificate of Inspection must be endorsed. Vessel is outside of three month window for Inspection and is not permitted to operate until annual inspection has been completed. 46CFR176.502

The name of the vessel shall be permanently marked on all ring buoys. Mark new name of vessel on all ring buoys.

Moving parts (belts) require guards on generators and engines. Aux generator missing guard around the belts.Install a cover or guard over moving belts.
