LADY VIRGINIA II

A drydock examination conducted in compliance with § 176.600 must be conducted while the vessel is hauled out of the water or placed in a drydock or slipway...Sea chests, sea valves, and sea strainers must be opened for examination. Sea valve for generator cooling water intake was unable to be tested due to the pipe breaking while trying to access the valve. Operator is to repair/replace the pipe and valve prior to credit dry dock.

During the examination all accessible parts of the vessel's underwater body and all through hull fittings, including the hull plating and planking, appendages, propellers, shafts, bearings, rudders, sea chests, sea valves, and sea strainers shall be made available for examination. Operator had removed the propellers for routine maintenance prior to the inspection. Operator is to reinstall the propellers for inspection prior to credit dry dock.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date installed. Hydrostatic release expired Sept 2025. Limited to 3 miles. Corrected prior to end of inspection.

All vessels must have navigation lights in compliance with applicable sections of the international and inland rules. Inspector found starboard sidelight extinguished/inoperative. Operation restricted to actual or forecast visibility of 2nm.

Each disposable hydrostatic release must be marked with an expiration date of two years after the date installed. HSR marked April 2024.

A vessel must comply with applicable radio requirements of Federal Communications Commission as set forth in 47 CFR part 80. Limited to 20 miles as vessel does not have a radio meeting the requirements of this part for operation beyond 20 miles.

At each inspection owner must have fire protection system ready for inspection. Pre engineered system manufactured 1/11 and has not undergone hydrostatic or visual in lieu of hydrostatic in accordance with MI notice 02-12 provided to operator.

A vessel must comply with applicable requirements for any radio installations. FCC safety radio certificate expired July 2023.

At each inspection all ovserved unsafe practices, fire hazards, and other hazardous conditions must be corrected. Tank space unable to be inspected due to high carbon monoxide and hydrogen sulfide levels.

Discharge ends of fuel tank vents must be fitted with removable flame screens of 30 x 30 mesh. Port and starboard vents do not have screens.

A vessel must carry a first aid kit that meets 46 CFR 199.175(b)(10). Vessel kit contains multiple expired items.

Each flare carried must be stowed in a portable watertight container, of a bright color and marked "DISTRESS SIGNALS" in legible contrasiting letters at least .5 inches high. Distress signals were stored in a non watertight tool case.

The certificate of inspection must be posted under glass or other suitable transparent material, such that all pages are visible in a place likely to be seen by passengers. COI was not posted.

Each survival craft must be secured to the vessel by a painter with a float free link. Vessel survival craft hydrostatic release was improperly installed and not secured to the vessel. Corrected by end of inspection.

A vessel must carry adequate and up to date charts of large enough scale to make safe navigation possible; vessel was unable to show that the elecronic charts being used were updated.

A vessel must comply with the applicable requirements for any radio as set forth in 47 CFR part 80. VHF radio onboard vessel is programmed with an MMSI number that does not belong to this vessel.

Each small battery installation must be protected from falling objects. Two battery boxes located under the operating station do not have covers.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew and the vessel from electrical hazards including fire and electric shock. Port side electrical shore tie is not longer used, connections are cut inside the vessel and are exposed. Stern shore tie connection is burned inside. Shore tie cable from dock has plug that is not properly installed and is taped. The electrical arrangement behind the operating station switchboard has 1) wires that are worn/aged with exposed conductors 2) butt connected splices 3) damaged/corroded current transformer 4) terminal lugs improperly installed 5) dead ended conductors

A pre-engineered fixed gas fire extinguishing system must: Automatically shut down...all engines that draw intake air from within the protected space; and A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge [and] An audible alarm that sounds upon discharge. LADY VIRGINIA II Fireboy system does not shut down generator, and does not alarm upon simulated discharge. Make proper repairs and test to satisfaction of [name withheld] OF PASSENGERS.

The [fuel] shutoff valve at the tank shall be operable from outside the tank compartment. On LADY VIRGINIA II, there is currently no fuel shutoff valve at the tank for the generator fuel line that is operable from outside the tank compartment. Make modifications as necessary to ensure the system fully meets this requirement PRIOR TO CARRIAGE OF PASSENGERS. 46 CFR 182.20-40(b)(3)

Each vessel that operates on the high seas...must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. On LADY VIRGINIA II, the EPIRB battery is expired. VESSEL LIMITED TO 3 NM FROM SHORE UNTIL CORRECTED.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. LADY VIRGINIA II currently has no operable MF-DSC/SSB radio installation. VESSEL RESTRICTED TO 20 NM FROM LAND UNTIL CORRECTED.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First-aid kit on LADY VIRGINIA II is missing multiple required items. PROVIDE PROPER APPROVED FIRST AID KIT NLT 07OCT21.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80.Operator of LADY VIRGINIA II was unable to produce a valid FCC Station License. PROVIDE NLT 07OCT21.

A suitable marine type strainer shall be fitted in the fuel supply line in the engine compartment. Drip pans fitted with flame screens may be required under fuel strainers other than those mounted on the engine. On LADY VIRGINIA II, the port side RACOR filter is made from a material other than steel and is not fitted with a proper flame impingement bowl. All vessels shall be provided with not less than two avenues of escape from all general areas...where the crew may be...normally employed. On the LADY VIRGINIA II, the E/R does have two hatches that open to the main deck; however, the second means of escape does not meet the requirements of this cite, including suitable means to escape, and marking requirements. Discussed and provided requirements to vessel rep. ENSURE BOTH MEANS OF ESCAPE FROM THE E/R MEET THE REQUIREMENTS OF THIS CITE NLT 07OCT21. 46 CFR 177.15-1 46 CFR 185.606

Each hatch exposed to the weather must be watertight and...be attached to the hatch frame or coaming by hinges, captive chains, or other devices to prevent its loss. On LADY VIRGINIA II, the forepeak hatch is broken and gasket/captive device not installed. RENEW OR REPLACE HATCH TO MEET FULL REQUIREMENT NLT 07OCT21. 46 CFR 171.124

A voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator that is in operation. For alternating-current generators a means for measuring frequency shall also be provided. LADY VIRGINIA II currently is not fitted with means to measure these operating parameters. INSTALL REQUIRED METERS NLT 07OCT21. 46 CFR 183.10-5(g)

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. On LADY VIRGINIA II, the portable extinguisher at the operating station has lapsed on required tests and inspections. PROVIDE EVIDENCE OF PROPER INSPECTION/SERVICING NLT 07OCT21.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Operator of LADY VIRGINIA II could not produce logs of any required drills. Drills conducted as part of the inspection; operator instructed to log these and begin conducting periodic drills and logging them as required. CORRECTED ON THE SPOT. 46 CFR 185.520 46 CFR 185.524

LADY VIRGINIA II Operator not able to produce records/logs of any of the required maintenance, testing, and inspection of the lifesaving equipment required by 46 CFR 185 SUBPART G, including EPIRB. Operator instructed to begin conducting and recording these required activities immediately. CORRECTED ON THE SPOT. 46 CFR 185.700 THROUGH 46 CFR 185.740 AS APPLICABLE.

Provide additional lights for PFD's

Install Emergency Lighting in below-deck hold forward of the engine room. Emergency Lighting must turn on automatically upon failure of main lighting system. Passengers are prohibited from accessing the space until this is installed. 46 CFR "Old T" 184.30-5

Passenger overnight accommodations in the below deck hold forward of the engine room substantially do not meet construction requirements in 46 CFR "Old T". Area must be modified prior to being used as overnight passenger accommodations. Submit plans to OCMI prior to beginning any modifications.
