Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Receptacle was plastic.

The Certificate of Inspection certificates must be posted under glass or other suitable transparent material, such that all pages are visible. Original Certificate of Inspection was not available.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Drywall anchors were found to be used in bulkhead to mount a float switch.

At each inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances. Two transverse frames in lazarette on the stern had cracks in welds.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] repairs on bow to be conducted and witnessed by a competent marine inspector. 46 CFR 176.700

Whenever there is a hazardous condition, as defined by § 175.400 of this subchapter, on board the vessel, the owner, master, agent, or person in charge shall immediately notify the Captain of the Port. O/O failed to immediately notify the COTP of hazardous condition after allision with pier and did not conduct damage assessment immediately, then proceeded to sail with passengers on board. 46 CFR 185.203

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. While operating with passengers, the vessel ran aground causing damage to the starboard propeller. Vessel to drydock and make vessel available for a drydock inspection.

Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. The starboard vent-line valve in the lazarette was found inoperable. 46 CFR 179.350(c)

Each lighting branch circuit must be protected against overcurrent either by fuses or circuit breakers rated at not more than 30 amperes. Lighting branch circuit breaker within bridge switch board found inoperable. 46 CFR 183.380(g)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Examine and test by operation all watertight closures in the hull, decks and bulkheads. During the course of our inspection, cracks of approximately 5 inches and 3 inches were observed on the hatch ring to the forward void compartment. 46 CFR 176.25-5(a)(2)

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present. One penetration on the aft port side bulkhead separating the fuel tank compartment and the engine compartment was observed to not be vapor tight. 46 CFR 177.405(e)

Glass and other glazing material used in windows accessible to passengers and crew must be of material that will not break into dangerous fragments if fractured. Two windows in the passenger space were observed to be broken. 46 CFR 177.1010

Inspect all railings and bulwarks and their attachment to the hull structure. The port stanchion, second one forward from the stern, was observed to be completely parted at the weld. 46 CFR 176.25-5(a)(4)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Multiple medicines in the first-aid kit were found past their expiration date. 46 CFR 184.710

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. The expiration date for the batteries on two lifesaving water lights were passed their expiration dates. 46 CFR 185.726(d)

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel’s name. The orange life buoy located on the upper deck was not marked with the vessel’s name. 46 CFR 185.604(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale o make safe navigation possible. The navigation chart carried on board the vessel was dated Sept 23, 2020 and was not updated. 46 CFR 184.420 (a)(1)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The starboard engine is leaking engine coolant at the head/block on #6 cylinder.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck, or, if it is readily accessible under service conditions, a screw-down valve without a reach rod may be fitted to the bilge line on the after side of the collision bulkhead. Screw down valve corroded and inoperable from the weather deck.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Piping in tank room not given adequate support. Substitute wire ties for metal bracketing.

Each floating waterlight must be approved in accordance with §161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Per manufacturer's instruction, battery must be replaced annually and be of the screw cap terminal type.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Vessels sea chest for generator has a pinhole leak. 46CFR177.300

Prior to carrying passengers, fire pump must pump a sufficient stream necessarily to put out a fire. Vessel's current fire pump is only putting out 18 psi.

High water alarm at operating station in the Engine Room is audible but not visible.

FCC Station License is order but not on boat. Provide USCG a valid station license.

Replace expired medication in first aid kit.
