MI observed light guard missing on the main deck starboard side overhead. Replace/repair within 14 days. 46 CFR 183.410(a)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: SEE ON FOLLOWING PAGE OF 835... (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”. (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. (3) Spaces into which carbon dioxide might migrate—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEARBY SPACE CAN COLLECT HERE. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED VACATE IMMEDIATELY.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. A hatch cover must:(2) Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. MI observed hatches not secured by chain. Repair/replace within 14 days. 46 CFR 179.360(b)(2)

All vessels shall be provided with bilge pumps IAW table 182.25-10(a). Power driven bilge pump in-op & hand pump absent

Except on a vessel limited to daytime operations, the compass must be illuminated. Compass is not illuminated. Vessel restricted to daytime opperation.

Vessel must carry adequate and up to date charts. Update electronic charts or acquire up to date paper charts

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel didn't complete DDX/ISE inspection within the two year period. Inspection must be completed prior to carrying passengers. 46CFR176.600

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment under all practicable conditions. Vessel crew unable to drain water using bilge pump.

EACH OFFICER EMPLOYED MUST HAVE HIS LICENSE ONBAORD AND AVAILABLE FOR EXAMINATION AT ALL TIMES VESSEL IS OPERATING. MASTERS LICENSE NOT ONBOARD.

EACH PORTABLE FIRE EXTINGUISHER MUST BE OF AN APPROVED TYPE. TYPE ARRPOVAL INCLUDES BRACKET. FOUR FIRE EXTINGUSHERS ON MAIN DECK NOT BRACKETED.

PLACARDS CONTAING INSTRUCTIONS FOR THE DONNING AND USE OF LIFEJACKETS MUST BE POSTED IN CONSPICOUS PLACES. DONNING AND INSTRUCTIONS NEEDED ON MAIN AND UPPER DECK.

THE NUMBER AND IDENTIFICATION OF ITEMS STOWED INSIDE MUST BE MARKED FOR LIFEJACKET CONTAINERS. CHILD-SIZE LIFEJACKETRS NOT LABALED.

Vessel altered its fixed CO2 system and must install an odorizing unit.

The vessel is currently broadcasting (1) Improper Call Sign, (2) Improper Name

Vessel should not have exhaust gas leaking from the piping or any connections. Repair source of exhaust gas leak to the satisfaction of a coast guard marine inspector.(46CFR182.15-20(a))

Vessel was unable to perform man overboard drill due to unsafe condition developing in the machinery space. (46CFR176.808(g))

Vessel is required to have spanner wrenches at fire hose station. Provide spanner wrenches at fire hose station. (46CFR181.310(d))

STBD shaft coupling lost oil pressure and failed, leading to loss of propulsion on STBD engine. USCG MIs must witness satisfactory testing after technician completes maintenance and repairs.

Vessel's port propeller shaft seal is not sealing as required/intended. Provide technician's report from the manufacturer that the shaft seal has been installed correctly per the manufacturer's instructions to the satisfaction of the attending Coast Guard Marine Inspector.

When repairs are made during an emergency, the operator shall notify the OCMI as soon as practicable after such repairs are made. Vessel's port propeller shaft mechanical seal came loose while carrying passengers resulting in flooding of watertight compartment. Vessel may not carry passengers until satisfactory repairs are made to the satisfaction of the attending CGMI.

Bridge control failed to engage fwd/astern propulsion, vessel must have control of shaft rotation. Conduct repairs to satisfaction of attending CG MI.

Excessive oil in engine room bilge. Vessel is restricted from operating.

Hole in watertight hatch on bottom of portside of stairs is allowing water to enter into engine room. Vessel is restricted from operating.

Bilge in engine room had excessive oil. Clean to the satisfaction of the attending Marine Inspector.

Starboard engine alternator missing guard. Add guard.

Electrical wire in engine room were secured with zip ties. Secure with metal supports

Light bulb in space under pilot house was uncovered. Cover bulb with guard.

There were dead end wires in space below pilot house. Remove or cap dead end wires.

Electrical box in space below pilot house was not mounted against bulkhead. Secure electrical box.
