The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: penetrations in fire-resisting divisions. The fire dampers needing repairs as listed in the Global Fire and Safety report dated 23Feb25 were inoperable. Repair fire dampers and verify all fire doors were adjusted accordingly. SOLAS II-2 Reg 14.2.1.1.1

The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. On page 17, of Hiller System’s inspection report Job # VG1058, dtd 2/9/2023, 10% of the smoke detectors in Zones 20 and 25 were marked unsatisfactory. Amperage on the electrical cardholder for No. 22P4554, MIPCO #2, in Panel P445 (1) 460 Volt located in the Machine Shop does not match the amperage referred to on the breaker. Containers, brackets, racks, and other similar stowage locations for lifesaving equipment shall be marked with symbols in accordance with the recommendations of the Organization, indicating the devices stowed in that location for that purpose. Missing IMO reflective placards for ring buoys on main deck (port/starboard side).

Every oil fuel daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space. Reach rod for emergency diesel generator daily tank is broken and not capable of securing valve in an emergency from outside the space.

Measures shall be to the satisfaction of the Administration to ensure the equipment is functioning in a reliable manner and that the arrangements are made.for routine test to ensure continuous reliable operations. Vessel failed to demonstrate proper testing of vital machinery systems in accordance with approved testing procedures. Vessel's engineering staff required to review completed testing procedures to automation for years 2020/2021 and repeat tests not performed to approved procedures.

Containers, brackets, racks, and other similar stowage locations for life-saving equipment shall be marked with symbols in accordance with Resolution A.760(18), AM. indicating the devices stowed in that location. If more than one device is stowed in that location, the number of devices shall also be indicated. IMO markings are missing from all life saving stowage locations on the exterior of the vessel. Properly mark lifesaving stowage locations to the satisfaction of the RO.

An International Tonnage Certificate (1969) shall be issued to every ship, the gross and net tonnages of which have been determined in accordance with the present convention. Vessel must have a valid Tonnage Certificate meeting the requirements referenced in ANNEX II of the convention. Provide on board to the satisfaction of the Administration.

A general emergency alarm system complying with the Code, shall be provided. General alarm bell in Cargo hold #2 B-Deck 1 of 2, Cargo hold #3 F-deck 3 of 4, 63 foot level passageway outside sheet metal workshop and A-deck cross passageway outside of galley port door are inoperable. Repair/replace inoperable alarms within 30 days, but not later then departure from scheduled shipyard availability in Charleston, SC.

Survival craft launching appliances shall meet the Code. During launching of the port side aft 73 person survival craft/rescue boat, it was noted that the cable for launching control from within the craft bird-nested at the drum and the vessel did not lower all the way to the water.

46 CFR 113.25-9(b):General alarm (GA) signal must be audible in the spaces identified in paragraph (a) of thi sseciton with all normally closed doors and accesses closed. Repair GAs in the following locations/spaces listed below and prove proper operation of GAs to the attending USCG MI: cargo holds 2, 3 and 4, bottom flat in forward bow thruster room, sheet metal shop on Level 63, in passageway of EDG room, and near galley entrance.

Bow Thruster room forward near emergency fire pump has multiple dead-end wires. Make permanent repairs to the satisfaction of the attending ACP ACS.

On A-Deck: Junction box is missing screws and explosion proof light is missing plug, repair or replace with approved equipment. Make permanent repairs to the satisfaction of the attending ACP ACS.

Fore peak tank ventilation fan is not properly secured. Properly mount fan. Make permanent repairs to the satisfaction of the attending ACP ACS.

Pipe supports throughout cargo holds are missing, bent, or broken. Make permanent repairs to the satisfaction of the attending ACP ACS.

Retroreflective on life jackets is not approved. Make permanent repairs to the satisfaction of the attending ACP ACS.

Form E has multiple pen and ink changes that are not signed or endorsed. Attending ACP ACS to review and correct form.

Planned Maintenance Program onboard documentation is not available as required by ABS Rules Part 7.14.13.5. Provide required onboard documentation to the satisfaction of the attending ACP ACS.
