Vessels on limited coastwise routes are required to carry six (06) red and six (06) orange flares or twelve (12) red flares. Vessel had six (06) red flares at the time of the inspection. Must add six (06) more flares for vessel's route. Corrected on the spot.

Must properly mount magnetic compass at operator's station to the satisfaction of USCG.

Must replace Type I retro-reflective tape on three (03) life jackets to satisfaction of USCG.

Vessel operators must be prepared to conduct tests of all fire fighting equipment including fixed systems at each inspection for certification. Vessel Fire-Boy system failed to shut-down starboard inboard main engine when tested at vessel annual exam. Must rectify to the satisfaction of attending USCG Marine Inspector prior to carrying passengers.

Vessel operators must be prepared to have all lifesaving equipment inspected and tested at each inspection for certification. Flashing light on ring buoy not functioning when tested at the annual exam. Vessel is restricted to operate between the hours of sunrise and sunset until rectified to the satisfaction of attending USCG Marine Inspector.

Vessel must be equipped with adult sized life jacket for each person authorized on the Certificate of Inspection (COI). Current COI requires twenty-seven (27) adult jackets and there were twenty-four (24) counted at the time of inspection. The max adult passenger count is reduced to twenty-two (22) until operators provide more adult life jackets to satisfaction of USCG.

notwithstanding the number of passengers permitted by any of the criteria described in paragraph (b) of this section, the maximum number may be further limited by stability or subdivision considerations. Vessel reported to have exceeded number of passengers allowed on the forward box deck, COI states 06 people, vessel captain admitted 07 were on the deck while passengers admitted there were 10 people total standing holding onto the railing. Vessel rep must post a placard identifying the max passenger limits on the bow with seating requirement as listed on COI endorsement.

On passenger decks of vessels engages in a ferry or excursion type operation, rails shall be at least 42 inches high. The space below the rail shall be fitted with bulwarks, chain link fencing, wire mesh or equivalent. Vessel is fitted with 12" course openings, correct spacing issue meeting old or new T. Must add 4" rail courses at next drydock in April 2023.

Safety orientation. Before getting underway, the master of each vessel subject to this subpart shall ensure that suitable public announcements, instructive placards or both are provided in a manner which affords all passengers the opportunity to become acquainted with: (1) Storage location of life jackets (2) Proper method of donning and adjusting life preservers of the type(s) carried on the vessel (3) type and location of all life saving devices carried on the vessel and (4) the location and contents of the Emergency Checkoff List" required by 185.25-5. Vessel found operating without a safety orientation for passengers. Vessel rep to submit updated company safety orientation briefing to include passenger requirement sitting on the forward bow deck to [name withheld] is required to participate in safety orientation refresher training prior to the carriage of passengers.

All vessels must have operable navigation lights in accordance with the Navigation Rules. No stern light. Provide navigation stern light that is in compliance with the Navigation Rules.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vsl required to have a new stability test conducted due to vsl wind heeling moment being greater, extended engine compartment and increased weight due to engine replacements than the original SST conducted by sister vsl in 1985.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge. The light did not work.

Fuel tanks and fittings must be so installed as to permit examination, testing, or removal for cleaning with minimum disturbance to the hull structure. Fuel tanks are encapsulated in FRP and below deck with no accessibility to inspected or service.

Vessel needs to replace 03 deteriorated adult life jackets to fulfill the required amount for carriage IAW vessels COI. Vessel carriage restricted to no more than 24 passengers. 46 CFR 180.71

Vessel needs to have working automatic float switches for all spaces with bilge pumps installed. 46 CFR 182.520

Vessel needs to have a valid FCC station license. 47 CFR 80

Vessel needs to have a valid FCC Safety Certificate. 47 CFR 13

Vessel needs to have working navigational lights IAW COLREGS and US Navigation Rules of the Road.

Vessel needs to have required publications onbaord. 46 CFR 184.420

Vessel needs to incorporate "In-water emergency procedures" to the vessels emergency instruction list IAW Sector Honolulu Work Instruction for vessels engaged in dive/snorkel operations.

Vessel needs to have all required medicine contained within its first aid kit. 46 CFR 184.710 & 46 CFR 160.041

Vessel needs to hava a DAPI audit conducted to show compliance to the program. 46 CFR 16.203
