It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. During operational tests of the main engines, the port engine was unable to be secured from the primary shutdown station located on the bridge. Repair/replace parts needed to demonstrate proper operational shutdown of port engine from primary station to the satisfaction of the attending MI.
