Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Multiple instances of dead-end wiring were observed on board, such as: the second deck battery box, engine room, and second compartment. Openings in the breaker box on board were observed and need to be covered. The electrical outlet in the engine room was observed without a cover.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. A pinhole leak was observed on the starboard main engine wand was leaking oil.

Water for cooling the exhaust pipe, other than a vertical exhaust, must be injected into the exhaust system as near to the engine manifold as practicable. The water must pass through the entire length of the exhaust pipe. An exhaust cooling water leak was observed on the starboard main engine.

On a nonmetallic vessel, the enclosures and frames of electrical equipment must be bonded together to a common ground by a normally non-current carrying conductor. Grounding wires connected to fuel shutoff valves were improperly installed.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Operational test of all overboard discharge and intake valves and watertight bulkhead pipe penetration valves. @ overboard discharges were observed on the starboard side bulkhead of the engine room to be fitted without drains.

General condition and cleanliness of engine room, including machinery parts, surfaces, and bilges lacked cleanliness.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Numerous sections of fiberglass hull and wood framing in the vessels structure were found to with significant wastage, namely in the forward compartment, second and third compartments, engine room, and lazarette.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. The painter must:Have a breaking strength of at least 680 kilograms (1,500 pounds). A substantial nameplate must be permanently attached to each buoyant apparatus. The nameplate must contain the name of the manufacturer, lot designation or serial number, approval number, dimensions, and number of persons capacity. Space must be provided for the date, and the identification of the independent laboratory. Starboard life float was observed to have a hole in the side and a worn/wasted lifeline. Both life floats were without a nameplate.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. An adult life jacket must be provided for each person carried on board a vessel. Numerous life jackets were observed to be in need of new retroreflective tape, and several lifejackets were in need of replacement due to wasted material.

Except as otherwise provided in this section, rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers or crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. Ladders leading to the 2nd deck from the bow and stern were observed to be without a lifeline.

Replace deck frames and deck plywood, seal collision bulkhead, seal hole on deck port side of ladder and grind water intrusion around STBD side stiffener in fore peak.

Remove every other ceiling panel for inspectors to asses deck condition.

Clean engine room batteries, engine oil and exhaust leak residue.

Replace FRP MDE exhaust pipe.

Repair air vents on fantail. (both sides)

Replace STBD side inboard steering stop.

Clean fuel tank for further inspection.

Using non combustable material to bracket the bundled electrical wires just foward of the engines, using a bracket every 24 inches.

Line attached to weak link is tankgled. Untangle line and connect the weak link properly under the spooled line so that the line can pay out properly in an emergency.

Install a longer painter line, as required, for lifefloats. 46CFR180.137(e)(3).

Replace retroreflective tape on lifefloats (02 pieces) with approved type. 46CFR160.027.

Replace retroreflective tape on lifejackets with approved type. 46CFR185.604(h).
