The fire detection and alarm system must be capable of immediate operation at all times that the vessel is in service. The fire control panel was not alarming when heat detection and manual pull stations were tested. 46CFR76.27-5(b)

Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. A hydrostatic release unit used in a float-free arrangement must be approved under approval series 160.162. Inflatable life rafts were rigged with wire that was not float free. 46CFR199.130(c)(7)

Each vessel must carry a lifejacket for each person on board. All kapok style lifejackets on board were found deteriorated and unserviceable. Persons on board will be reduced by 683 persons. Total persons allowed on board is 1,482. 12JAN26: Revised to the following- Persons on board will be reduced by 195 persons. Total persons allowed on board is 1,970. 46 CFR 199.70(b)(1)

For each watertight door which is in a required subdivision bulkhead, an indicator light must be installed in the pilothouse and at each other vessel operating station from which the door is not visible. The indicator must show whether the door is open or closed. WTD indicator on the bridge has multiple inoperable lights. 46CFR170.270(e)

All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. Fire screen door and ladder well doors not closing numbers,1-70-2. 46 CFR 72.05-25(b)(9)(ii)

Doors, other than those which are normally locked, such as from staterooms, fan rooms, lockers, etc., shall be of the self-closing type capable of closing against a 3 1/2 degree list. Doors 1-59-1 and 4-68-2 did not self-close. 46 CFR 72.05-25(b)(9)(i)

Each vessel must have a means of communication among the navigating bridge and the steering gear room, if outside the engine room. Vessels 1JV sound powered phone for the bridge and steering gear room was inoperable. The vessel has multiple redundant communication systems, including handheld radios and dial telephones, must be corrected by 28 FEB 2026. 46CFR 113.30-5

Each lifeboat must be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months during an abandon-ship drill. Port side lifeboats have not been launched in the last 3 months. 46CFR199.180(d)(4)

As far as is reasonable and practicable, rescue boats, other than lifeboats which are also rescue boats, must be launched with their assigned crew aboard and maneuvered in the water each month. Such launching and maneuvering must occur at least once every 3 months. Port side rescue boat has not been launched in the last 3 months. 46CFR199.180(d)(7)

Each vessel must have an illuminated repeater for the gyrocompass required under paragraph (b) that is at the main steering stand unless the gyrocompass is illuminated and is at the main steering stand. Gyrocompass repeater in the steering gear compartment reading was not the same as the reading from the gyrocompass on the bridge. Corrected on the spot. 46CFR96.17-1(c)

Each muster station and embarkation station must be adequately illuminated by lighting with power supplied from the vessel's emergency source of electrical power. Vessel has multiple embarkation lights out on both the port and stbd sides. Corrected on the spot. 46CFR199.110(c)

Each generator prime mover must have an overspeed device that is independent of the normal operating governor and so adjusted that the speed cannot exceed the maximum rated speed by more then 15 percent. Emergency diesel generator overspeed failed to function when tested. Corrected on the spot. 46CFR111.12(b)

Suitable covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. Guard was missing that covers shaft on forward fire pump. Corrected on the spot. 46CFR72.40-20(a)

If mechanical ventilation is provided for in a protected space, the ventilation system must automatically shut down prior to discharge of the system to that space. Ventilation Fire Damper 01-24-4 would not close automatically as designed when fixed HALON system was tested in Flammable Supply Storage room. Corrected on the spot. 46CFR95.16-30 (a)

Each Class 3 WTD must be designed, constructed, tested and marked in according with ASTM 1196 and ASTM 1197. WTD at Frame 42, had gouge in the sealing edge of the frame, the door movement warning alarm was inop and the door could not be opened using the local hand pump. 46 CFR 170.270(c

The port side Ballast Tank No. 6 has approx. 1/8" diameter through hole damage inboard side in way of the lower bilge. Rectify prior to using the ballast tank. 46 CFR 71.40-1(a)

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or whenever any important repairs are made. The plating underneath the fire pump in Service Area #1 is wasted beyond 30% in several areas that is tank top of Ballast Tank #1. Rectify prior to 28FEB2026. 46 CFR 71.40-1(a)

An inspection, either general or partial depending upon the circumstances, shall be made whenever any important repairs or alterations are undertaken. Vessel was removing floor tiles in the way of 03-53-0 passageway outside of lavatories and discovered a hole and wasted deck. Submit repair proposal.

The Attending Marine Inspector is not limited from making tests or inspections as deemed necessary to be assured of the safety of the vessel. The MI requested to see operation of Boiler Safety-Valves. Crew was unable/unwilling to demonstrate operational test of boiler safety-valves. Vessel is not authorized to conduct missions prior to proving operation of safety valves to the Coast Guard.

Doors in stairway enclosures and Main Vertical Zone bulkheads shall be of the self-closing type capable of closing against a 3.5 degree list. Fire door 01-42-1 failed to close completely.

The vessel's firefighting equipment is to be at all times ready for use. Portable extinguisher #17's discharge hose is wasted. Replace extinguisher.

The OCMI is responsible for properly prescribing manning on vessels. The machinery inspector noted that the engineering manning level is inadequate based on the age, required maintenance, crew and passenger count, and multiple engine room configuration of the ship. In order to address this issue fill out and return Manning proposal supplement

Each passenger vessel of 500GT and over must carry on each side of the vessel at least one rescue boat. Vessel's stbd rescue boat davit is inoperable. Vessel submitted satisfactory repair plan and is permitted to operate with one rescue boat in accordance with MOU between MSC and USCG dated 30 May 2021. Due date is per repair plan submitted to USCG Sector Mobile on 18 May 2023.

A survey, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its equipment, or whenever any important repairs or renewals are made. UT Shots provided IVO STBD fuel storage tank 6-69-1 showed areas of wastage greater the 25% and a wasted/holed sounding tube pipe that was causing fuel to leak. Plate with wastage greater the 25% & sounding tube should be cropped and renewed

Doors shall have a latch with a minimum throw of 3/4" which can be operated from either side of the door. Vessel self-reported the following fire doors FSD61 01-82-1, FSD60 01-80-1, FSD58 01-72-1, FSD52 01-66-1, FSD50 01-63-1, FSD46 01-54-1, FSD43 01-50-1, (not numbered) 01-42-1 & FSD68 1-29-0 have latches that will not secure or broken hardware for the doors that need replaced.

Periodic Safety Test Procedures (PSTPs) for SSDG go against the recommendation of the Manufacturer according to [name withheld] and submit updated PSTPs to MSC for review and approval.

Each passenger vessel must carry a combination of lifeboats and liferafts that have an aggregate capacity sufficient to accommodate the total number of persons on board. Vessel was missing one 25 man life raft. Vessel’s total passenger count is reduced by 25 persons until missing liferaft is replace. 46 CFR 199.201

Each passenger vessel of 500 tons gross tonnage and over must carry on each side of the vessel at least one rescue boat. Vessel’s stbd side rescue boat davit is inoperable. Vessel may operate as permitted in Sector Mobile issued in letter 22-319. 46 CFR 199.202

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Vessel was not able to demonstrate jacket water temp & low oil pressure (SSDG), low battery voltage(SSDG), high lube oil temp (EDG), high jacket water temp(EDG), Steering Gear Overload, Sitchboard interlocks, high lube oil temp (ADG) & Battery Charger Fail on ADG's). Vessel did not have means to test in presence of MI's during COI. Vessel must demonstrate tests in presence of MI's once they have the proper tools/equipment to do so IAW approved procedures. 46 CFR 61.40-1

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work, except as modified by § 71.55-1(b). The general scope of the plans shall be as noted in § 71.65-5. Vessel replaced some of the fire extinguishers onboard since the previous approved/stamped fire/safety plan. Contact cognizant OCMI to update plans to accurately reflect onboard arrangement and amend COI to reflect fire extinguisher weight based to performance based harmonization. 46 CFR 71.65-10

Tests required before entering or getting underway;primary and secondary steering gear; to include the full movement of the rudder to the required capabilities of the steering gear. Vessel unable to achieve left/right 35 degrees. Helm indicates 35 degrees however rudder only achieves 33 degrees. Vessel to provide objective evidence from troubleshooting attesting to satisfactory operation of full steering capabilities prior to departing Norfolk, VA. 33 CFR 164.25(a)(1)(vii)

All passenger vessels shall have efficient guard rails.Found wasted top rail on O1 deck below lifeboat 10, wasted top rail near Purser's Office at O1-85-1, wasted bottom rail on fantail in port and stbd quarters, wasted bottom rail FR 67 port side and FR 80 on O2 deck in vicinity of lifeboat 8. Repair or replace to satisfaction of attending CGMI. 46 CFR 72.40-5

The inspection shall ensure structure, piping, and other equipment is in satisfactory condition and fit for the service for which it is intended. The exhaust pipes above O2N2 station on stbd side, labeled #5 and #1 were found unserviceable. Repair or replace to satisfaction of attending CGMI.46 CFR 91.25-10. 18Jan22 This has been added to the vessels drydock repair plan for the upcoming drydock scheduled for 18Jan22.

The inspection shall ensure structure, piping, and other equipment is in satisfactory condition and fit for the service for which it is intended. Bottom support for drain pipe found wasted on O2 deck, port side, in vicinity of Frame 65 and rescue boat. Repair or replace to satisfaction of attending CGMI.46 CFR 91.25-10

When more than 12 nm from the nearest land, any discharge of oil or oily mixtures into the sea from a ship is prohibited except when the ship has in operation oily water separating equipment as required by part 46 CFR 155 subpart B. The vessel’s oily water separating equipment is inoperative.33 CFR 151.10(a)(6)

The spring in a relief valve in service for up to and including 250 psi shall not be reset for any pressure more than 10 percent above or 10 percent below that for which the relief valve is marked. The SSDG air receiver Nos. 1 and 2’s relief valves setting are marked 145 and 150 psi respectively and have been adjusted down to 110 psi.46 CFR 54.15-10(g)

Cylinders containing hazardous ships’ stores that are compressed gases must be filled, marked and inspected in accordance with 49 CFR 173.301 through 173.308. Engine room oxygen compressed gas cylinder is out of its hydro-static test date. Remove cylinder from vessel or hydro-statically test IAW 49 CFR 173.46 CFR 147.60(a)(3)

Cylinders containing hazardous ships’ stores that are compressed gases must be filled, marked and inspected in accordance with 49 CFR 173.301 through 173.308. Disposable compressed gas cylinders containing refrigerant were found in the A/C flat. Remove disposable compressed gas cylinders from vessel.46 CFR 147.60(a)(3)

Each hand portable fire extinguisher shall be marked with a corresponding number. Machinery space fire extinguishers and locations are numbered incorrectly.46 CFR 78.47-30(a)

All general alarm bells shall be identified by red lettering at least ½ inch high: “GENERAL ALARM-WHEN BELL RINGS GO TO YOUR STATION”. The general alarm beacon at the pump room IWO watertight door has no placard. 46 CFR 78.47-7(a)

Port rescue boat is inoperable. Tests and inspections shall be carried out to demonstrate the proper condition and operation of rescue boats. Provide proof of proper operation to attending USCG Marine Inspector.

Both lifeboat Nos. 6 and 9 are out of service for repairs. Passenger count reduced by 115 until lifeoboat repairs have been effected and both are back in service.

Doors in stairway enclosures & double swing doors shall be of the self closing type. FSD Nos. 78, 97, 98, & 109 and FSD Fr. 42 don't latch close.

Fantail natural ventilation closure for aft steering gasket is wasted. Starboard DFM shore fill connection at fantail has small discharge containment that is wasted. Fantail distilled water TLI MCT firestop found improperly maintained.

In all ventilation systems, manually operated dampers shall be provided in accessible locations outside the space served to shut off the passage of air in the event of fire. Found ventilation dampers Nos. 01-64-9 and 2-43-5 frozen and inoperable.

Each panel board must have the rating of setting of the overcurrent protection device for each circuit. The main machinery space main panel board has a breaker labeled for 30amps that has a 40amp breaker, and a breaker labeled for 40a that has a 30a breaker. Confirm with drawings the correct breaker amperage and make repair to the satisfaction of attending Marine Inspector.

A relief valve shall not be reset for any pressure more than 10% above or below that which the relief valve is marked. Relief valves for pressure vessels 1,2 and 3 in main machinery space have data-plates stamped at 145psi, but are readjusted as indicated by a hanging tag at 110, 125 and 110psi respectively. Repair or replace to the satisfaction of attending Marine Inspector.

Dampers shall be designed to close. The supply vent damper in the Aux Generator space was not closing properly. Repair to the satisfaction of attending Marine Inspector.

The annual inspection shall include an examination of fire detecting equipment to ensure it is satisfactory and fit for the service for which it is intended. Call box in Fire Zone 38 for service area number 2 was inoperable.

Manually operated dampers or other suitable means shall be provided in accessible locations for shutting off air in the event of a fire. Damper in O2N2 compressor room exhaust was not functioning. Make permanent repairs prior to operation of the O2N2 compressors. 46 CFR 72.05-50(i)

The propulsion steam plant's blowdown pipe which penetrates the hull below the waterline on the aft stbd side of the machinery space has a pin hole between the skin and the stop valve, as well as between the boiler isolation valve and the stop valve. Make repairs to the satisfaction of the USCG and Load Line issuing authority prior to departure.

Demonstrate the launching, operation, and recovery of the four starboard open lifeboats to the satisfaction of the attending Marine Inspector.

The #3, #4, #5, #6, #7, #8, #9, and #10 lifeboat tailshaft through hull penetration sealing system non-metallic hoses are no longer serviceable. Replace the non-metallic hoses with OEM specified or equivalent non-metallic hoses, and prove the proper operation of all four open lifeboats to the satisfaction of the attending Marine Inspector. The total persons allowed is reduced by 920 until the requirement is cleared.

The #8 lifeboat davit forward arm outboard sheave and the #6 lifeboat davit aft arm inboard outermost sheave are corroded. De-scale the sheaves, provide a gauging report and the davit manufacturers maximum wastage allowance to the satisfaction of the attending Marine Inspector.

The #4, #6, and #9 lifeboats fiberglass is delaminating in differect locations in the fwd area of the interior hull. Repair the forward outboard bilge steps using OEM specified repair procedures to the satisfaction of the attending Marine Inspector.

The #5 lifeboat's forward releasing gear keel plate's fiberglass is delaminating. Repair the fiberglass using OEM specified repair procedures to the satisfaction of the attending Marine Inspector.

The #4, #5, #6, #7, #8 and #9 lifeboats hull grab rails are corroded in various locations. Repair or replace the corroded grab rails to the satisfaction of the attending Marine Inspector.

The #9 lifeboat is missing its data plate. Replace teh data plate to the satisfaction of the attending Marine Inspector. 46 CFR 160.135-17

The #3 lifeboat exhaust manifold riser has been repaired using a synthoglass type material. Provide documentation from the lifeboat OEM that the repair is authorized, repair the exhaust manifold riser assmebly in accordance with OEM repair procedures, or replace the exhaust manifold riser with OEM approved parts. Once repairs are complete prove the proper operation of the #3 lifeboat engine to the satisfaction of the attending Marine Inspector. The total persons allowed is reduced by 115 until the requirement is cleared.

The #9 lifeboat forward releasing gear keel plate's fiberglass is delaminating and the plate is corroding. De-scale the plate, provide a gauging report and the lifeboat's OEM maximum wastage allowance, and repair the fiberglass using OEM specified repair procedures to the satisfaction of the attending Marine Inspector. The #5 lifeboat fuel tank fill cap assembly is attached using an epoxy life substance. Provide documentation from the lifeboat OEM that the repair is authorized, repair the fuel tank fill cap assembly is accordance with OEM repair procedures, or replace the fuel tank full cap assembly with OEM approved parts to the satisfaction of the attending Marine Inspector.

The #6 lifeboat radiator cap assembly has been repaired with an epoxy like substance. Provide documentation for the lifeboat OEM that the repair is authorized, repair the radiator cap assembly in accordance with OEM repair procedures, or replace the radiator with OEM approved parts. Once repairs are complete prove the proper operation of the #6 lifeboat engine to the satisfaction of the attending Marine Inspector. The total persons allowed is reduced by 115 until the requirement is cleared.

The #8 lifeboat engine's salt water cooling system is not circulating water. Repair the salt water cooling system and prove the engine's proper operation including proper salt water circulation to the satisfaction of the attending Marine Inspector. The total persons allowed is reduced by 115 until the requirement is cleared.

The #6 lifeboat releasing gear is not functioning properly. Repair the releasing gear and prove its proper operation to the satisfaction of the attending Marine Inspector. The total persons allowed is reduced by 115 until the requirement is cleared.

Cable and pipe penetrations in fire bulkheads and decks must maintain the proper classification as approved by Coast guard. Materials must meet CG approval as tested to IMO Res. The stbd engine for the port-side enclosed lifeboat (tender vessel) has been rebuilt. Lifeboat must be maneuvered in the water with the newly renewed engine to the satisfaction of the attending CG MI, prior to the completion of next drydock or 30 May 2017, whichever is sooner.

New refrigeration units were installed in main galley which required modification to the vessel's electrical system, two 440v to 220v transformers installed. Submit installation plans and gain approval from Marine Safety Center.

Vessel has excessive wastage on the ballast tank top steel plating located on the tank top of service space #6. The Emergency Diesel Generator was found to be inoperable. Make the EDG ready to demonstrate the safety shutdowns and alarms, including the automatic start. Vessel is required to have an appropriate tug escort during transit from Naval Station Norfolk to MHI.

Found the Control Desuperheater Valve on the port boiler to be leaking. Repair or replace the valve.

Found leaking non-metallic cooling water line on the ship's service diesel generator. Replace in kind.

Found a lube oil leak on the Turbocharger Lube Oil piping of the #1 Auxiliary Diesel Generator. Repair or replace the flange.

Found fuel oil leak on the Starboard DFM Storage Tank, 6-87-1. Determine the source of the leak and provide a repair proposal.

Found Watertight Door #2 operation to be excessively slow. Repair necessary components and demonstrate operation to the attending marine inspector.
