Immediately after addressing safety concerns, the owner,
agent, master, or person in charge of a vessel involved
in a marine casualty shall notify the nearest Sector
Office whenever a vessel is involved in a marine
casualty consisting of an occurrence materially and
adversely affecting the vessel's seaworthiness or
fitness for service or route. On September 13, 2019 the
vessel had an allision with the pier in Erie, PA. The
vessel's gangway was damaged and required repairs.
Those repairs were completed without prior Coast Guard
oversight. The vessel is not allowed to carry
passengers until the repairs have been witnessed by an
attending Coast Guard Marine Inspector.

vessel only had 4 flares on board during the inspection.

of the passengers and crew foremost in mind by directing
the vessel in order to prevent a casualty. During the
inspection, the vessel was moored port side for
maintenance and the vessel's master was not able safety
depart the pier without rubbing down the side of the
pier. Provide a detailed plan and procedures to be
implemented into your daily operations for mooring and
departing the pier.

order to provide adequate protection against fire,
explosion, machinery failure and personnel injury.
During the inspection the following hoses were
discovered in poor condition needing replacement:
1. Breather Hose on the starboard main.
2. Small coolant hose on the starboard and port main
3. Hose on the raw water discharge on starboard main.
4. Provide chaffing gear on coolant hose on starboard
generator.
All hoses need to be replaced with approved hoses
meeting either: SAE J-1942 or USCG Type A1, A2, B1 or B2
flexible hose. Reference 46 CFR 182.720 for more
information.
