Alterations and repairs to inspected vessels must be done under the direction of the USCG OCMI. The operator made undocumented alterations to the vessel which are not documented in the approved stability information carried onboard the vessel. The undocumented changes occurred throughout the vessel's service history and without the direction of the USCG OCMI. The vessel was originally built with two ONAN 12 Kw generators installed in the engine compartment. Both generators and associated piping were removed at an unknown date and stability was not recalculated. The vessel was originally built with a water sprinkler fire protection system. All sprinkler components were removed at an unknown date and stability was not recalculated. This vessel is 75 feet in length and built in 1986. The original stability letter indicates a simplified stability test was performed which should not have been accepted. Discussions took place between the owner and OCMI in 2011 when the stability passenger weights changed and the original stability test was deemed a mistake, however, the COTP did not require a new stability test. The undocumented alterations were unknown when the 2011 COTP decision was provided as per email documentation that was preserved in the vessel hard file. The original simplified stability proof test results are no longer valid due to the undocumented alterations found throughout the vessel's service history and incorrect documentation/approval of a simplified stability test for a vessel over 65 feet in length as per 46 CFR 171.020. The vessel shall comply with stability regulations found in 46 CFR 178. Updated the "Action" to Code 17 on December 11, 2024 due to the operator failing to meet the extended due date. 46 CFR 178.310(a) 46 CFR 170.005

Vessels of more than 65 ft in length as well as a vessel with more than 1 deck above the bulkhead deck exclusive of a pilot house must have either permanent draft marks or a loading mark. The vessel is more than 65 feet and has two passenger decks. Draft marks or a loading mark are not indicated on the hull. Provide draft marks or a loading mark once a new stability letter is issued indicating its placement location on the hull. 46 CFR 185.30-3

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, which are completed to the satisfaction of the cognizant Officer in Charge, Marine Inspection (OCMI) on or after March 11, 1996, must comply with this part. Replacement of existing equipment, not including wire or cable, installed on the vessel prior to March 11, 1996 need not comply with the regulations in this part. The operator removed all AC electrical conductors and the entire vessel is reliant on DC electrical to include all lighting and navigation equipment. Submit a one line DC electrical diagram for review. 46 CFR 183.115

Spaces containing machinery shall be fitted with at least two ducts to furnish natural or mechanical supply and exhaust ventilation. Where forced ventilation is installed, the duct extending near the bottom shall be the exhaust. The vessel uses mechanical ventilation fans and there is no duct from the exhaust to the bottom of the machinery compartment. 46 CFR 182.20-45(c)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The engine room starboard side exhaust duct hatch closure was missing during the inspection. 46 CFR 182.465(h)

A pre-engineered fixed gas fire extinguishing system must: Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector and automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. The pre-engineered FireBoy system had its safety pin installed when inspected by the MI; the pin prevents manual activation from outside of the space. The pin was removed and the system was tested. The test revealed that the mechanical ventilation and engine will not shut down if the FireBoy is deployed. 46 CFR 181.420(a)

An adult life jacket must be provided for each person carried on board a vessel. 66 adult Kapok life jackets failed inspection. The total amount of adult persons on board is reduced to 71. 46 CFR 180.71(a)

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Primary overcurrent protection is not installed from the battery switch to the DC panel. The 10 AWG conductor is not protected. 46 CFR 183.05-50

Where fire extinguishing equipment are not required but installed, the system or equipment shall meet the regulations in this part. The vessel is outfitted with a belt drive fire pump on each main engine. The port fire pump began leaking at the impeller housing when tested and lost pressure on deck. 46 CFR 181.05-5

Shutoff valves installed so as to close against the fuel flow shall be fitted in the fuel supply lines, one at the fuel tank connection. The shutoff valve at the tank shall be operable from outside the tank compartment. The port fuel tank shut off valve's remote pull cable failed when tested. 46 CFR 182.20-40(b)(3)

Where liquid level indicators are installed on fuel tanks they shall be of heat resistant materials, adequately protected from mechanical damage and provided at the tank connections with devices which will automatically close in the event of rupture of the gage or gage lines. The port/stbd fuel tanks were found with plastic sight glass gages installed and no automatic shut off device in the event of failure. 46 CFR 182.20-25(a)(5)

Propulsion machinery shall be of suitable type and design for propulsion requirements of the hull in which it is installed and capable of operating under constant marine load without exceeding its design limitations. The stbd main engine sea water cooling valve was found leaking at the hull. 46 CFR 182.05-1

The operator shall provide plans for the bilge piping system and all updates to the system to include piping material. 46 CFR 177.05-1

Fuel lines shall be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration. Fuel lines on the port/stbd main engine were found leaking at the connection to the fuel filter and were not found supported. Fuel lines above the stbd fuel tank were not found supported. 46 CFR 182.20-40

Repairs or alterations to machinery must not be made without the approval of the [name withheld] starboard reduction gear sustained mechanical limitation that resulted in reduced speed on starboard propulsion engine. If repairs are not utilizing (In-Kind) parts, manufacture specification of the replacement parts shall be forward to OCMI for review and approval.

Install missing air filter on port and STBD turbos.

Provide additional (15) adult life jackets and (03) child sized to life jackets storage identified on upper deck.

Repair inoperable bilge high level alarms for port and STBD engine room.

Fixed Fire Extinguishing system must be in serviced and properly operating as required by 46 CFR 181.420. Pre-engineered FGFE system unbale to shut down starboarb engine during test. Repair FGFE IAW manufacture instruction.

Vessel must have gauges indicate engine cooling water discharge temperature and oil pressure must be provide for all propulsion as required by 46 CFR 182.20-5. Show proper operations for port engine gauges.

Vessel must have adequate emergency lights as required by 46 CFR 183.432. Repair inoperable emergency lights on upper deck.

Flexible hose designed for use with hose clamps must have two clamps at both ended as required by 46 CFR 182.720. Properly install hose clamps on fuel filters for both main engines.
