STCW application submitted to NMC. •3AE missing all STCW endorsements. Member will remain on board so Redline is not crossed for Logistic UNREP Operations. STCW application submitted to NMC. •BOSN missing STCW Able Seafarer-Deck endorsement. CARGO BOSN can cover duties as Ship/Cargo BOSN. STCW application along with Able Seafarer assessments submitted to NMC. •Chief Engineer missing STCW Advanced Oil Tanker Cargo Operations. STCW application has NOT been submitted. Chief Engineer does not participate in any cargo transfers. Member will submit application to NMC. •1AE missing STCW Advanced Oil Tanker Cargo Operations. Shipboard incinerators shall be operated only in accordance with the manufacturer’s operating manual. The incinerator door did not close properly which could prevent proper sealing to the combustion chamber during operation, including a potential fire and smoke hazard. Periodic Safety tests must demonstrate the proper operation of the primary and alternate controls, alarms, power sources, transfer override arrangements, interlocks, and safety controls. Systems addressed must include fire detection and extinguishing, flooding safety, propulsion, maneuvering, electric power generation and distribution, and emergency internal communications. (b) Tests must be conducted at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. Note 1 to paragraph (b):Normally, these tests are conducted annually. To be completed by 11MAY2025 vessel can not operated in PUMS.

The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. If the signals have not received attention within two minutes, an audible alarm shall be automatically sounded throughout the crew accommodation and service spaces, control spaces, and machinery spaces of category A. Third party fire servicing report stated that the vessel's fire detection system was not tied into an alarm that would sound after two minutes of inattention and that a test of that alarm was not carried out. 74 SOLAS (81 Amend.) Smoke detectors shall be installed in all stairways, corridors, and escape routes within accommodation spaces. Vessel's stairways, corridors, and escape routes within accommodation spaces are equipped with heat detectors, not smoke detectors. 74 SOLAS (81 Amend.) II-2/13.2.2, 50a

Measures shall be taken to the satisfaction of the Administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. Found overspeed automation testing for both diesel generators was last conducted in 2021 as per documentation, and the crew was unable to perform at this time. 74 SOLAS (81 Amend) II-1/45.2, 16a

After any survey of the ship has been completed, no change shall be made in the items covered by the survey without the sanction of the administration. Several different models of heat detector are installed throughout the vessel in various spaces, including several that are rated for an ambient temperature of maximum 100 degrees F installed within the main machinery space. The function of the detection system shall be periodically tested by means of equipment producing hot air at the appropriate temperature. A minimum of two two-way portable radiotelephone apparatus for each fire party for fire-fighter’s communication shall be carried on board. Those two-way radiotelephone apparatus shall be of an explosion proof type or intrinsically safe. Ships constructed before 01JUL14 shall comply not later than the first survey after 01JUL18. Fire doors in main verticle zone bulkheads, galley boundaries and stairway enclosures other than power operated doors and normally closed shall satisfy the following reruirements. Fire door 06-70-1 would not close properly and fire door 03-102-1 missing kick pannel.

Doors and door frames in "b" class divisions and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that divisions, this being determined in accordance with the ftp code. Machine shop door on 25ft flat was damaged, bent, and missing lock assembly.

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in a normal manner. open ended wires were present in the overhead above start air receivers from newly installed evaporators.

No change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. Vessel master reported temporary valves were installed in the deck foam fire extinguishing system for cargo zones 1 and 2.

Emergency generator automatic hydraulic start system was unable to provide three consecutive starts. System pressure dropped from 3000psi to 100psi. Repair, provide tech rep report, and demonstrate proper operation of EDG hydraulic starts to the satisfaction of the attending ABS surveyor.

Food rations for all persons onboard each lifeboat must be airtight and kept watertight. 276 units are required of 10,000kJs; 69 per unit.

Ventilation dampers 06-92-4 and 06-92-3 were inoperable. Means shall be provided to close all openings which admit air or allow gas to escape from the machinery space. Repair and demonstrate to the satisfaction of the attending ABS surveyor.

Floating orange smoke signals on bridge wings are not USCG approved. Replace with USCG approved floating orange smoke signals to the satisfaction of the attending ABS surveyor.

Lifeboats #1 and #4 shall be evaluated/serviced to address the fiberglass hull and its seaworthiness. Crew personnel/passengers are restricted to no more than 68 persons. Servicing shall be by manufacture representative. Provide servicing information to the satisfaction of the attending ABS surveyor.

The fire detection system was modified after professional certification and was no longer certified. System shall be recertified via professional servicing to the satisfaction of the attending ABS surveyor.

Three port side and three starboard side cable penetrations in class A boundaries located between the EOS and the steering gear flat were observed in poor condition and altered without Class or USCG approval. Repair to the satisfaction of the attending ABS surveyor. No extensions will be granted.

One of ship's two service diesel generators are OOC. Vessel may conduct berth shift from pier 8/ berth 1 to pier 10/ berth 5 at Naval Station Norfolk. Vessel is required to have two assist tugs with aggregate total horsepower 7,000 or 70 ton bollard pull.

Vessel did not have required markings on/or in the vicinity of all life-saving equipment. Install required symbols as required in accordance with vessels Safety Plan to the satisfaction of Class Society.

Found most of the cargo hoses past due for the required annual hydrostatic test. Complete hydro test of the cargo hoses to the satisfaction of the Class Society.

The following fire protection systems shall be in good working order so as to ensure their required performance if fire occurs; structural fire protection, including fire resisting divisions, and protection of opening and penetrations in these divisions. There are two fire dampers in the supply ducting to the incinerator room, one is manual and the other is electrical, but not wired into any system. The exhaust damper is also manually operated. Instructions should be added to existing fire fire fighting procedures for this space to include manually operating these dampers before releasing the fire fighting agent.

Starboard main engine had chaffed fuel line resulting in fuel spray in the engine room. Replace/repair fuel line and clean fuel from engine room. Repair to the satisfaction of the attending class surveyor.
