Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Main engine fuel shutoff assembly failed to close fuel supply valve. Correct prior to carriage of passengers. 46 CFR 182.455(b)(4)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] missing for secondary means of escape on watertight door forward in engine room and through forward void deck hatch. 46 CFR 185.606

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Emergency broadcast instructions are missing. 46 CFR 184.506

Each vessel must be marked as required by part 67, subpart I, of this chapter. : The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel name not marked on bows and hailing port not visibly marked on stern. 46 CFR 185.602(a)

The marine inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that they are in satisfactory condition. The starboard generator raw water discharge piping was leaking at the elbow closest to the bulkhead. Repair issue to the satisfaction of the Marine Inspector.

Piping used in a vital system must: Be composed of ferrous materials. Temporary repair found on underside of fire pump intake piping just before the manifold leading into the fire pump. Submit repair details for making a permanent repair to fire pump piping. Alternatively, provide three, 2.5 gallon fire buckets onboard as a fire pump is not required on this vessel.

Each survival craft must be: secured to the vessel by a painter with a float free link permanently attached to the vessel. The link must be constructed essentially as shown in figure 160.073–10. The link must be formed from a single saltwater corrosion-resistant wire. A loop at least 50 mm (2 in.) in diameter must be provided at each end of the wire. Each loop must be permanently secured…. The breaking strength of each link must be between: 450 N (100 lb.) and 600 N (134 lb.) for links intended for life floats and buoyant apparatus of 10 persons and less capacity….. Float free link on life float is insufficient, replace with properly rated wire.

A voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator that is in operation…..Repair or replace starboard generator ammeter. 46 CFR 183.10-5(g) Old T

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.”…. Entrance to engine room space does not have a CO2 warning placard, install placard in a visible location on the exterior area of entry into engine room space.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coat Guard officials upon request. Test records for empolyees were not available onboard and crewmembers onboard were not listed under document showing which employees were subject to drug tests.

At each initial and subsequent inspection for certification of a vessel, the owner shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following: Operational test of all emergency electrical systems. General alarm did not sound despite it being on a closed emergency circuit at the time of the inspection.

Except as noted in paragraph (d) of this section, the data shall be stamped directly on the pressure vessel. The data shall be legibly stamped and shall not be obliterated during the service life of the pressure vessel. The data plate that meets the requirements of 46 CFR 54.10-20(a) regulations for the pressure vessels (air receivers) is missing.

Exhaust piping shall be led to the point of escape with a minimum number of bends and elbows. It shall be gas-tight throughout its length and shall be properly supported by non-combustible hangers or blocks. MIs onboard and at least one crew member observed an apparent exhaust leak in the engine room while both generators and the main engine were running. Identify and prove that the engine room exhaust piping is gas-tight.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Drills and training logs were not kept up to date. Logs were updated in the presence of the attending MI.

46CFR 183.05-20 - Batteries shall be effectively screened by a cage or simliar structure to minimize the danger of accidental spark through dropping a metal object across terminals. Numerous storage conatiners of ships batteries not screened or covered as required.

46CFR 176.25-25 - The fire pump shall be tested and teh fire hose shall be satisfactorily subjected to the available fire pump pressure. Fire hose tear witnessed during test. Replacement hose required.
