During Damage Survey on 02 SEP, it was observed that a majority of the deck level shroud stay turnbuckles are nearly bottomed out. Prior to repairing damaged structures, de-tension mast shrouds to remove trapped stress on vessel structural members.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] deck beams may be repaired within parameters discussed during marine inspector and shipyard operator meetings on 27 AUG and 02 SEP, which refined the written repair plan emailed on 26 AUG 2025 by Newport Harbor Shipyard. Transverse deck frame 2 will be replaced, the remaining damaged frames will be repaired with a scarfed wood piece and sister. Notify marine inspectors to view transverse frame repairs prior to reinstalling plywood sub floor. Notify marine inspectors to view bottom plywood subfloor prior to relaunching vessel.

The purpose of un-stepping the rig is to provide a close-up visual inspection of the entire rig including the mast and mast step as well as critical attachments of shrouds and stays at both the mast and hull (chain plates). During Damage Survey on 02 SEP, it was observed that a majority of the deck level shroud stay turnbuckles are nearly bottomed out. Mast last un-stepped 07 MAR 2018, and six year interval was extended to 30 NOV 2025 due to vessel not using sails. Given the structural failure and repairs being made to major hull members, USCG is requiring mast be un-stepped prior to operating vessel with passengers.

When required by the marine inspector, a portion of the inspection must be conducted while the vessel is underway so that the hull and internal structure can be observed. Prior to returning to passenger service, vessel will demonstrate a sea trial in typical ocean conditions for vessel charter operations.

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant OCMI. Vessel suffered damage at port bow. Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel experienced a loss of propulsion on the port engine caused by a rupture in the port main engine turbocharger oil return line. Vessel to effect permanent repairs prior to movement to the satisfaction of the attending Coast Guard marine inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of the condition of the superstructure, masts, and similar arrangements constructed on the hull, and on a sailing vessel all spars, standing rigging, running rigging, blocks, fittings, and sails. MI identified multiple cracks on the beam in the vicinity of the bow stay. Rigging Survey to certify the cracks pose no danger to causing a de-masting must be completed prior to the vessel operating.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. During power trials, the steering system failed causing the steering ram mount to break and piping to rupture.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A means to reset devices used to automatically shut down ventilation systems and engines. Upon simulated activation of the pre-engineered system for the port engine, the engine would not restart.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Stbd engine transmission cooling line is dripping transmission fluid at a rate of approx. 15 drips per minute.

Life floats and buoyant apparatus must not be stowed in tiers more than 1,220 millimeters (4 feet) high, when stowed in tiers, the separate units must be kept apart by spacers. Life floats stacked 2 high with no spacers.

The use of twist-on type wire nuts is permitted under the following conditions: the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Three conductors connected via wire nuts in forward Port Engine room.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Cables and wires found in various locations on vessel with no supports or in distances further than 24 inches. Spaces included: Port engine room, STBD aft underwater viewing space, and bar area by mast step.

Flexible nonmetallic hose may be used in lube oil, fuel oil and fluid power systems, subject to the following requirements: Flexible hose runs must be visible, easily accessible, protected from mechanical damage, and must not penetrate watertight decks or bulkheads. Port and STBD propulsion engine return diesel fuel lines pass through multiple bulkheads. Action Code 705: Complete during scheduled re-power haulout on or before January 6th, 2025.

All wiring and cables shall be suitable for marine service and for the particular installation used. In addition, Flexible or portable cords such as SO, STO, and similar types cannot be used for permanently installed wiring. Portable cord was found extending the electrical circuit from inverter to bar area. Circuits from battery to inverter are not marine grade or UL Boat Cable. Prior to any electrical modifications; Provide single line electrical drawing of proposed electrical installation to facilitate A/C receptacle power supply to bar, drawing shall also include list of materials, length of circuits, #AWG, and over-current protection rated for the entirety of the circuit.

Stainless Steel Wire Rigging: The following guidelines apply to bare 316 grade stainless steel wire cable that is not “served,” coated or otherwise covered. The intervals are not absolute but considered good marine practice. Life expectancy for stainless on west coast is 10-15 years per NVIC 02-16. Replacement intervals outside these guidelines may be considered on a case-by-case basis with the approval of the local OCMI where an individual norm can be justified due to design, condition and service exposure. Stainless steel standing rigging is at or near 15 years, provide plan or update PMP to reflect replacement or renewal of stainless steel wire cable on rig.

A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds. Vessel reported the loss of its stbd rudder.

Portable fuel systems, including portable tanks and related fuel lines and accessories, are prohibited except where used for portable dewatering pumps or outboard motor installations. Vessel was storing 25+ gallons of diesel in 5 portable jerry cans that were stored under the bar to be utilized for refueling the vessel in between trips. Remove from vessel and discontinue practice unless fuel tanks meet requirements set forth in 182.440. PTCP

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Sternlight was found inoperable, vessel is restricted from operating in periods of reduced visibility to include periods between sunset and sunrise (Rule 20).

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station. Pre-engineered fixed gas fire extinguishing system installed in port and STBD engine room's equipment for indicating a discharge of the system (light/audible/and means to reset engines was not located at the operating station. Equipment was located at the previous operating station that has since been discontinued. Reinstall mentioned equipment at the current operation station and demonstrate proper operation to attending Marine Inspector.

A fixed gas fire extinguishing system may protect more than one space. The quantity of extinguishing agent must be at least sufficient for the space requiring the greatest quantity. Port engine room had penetrations in bulkhead that were not gas tight and missing panels to prevent fire extinguishing media from traveling out of space and into tunnel of catamaran.

Conductors in power and lighting circuits must be No. 14 American Wire Gauge (AWG) or larger. Wires/cables utilized to power submersible bilge pumps and interior lighting were 16AWG. Remove and replace all conductors utilized for power and lighting to 14AWG; Cable and wire must be UL Boat or UL Marine cable and be a minimum 14AWG.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child and Adult PFDs were stowed in the same compartment under the passenger bench with no means to separate the child life jackets. Provide means to separate or change stowage location and properly mark.

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. Two child PFDs were found with broken or missing clasp for tightening and securing jacket to passenger. Vessel is restricted from carrying more than 10 passengers under 90lbs.

Provide purchase and installation dates for all rigging key components listed in Appendix (1) to Enclosure (1) of NVIC 02-16 and vessels PMP equipment list. Vessels PMP does inventory key rigging equipment but does not list when components were installed/put into service.

Provide approved sail plan showing sail areas and centers of effort as well as arrangement, dimensions, connections of the standing rigging. MI was not provided with an approved sail plan to verify sails onboard were that of the approved plan.

Rigging Preventative Maintenance Program (PMP) requires that vessel operator will conduct monthly rigging inspections and log these inspections to document rigging components condition. PMP logs indicate that the last monthly inspection was conducted on 12AUG2022.

A submersible electric bilge pump shall be permanently mounted. Float switch for submersible bilge pump in STBD head was found not mounted to hull which could inadvertently caused pump to turn on and run with no water and drain 12volt DC system. Mount float switch to hull.

Each small battery installation must be located in a well ventilated space and protected from falling objects. Terminals on battery located in aft helm station had no cover for terminals. Provide cover for battery to protect terminals from being inadvertently connected by a falling object.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Battery located in aft helm station had wing nuts on terminals. Provide permanent means to secure battery connections.

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the required spaces. Alarm panel at operating station does not indicate from what space the alarm is originating from. Label each alarm for each space.

Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in a Serious Marine Incident, testing must be conducted within 2 hours of when the SMI occurred. Vessel shall carry at least 2 alcohol testing strips for minimum manned vessel in the event the vessel has a SMI and is 2 hours away from the dock.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel reported that the port engine shifting cable/linkage broke. Repair to the satisfaction of the attending USCG Marine Inspector. 46 CFR 182.200 (a)

For a life float - Approved under approval series 160.027 or other standard specified by the Commandant. Found 3 of 4 life floats at the limits of serviceability.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station for bilge flooding. The port aft bilge alarm was inoperable.

The vessels 10 B:C fire extinguisher must be located at the primary operating station. The 10 B:C was found at a decommissioned operating station on the aft of the vessel.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Provide a preventative maintenance plan in accordance with NVIC 02-16 (sail/rigging)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, & other hazardous situations must be corrected. Vessel has installed extra oil reservoir for steering system, vessel required to remove.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel has minimal rig inspection log & has not inspected above deck level of rig in 12+ months. Vessel is required to have 3rd party rigging survey completed & is not authorized to use auxiliary sail until completed.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Vessel does not have a visual indicator at operating station & is required to have extra crew member @ panel while operating w/passengers onboard until corrected.

The materials and construction of fuel lines, including pipe, tube, and hose, must comply with the requirements of this section. Vessel has temporary patch for stbd side fuel line approx. 1 foot after emergency fuel valve. Vessel is required to make permanent repairs and check daily.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. Vessel has stbd float free link in need of renewal due to broken end.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight. Vessel has circle/wire light hole on port transom 3 feet above the waterline. Vessel to permanently repair.

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel. Vessel required to secure starting batteries port & stbd to include covers.

A visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. Vessel required to label high water visual/audile alarms at oeprating station.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel experienced a catastrophic failure in the starboard shaft, where it sheered the shaft off, broke the shaft coupling, and allowed significant water entry to the engine room. Vessel must prove proper repairs to all affected engine room equipment prior to movement.

The rules concerning lights shall be complied with from sunset to sunrise. Vessel's starboard running light extinguished. Vessel to provide starboard running light. Vessel may not operate between sunset and sunrise or during periods of restricted visibility.

Vessel must be able to recover a helpless person from the water. Equipment provided was found to be insufficient due to weight limitations. Vessel to prove proper operation of man overboard recovery equipment.

Each survival craft on a vessel must be in good working order. Port side 12 man life float has a hole and internal foam exposed. Vessels total persons permitted is reduced by 24 people to equal 30 persons. Until 12 man life float is properly serviced, replaced, or install appropriate weak link.

Each life float must be individually attached to the painter and long enough that each life float can float without contacting any other life float in the group. Attach each life float individually to the painter.

Vessel cannot operate under sail until third party rigging survey is provided.

Each survival craft on a vessel must be in good working order. Port side 12-man life raft must be repaired or replaced due to a hole.

Provide repair plan and make proper repair of the starboard outboard hull in the engine room to the satisfaction of an attending marine inspector prior to carrying passengers.

Replace port and starboard hardware for the fairlead for the jib prior to operating under sail.
