A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Expired aspirin in first aid kit, replace to the satisfaction of the attending marine inspector. 46 CFR 184.710

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Battery terminals were affixed with wing nuts, replace with permanent nut style connector to the satisfaction of the attending marine inspector. 46 CFR 183.350(c)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Stowage of life jackets not marked, mark stowage of adult/child life jackets to the satisfaction of the attending marine inspector. 46 CFR 185.604(f)

A hatch exposed to the weather must be watertight. Gaskets on port forward and port amidships were loose, repair gasket to the satisfaction of the attending marine inspector. 46 CFR 179.360(a)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel was missing a valid Safety Radio Telephony Certificate. 46 CFR 184.502

Each paddle must be of at least 4 feet in length, lashed to the life float to which it belongs and buoyant. Paddles for both life floats were less than 4 feet in length. Replace existing paddles with appropriate paddles of the proper length. 46 CFR 180.175(f)(2)

The name of a vessel must be marked or painted in clearly legible letters and numbers on each life float and buoyant apparatus. Each life float must meet the requirements in subpart 160.010 of this chapter for a peripheral body type buoyant apparatus. One of vessel's life float's Type II retroreflective material is worn along the body. Replace the affected area's retroreflective material to the satisfaction of the attending marine inspector. 46 CFR 160.027-2(a) 46 CFR 160.010.4(p)

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Port side Reduction gear cooling flexible hose is cracked and worn. Replace hose and install to the satisfaction of the attending marine inspector. Old T 46 CFR 182.01-5

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Exhaust cooling raw water hose for port engine outer rubber jacket wasted and its integrity compromised. Replace hose and install with chafing gear to the satisfaction of the attending marine inspector within seven (7) days of receiving this CG-835V. Old T 46 CFR 182.01-5

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices. Starboard engine reduction gear pressure sensor cable is improperly capped. Reinstall cable in accordance with manufacturer's instructions, properly terminate cable, or completely remove cable. 46 CFR 176.830

Fixed gas fire extinguishing systems are required to be serviced and inspected. Vessel required to provide satisfactory evidence of the required servicing. Port and starboard Fireboy were last serviced OCT 2021. Provide satisfactory evidence of required servicing. 46 CFR 176.810(b)(2)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with a through-hull fitting below the deepest load waterline, such as a lazarette; a machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space; and a space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. The port engine compartment, starboard forward and starboard engine compartment high water alarms did not provide visual indication. Repair or replace the visual indicator for the high water alarms. 46 CFR 182.530(a)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instruction not present on vessel. Affix Emergency instruction at operating station of vessel. 46 CFR 185.510(a)

A Certificate of Documentation is required for the operation of a vessel in certain trades, serves as evidence of vessel nationality, and permits a vessel to be subject to preferred mortgages. A valid Certificate of Documentation was not present. Provide a valid Certificate of Documentation. 46 CFR 67.1

The master of the vessel shall ensure that: The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel did not log test of EPIRB monthly. Vessel representative demonstrated test and logged while present on vessel. 46 CFR 185.728(c) 46 CFR 185.728(c)

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Provide evidence of satisfactory servicing and testing of fixed gas fire extinguishing system to the attending Marine Inspector. 46CFR176.810(b)(2)

Each vessel must have permanent draft marks at each end of the vessel or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum allowable trim and amidships to indicate the maximum allowable draft. No loading or draft marks on the vessel's hull. 46CFR185.603(c)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessels repair proposal for mast and bow beam repairs dated 5/4/2021 has been reviewed and accepted. Vessel owners request from 04/30/21 to operate temporarily without mast or rigging until repairs to the mast are complete is also granted for a period of 21 days. If repairs to the mast exceed June 1st 2021, the vessel must cease operations until repairs are complete or stability calculations are submitted to MSC to document the weight changes and consider minor variations in stability criteria for sailing vessels vs power vessels. The vessel must complete successful sea trials under sail once the mast is stepped prior to operation under sail.

Engine exhaust pipe installations built in accordance with the requirements of ABYC P-1 will be considered as meeting the requirements of this section. Stbd engine exhaust hose corroded. Replace. 46CFR182.430(k)

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI. Vessel only has two fire buckets onboard. Add a third bucket with lanyard. 46CFR181.610

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Provide evidence of monthly checks.

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. Vessel only has six flares on board. Vessel restricted to a Lakes/Bays/Sounds route or 30 min runs. 46CFR180.68

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installa- tion meeting the requirements of § 80.1101(c)(2). VHF radio does not have the correct MMSI information. Update VHF with current MMSI. 47CFR80.905

Each life jacket and ring buoy must be marked in clearly legible block capital letters with the vessel's name. All life jackets currently have the previous vessel name. Provide proper marking and identification on all life jackets. 46CFR185.604

The employer shall provide an Employee Assistance Program (EAP) for all crewmembers. Demonstrate compliance with 16CFR16.401. 46CFR16.401

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel may not place deck hand in the water until the requirements of Work Instruction 31 are met. 46CFR176.840

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. Vessel’s EPIRB is not registered to current owner [name withheld] registration. 47CFR80.1061(f)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Align Preventative Maintenance Plan with NVIC 02-16. 46CFR176.840

02 portable marine fire extinguishers rusted and deemed unfit for service. Provide 02 portable marine fire extinguishers that are not showing signs of rust/wastage.

Magnetic compass light missing/non operational. Provide magnetic compass light.

03 Fire buckets missing lanyard. Provide lanyard for fire buckets.

Jib roller furling system not working as intended/jambed. Conduct maintenance on ruler furler to show proper operation.

Port and stbd forward void deck hatches would not seal properly. Repair in accordance with 46 CFR 171.124.

Port fuel shut-off handle stop was damaged causing valve to close and partially reopen when activated.

Provide Certificate of Documentation in accordance with 46CFR 67.7.

Relocate e-pirb to float free location in accordance with 46 CFR 180.64.

Air intakes to engine room will not seal in event of fire. Seal intakes in accordance with 46CFR 181.420.

Port fuel return leaking. Fix leak in accordance with 46 CFR 182.455.

Stbd fuel shut off hardware is deteriorated. Renew hardware in accordance with 46CFR 182.455.
