Except as allowed by paragraphs (b), (c) and (d) of this section, each bessel certified to operate on a limited coastwise route shall be provided withthe survival craft required by 117.204 (a) through (d) of this part, as applicable. Vessel removed 02 Inflatable Buoyant Apparatus. Vessel is restricted in service to Lakes, Bays, and Sounds until full compliment of IBAs are restored. 46CFR117.205

A stability test of a vessel ma be dispensed with if the Coast Guard determines that a accurate estimate of the vessel's lightweight characteristics. Vessel must submit weight-moment calculations in accordance with Marine Techical Note 4-95 to the Marine Safety Center. Vessel is restricted to Lakes, Bays, and Sounds until calculations are approved by MSC.

Battery terminals shall be permanent type and in good condition. Batteries for M/E were corroded and secured with wing nuts.

Vessel shall be inspected with compliance with the standards required by this subchapter and for items not covered to the satisfactory of the [name withheld] bottles under galley were not secured causing a potential safety hazard.

An exposed hazard such as gears and rotating machinery must be protected by a cover. Both generators had exposed rotating belts causing a safety hazard.

The general construction of the vessel shall be such as to minimize fire hazards. Structural fire protection above port generator was loose/missing.

Auxiliary machinery shall be suitable in type and design for the purposes intended. Starboard generator oil pressure gauge was inoperable.

Each EPIRB must be registered with NOAA before installation and that information be kept up to date. EPIRB NOAA registration did not match.

The operator must secure each type III device in a manner to secure discharge. MI found no method of securing MSD, corrected on the spot.

All electrical installations shall be to the satisfaction of the OCMI. The distribution panel was not labeled correctly. 7 days.

The EPIPRB shall be logged or documented. No monthly test was logged.

Each IBA must be equipped in accordance with the manufactures approved servicing manual. Vessels IBS's were not serviced annually.

Waste receptacles must be constructed of non-combustible materials with no openings. Several plastic receptacles were constructed of plastic and open on top. Crew removed.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Attending marine inspector did not witness satisfactory drill performance from crew. Conduct training and drills to increase crew competency.

All microprocessors or computer base systems must meet the requirements of part 62 in subchapter F of this chapter. Provide line drawing of power supply to throttle controls and do DVTP in accordance with manufacturer's instructions. Provide tech report.

A Voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator that is in operation. Make permanent repair of voltmeter and ammeter. 46 CFR 183.10-5 (Old T)

In general the electrical source located within the vessel and distribution system shall not be grounded. Provide way to detect grounding. 46 CFR 183.10-30 (Old T)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. Make repair to fore-peak bilge pump. 46 CFR 182.25-1 (Old T)

Each vessel is provided with safe, readily available... firefighting equipment of the type necessary to extinguish any fires... Vessel's fire hoses had pinholes and worn threads. Outfit vessel with appropriate hoses. 46 CFR 181.01-5 (Old T)

Each vessel certificated to operate on a rivers route within one mile of land is not required to carry survival craft. Vessel is certificated for LBS and limited coast wise route, but does not have any IBAs on board. Restricted to 1 mile of land. Outfit vessel with IBA in accordance with COI.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be insulated where necessary to prevent injuries. Identified exposed hot surface near wet exhaust flange. Outfit exhaust piping with sufficient insulation. 46 CFR 116.970

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Identified missing engine mounts on port and center propulsion engine. Outfit engine with missing mount. 46 CFR 119.200

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible... Absence of up-to-date navigational charts of an approved type. 46 CFR 121.420

All vessels of less than 100 GRT—one mate (except vessels on voyages not exceeding 12 hours in duration may, if the OCMI determines it to be safe, be operated without mates). IAW NVIC 1-91 and OCMI discretion, NVIC 1-91 allows for the deletion of the required mate if they have 30 days experience on board the vessel and 30 hours at the helm under supervision of a master or mate amongst other stipulations. USCG MI doubts that any of the newly hired, unlicensed personnel have met all the required and/or recommended training. Operator will provide either documentation of licensure for merchant mariners under employ or evidence that the required training to forego the requirement of a mate has been conducted for the all personnel designated in writing as "Senior Deckhand." Until at least one unlicensed crew member has documented employer training meeting the requirements of NVIC 1-91, this vessel will operate with the following manning:1) 150 or less passengers - 1 Master, 2 Deckhands2) 151 or more passengers - 1 Master, 2 Deckhands, 1 Mate 46 § 15.810(b)(4) - Mates; NVIC 1-91, Enclosure (1)(Part IV) Senior Deckhand

install bonding strap on fule fill line at the flex joint to bond the port side proir to using the port side to fill the tank

Install guards over all rotating parts on all engines in the engine room

replace missing installation and metal in engine room port side aft.

repair leaking salt water head supply line in the engine room port side aft

02 light covers in the main accommodation area found damaged. Renew.

STBD Laz: 02 broken welds found on exhaust. Repair.

ER: STBD Main Engine missing belt cover. Renew.

ER: Temperature sensor wires found exposed. Properly secure wires.

Conduct man over board and fire drill to the satisfactory of the attending marine inspector.

A commercial vessel over 65', self propelled shall have a type A-AIS intalled. 33CFR164

Replace retroreflective material on aft deck lifebuoy.

Remove crank case vent piping from overboard discharge ports (port & Stbd). Blank off both overboard port. Route crank case vents to internal system.

Remove pvc piping from sea water systems in engine room and provide metallic piping.
