On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with a through-hull fitting below the deepest load waterline, such as a lazarette. Vessel does not have a visual and audible bilge alarm in their lazarette void space. Crew is to monitor the bilges every 30 minutes while underway. 46 CFR 182.53(a)(1)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel did not have any drill logs onboard. 46 CFR 185.520(f)

Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel did not have any FCC licenses onboard. 47 CFR 80. 13

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel is missing one portable battery light. 46 CFR 183.430

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Markings of vessel's name was faded and needs to be re-marked. 46 CFR 185.604(b)

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Vessel's retroreflective material on the ring life buoy is faded and needs to be replaced. 46 CFR 185.604(i)

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Vessel did not have the lanyard secured around the body of the ring life buoy. 46 CFR 180.70(d)(3)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Upper deck is not marked with the maximum allowable passenger capacity as required. Install a durable level at the entrance to the upper deck indicating "Maximum 12 Passengers", in accordance with the vessel's stability letter. 46 CFR 185.602(g)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of 46 CFR 80.1101(c)(2). VHF radio not interfaced with GPS; unable to transmit position data (lat/long) as required. Ensure GPS is properly connected to VHF to enable DSC position reporting. 47 CFR 80.1101(c)(2)

Stations in the maritime service must be licensed by the FCC. FCC Station license not present for the inspection. Provide valid FCC station license for verification. 47 CFR 80.13(a)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: the radio operator must hold a marine radio operator permit or higher class license. MROP was not present for the inspection. Provide valid permit for verification.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with 47 CFR 80.59 once every five years. The technician will issue a Communications Act Safety Radiotelephony Certificate. Radiotelephony certificate was not present for the inspection. Provide valid certificate for verification. 47 CFR 80.903

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date: Charts; Sailing directions; Coast pilots; Light lists; Notices to mariners; Tide tables; Current tables; and all other nautical publications necessary. Charts were outdated, and several required navigational publications were missing or not readily available. Obtain and maintain up-to-date nautical charts (either electronic or paper formats are acceptable) and ensure all required navigation publications are onboard. 46 CFR 184.420

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The stowage compartments for adult life jackets were not labeled to indicate the quantity or size of life jackets contained within. Label all life jacket stowage compartments clearly to indicate both the number of life jackets stored and their designated size. 46 CFR 185.604(f)

Remote fuel shutoff stations shall be marked in at least 1-inch letters indicating purpose of the valve and direction of the operation. Fuel shutoff valve not labeled to indicate direction of operation. Label must be installed. 46 CFR 185.30-20

All applicants for an original officer endorsement, must provide to the Coast Guard: (i) The American National Red Cross Standard First Aid course or American National Red Cross Community First Aid & Safety course; or (ii) A Coast Guard-approved first-aid course; and(2) A currently valid certificate of completion of a CPR course from either—(i) The American National Red Cross; (ii) The American Heart Association; or (iii) A Coast Guard-approved CPR course. The vessel operator did not possess a valid certificate of completion for first-aid and cardiopulmonary resuscitation (CPR) training. Obtain and maintain current certification in both first-aid and CPR from an approved provider. Provide valid proof of certification to the Coast Guard to meet the requirements for officer endorsement eligibility. 46 CFR 11.201(i)

Marine employers must establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers. DAPI program records were not present at the inspection. Provide records for verification. 46 CFR 16.230(a)

No record of required drills were available for review at the time of inspection. Supply/maintain a written log documenting date/time and general description of each drill conducted aboard the vessel. 46 CFR 185.520(f)

All vessels shall be provided with an approved adult type life preserver for each person carried. Marine inspector found 29 unserviceable adult PFDs due to cracking in the outer shell exposing internal foam. Vessel is limited to only carry 8 adult pax and 5 child pax. 46 CFR 180.25-5

Each PFD must have at least 200 sq. cm. (31 sq. in.) of retroreflective material on its front side, at least 200 sq. cm. on its back side and at least 200 sq. cm. of material on each reversible side, if any. Retoreflective material needs to be replaced on all PFDs. 46 CFR 160.077-13c

Electrical installations on vessels shall be such that services essential for safety will be maintained under various emergency conditions and the safety of passengers, crew and vessel from electrical hazards will be assured. Vessel had 04 (four) exposed dead ends and wires with corroded outer sheath exposing wire. 46 CFR 111.05-15(a)

Alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] installed two power poles on the vessel to anchor the vessel in shallow water with the intent of allowing passengers on the upper deck. The vessel is prohibited from carrying passengers on the upper deck under any condition including ,but not limited to, underway, moored, at anchor, or aground. For further consideration regarding passengers on the upper deck, submit a proposal to the cognizant OCMI to reevaluate stability characteristics.

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines. Port side fuel tank was missing valve handle, preventing the ability to shut off fuel. Rectify Prior to the Carriage of Passengers and Prior to Movement. 46 CFR 182.15-49(3)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station. Vessel's high-water bilge alarm did not produce an audible alarm at the operating station. Rectify Prior to the Carriage of Passengers and Prior to Movement. 46 CFR 182.530(a)

Except as allowed by paragraph (d) of this section, each life jacket must be approved in accordance with 46 CFR 164.018. Rectify Prior to the Carriage of Passengers. 46 CFR 180.71(c)

A vessel must comply with the applicable requirements for any radio, including the requirements for installation certificate. Radio must be installed to the satisfaction of an FCC Radio Technician. 46 CFR 184.502

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel's name is illegible and not clearly marked. 46 CFR 185.604(b)

All lights, receptacles and switches exposed to the weather shall be watertight. Light switch bezel is missing 02 rocker switches, exposing electrical wiring and adjacent switches to elements and risk of electrical shock. 46 CFR 183.05-35(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot and Coast Guard Light List. Vessel is missing U.S. Coast Pilot and is carrying an outdated Coast Guard Light List. 46 CFR 184.420(a)

For each marine casualty required to be reported by 185.202, the owner, agent, master, or person in charge of the vessel shall determine whether there is any evidence of alcohol or drug use by individuals directly involved in the casualty. Alcohol test strips on board are expired. Provide new, valid alcohol test trips before vessel carries passengers. 46 CFR 185.210 (a)

Wiring, where subject to mechanical damage, shall be protected IAW the national electric code, Art 352 for metal raceways, Art 436 for conduits, & Art 348 for metal tubing. Wiring in starboard head was not properly secured. Prove proper securing of wires. 46 CFR 183.05-50 (a)(2)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking at least 1 inch numbers and letters at the entranceway to that deck. Vessel had no markings restricting passengers from the upper deck. Provide markings that restrict passengers from upper deck. 46 CFR 185.602 (g)

Proposed alterations shall be approved by the OCMI, before work is started. Drawings, sketches, or written specifications describing the alterations in detail may be required by the OCMI. Vessel is altering fuel tank arrangements. Provide construction plans before commencing alterations. 46 CFR 176.20-1(b)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] alterations must be approved by the OCMI before work is started. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Vessel reported a weld fracture causing a slow leak in the vicinity of the vessel's keel.

An enclosed space containing machinery powered by gasoline must be equipped with a flammable vapor detection device in compliance with 182.480. Flammable vapor detection device was inoperable. Vessel must have a flammable vapor detection device in compliance with 182.480 prior to the carriage of passengers.

At each inspection for certification and at every other vessel inspection all observed unsafe practices and fire hazards must be corrected. Starboard fuel line is cracking/wasted. Fuel line must be replaced where wastage exists.

For each marine casualty required to be reported, the owner, agent, master, or person in charge of the vessel shall determine whether there is any evidence of alcohol use. Alcohol testing devices provided on board vessel were expired. Alcohol testing devices on board vessel must be valid and ready for use.

An enclosed space containing machinery powered by gasoline must be equipped with a flammable vapor detection device. The vapor detection device in the lazerette is not working.

Procedures for checking the proper operation of a flammable vapor detection system must be posted at the primary operating station. No instructions are posted.

Bilge system- On a vessel of at least 26 feet a visual alarm and audible alarm must be provided at the operating station to indicate a high water. The audible alarm is not working but the light is. Repair the audible alarm.

Medical first aid- training in personal survival tech as set out in STCW.. Including NVIC 1-91 50% of small passenger vessel crew must have valid CPR training. Currently both members of crew don't hold a valid CPR course.

Damage was found in the radar support framing, upper deck house railing, and forward cabin door safety glass. An assessment of all repairs plans will be conducted to the satisfaction of the attending marine inspector.

COD not current. Provide proof or renewal prior to carrying passengers.

Radio safety certificate expired. Provide copy prior to sailing more than 1000ft from shore.

Provide means of communication with deckhand and captain during emergencies.
