Vessel required SFP in engine room. Insulation found disturbed in multiple locations in overhead of engine room.

Your vessel must undergo an annual inspection within 3 months before or after the anniversary date. Vessel annual inspection due no later than 21JUL25.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] room escape is not marked appropriately. Current arrangement of carpet is a hindrance in emergency situation.

A power driven vessel underway shall exhibit a masthead light forward visible at a minimum of 6 miles. Light visibility is decreased due to opacity of the material.

After loading and prior to departure, and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessel's stability letter. Vessel has assigned minimum freeboard measurements on stability letter that are not able to be readily corresponded to draft readings. Provide a plan that depicts maximum drafts for review by the Marine Safety Center for inclusion on stability letter.

Conductors must be protected in accordance with their current carrying capacities. The steam table breaker has been replaced with a circuit breaker of larger capacity than designed.

A voltmeter and an ammeter shall be provided that can be used for measuring voltage and current of each generator that is in operation. For alternating-current generators a means for measuring frequency shall also be provided. THOMAS LAIGHTON currently is not fitted with means to measure these operating parameters. Install required meters. 46 CFR 183.10-5(g)

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The THOMAS LAIGHTON currently does not have a valid FCC Bridge to Bridge Certificate. Provide valid certificate as required.

Provision shall be made by means of plugs, covers, etc., to prevent the admission of air into the space protected The THOMAS LAIGHTON has an installed fixed fire extinguishing system, but a ventilation duct from the protected space has no means of closure to prevent admission of air into the protected space, or to prevent the escape of the extinguishing agent. Provide a proposal by 23JUN2021 to install a means of closure for the ventilation duct, and install to the satisfaction of MI only after receiving approval. Also provide satisfactory proof that volume of extinguishing agent is sufficient for the entire protected space, including the ventilation duct up to the point of closure. 46 CFR 181.25-1
