46 CFR 176.310(a): A Certification Expiration [name withheld] indicates the date upon which the vessel's Certificate of Inspection expires. The inspection sticker on the bridge was placed on top on an old inspection sticker, and the expiration date was unreadable. The inspection sticker needs to be removed.

46 CFR 185.604 (b) Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The ring life buoy location on the top deck did not have the vessel's name marked on it. Mark the vessels name on the life buoy in accordance with the requirements in this section.

46 CFR 179.360(a) A hatch exposed to the weather must be watertight. The seal on the watertight hatch going into the steering gear room fell off upon opening the hatch. The seal must be replaced or reattached.

46 CFR 176.802(a)(2) Inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves. The port side intake in the engine room was leaking. Test the seal to verify watertight integrity

46 CFR 179.320 (c): A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Multiple wires penetrating through the bulkhead in the engine room were not watertight. Reseal the penetrations to be made watertight.

46 CFR 176.802(a)(2) Inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves. Excessive water was discovered in the steering gear space. Check through hull fitting for watertight integrity.

182.15-20(d) Where exhaust piping passes through watertight bulkheads the watertight integrity of the bulkhead shall be maintained. Non-combustible packing shall be used in bulkhead penetration glands. Water cooled exhaust pipes may be welded to steel bulkheads in way of penetrations if suitable arrangements are provided to relieve the stresses resulting from the expansion of the exhaust piping. Starboard Main Diesel Engine (MDE) exhaust is leaking water where flexible hose and aluminum exhaust port meet due to a potential crack in the aluminum or flexible hose. Identify where water intrusion is coming from and make repairs in kind. Vessel shall not operate until exhaust leak is repaired.

(d) Where exhaust piping passes through watertight bulkheads the watertight integrity of the bulkhead shall be maintained. Non-combustible packing shall be used in bulkhead penetration glands. Water cooled exhaust pipes may be welded to steel bulkheads in way of penetrations if suitable arrangements are provided to relieve the stresses resulting from the expansion of the exhaust piping. Generator exhaust is leaking water from fatigue cracks on the vernalift (discharge side) in the lazarette. Vernalift will need to be replaced in kind. Vessel shall not operate until exhaust leak is repaired.

182.15-15(b)(3) That part of the exhaust system between the point of cooling water injection and the engine manifold shall be water-jacketed or effectively insulated. Exhaust insulation in engine room needs to be properly installed on exhaust piping. Wiring between insulation pieces are missing throughout engine room.

Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose. Flexible hosed used on port MDE at bilge pump is missing a hose clamp, install a second hose clamp to ensure two hose clamps per fitting.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguisher in engine room needs to be located closer to the ladder accessing the space.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. Masthead light is frosted due to sun exposure, lens needs to be replaced. Multiple navigation lights have condensation in lens due to broken gaskets.

The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Need to update vessel hailing port.

(a) At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Miscellaneous lights throughout vessel missing light covers.

(a) At each initial and subsequent inspection for certification and reinspection the -marine inspector shall examine and test the main and auxiliary steering apparatus to the extent necessary to determine that its condition is satisfactory and that it is fit for the service intended. The main and auxiliary steering apparatus shall be tested by operation and the inspector shall examine the mechanism thereof throughout its length. Rod end bearing on push rod where it meets the tiller arm is seized and beginning to wear on itself causing metal shavings to accumulate on tiller arm. Both rod arm bearings need to be replaced in kind.

The life floats stored on the aft portion of the upper deck have small pinholes from birds pecking at the cover. Repair life floats with authorized repair kit prior to the carriage of passengers on a route in excess of 1 mile from shore.

Port navigational running light not illuminated. Correct as required by navigational regulations.

Bilge suction line for the forepeak had a tear in it due to the line not being able to properly drain. A tee will be installed during the repair to faciliate drainage.

Galvanized elbow on the outboard side of the MSD was corroded. Owner to renew elbow.

Temp repair made to the machinery space bilge piping. Make permenent repairs prior to carrying passengers in 2021.

Wet exhaust flexable pipe needs 2 hose clamps.

EPIRB registration & battery are expired. Vessel is limited to no more than 3 miles from shore until a new EPRIB registration and battery are verified.

Several paddles are broken and all are far less than 4' as required. Also, paddles are not lashed to the life floats. Correct within two weeks. Until then vessel is limited to no more than 1 mile from shore.

Two lights do not have approval numbers on them and at least one is broken and not operable. Also, lines connecting the lights to the life floats are too short and must be at least 18'. Two weeks to correct. Until then vessel is limited to no more than 1 mile from shore.

Vessel does not have alcohol testing equipment on board for use. Provide approved means for SMI testing requirements.

Supervisor not trained for EAP at least one hour. Obtain at least one hour of EAP training to meet regulatory requirements.

Vessel does not have thier drug testing program policy statement posted.

Company has not submitted last years MIS reports to the CG as required. Submit MIS report within 2 weeks.

Vessel does not have any MOB, Fire, Abandon ship drills logged. Also, vessel is not logging EIPRIB checks. Vessel required to have a log book or other documentation process implemented on board for the tracking of all required log entries.

Operator could not provide a current radio safety certificate. An expired cert from 2017 was found but no current cert could be found. Have radios serviced and provide a new safety radio certificate.

Vessel did not have up to date nav pubs such as Coast Pilot, Light list, Notice to Mariners, or up to date charts for operating area. Provide all required nav pubs and charts.

Vessel was unable to get underway at time of inspection. Drills must be conducted in presence of CG inspector for credit for annual inspection.

Removed one Adult life Jacket no longer in good servable condition. Renew one Type I USCG Approved Life Jacket iaw 46 CFR 180.71

Noted several of the main deck interior seating/benches were loose and not properly secured. Secure all loose seating/benches in the main passenger cabin iaw 46 CFR 177.820

Observed loose chunks of the overhead foam in the #2 Compartment bilge. Remove loose chunks of foam iaw 46 CFR 177.700

Observed one electrical cable suffering tube was not water tight. Properly seal the loose electrical cable penetrating thru engine room, located fwd bulkhead on port side iaw 46 CFR 179.360

Observed two of the engine room florescent light fixture covers were warped. Renew covers or renew light fixtures iaw 46 CFR 183.410

First Aid kit, Iodine expired, renew iaw 46 CFR 184.710

Engine room bilge alarm, faint audible indication. Repair or renew the Pilot Houses, Engine room bilge alarm iaw 46 CFR 182.530

Observed weak-link for the aft life floats was rusted, broken and no longer serviceable. Renew and install an approved weak-link on the aft life floats IAW 46CFR 180.137(a).

Observed line/painter connecting each life float was of insufficient length (8ft). Re-install with adequate line/painter, 20 ft line on aft life floats and 30 ft line on pilot house life floats per 46CFR180.137(e)(3).

Observed EPIRB Hydro release expires in June 2016. Renewed EPIRB hydro static release on or before 01JULY2016.

Observed two light fixtures, one fluorescent, one incandescent, on the aft main deck in way of the aft stairs were filled with water. Repair both fixtures to prevent water intrusion per 46CFR 183.410

Observed wires exposed on the vessels electrical shore tie cable. Make repairs to correct and eliminate the exposed wire per 46CFR183.340

Observed the pilot house life float ores/paddles were not lashed to life floats. Lash ores/paddles to the life floats per 46CFR180.175(f)(2).

Observed fire pump (inboard of the stbd main engine) was leaking water, and cover missing a couple of screws. Make repairs, add missing screws and eliminate water leakage from the fire pump IAW 46CFR176.816

Observed on the generator, a fuel hose out covering was worn. Renew the worn fuel oil hose, with an approved hose IAW 46 CFR 182.720

Observed the generator frequency meter at the operating station/pilot house was reading 56 HZ. Adjust or repair the generator frequency meter per 46CFR183.200
