Each life jacket must be marked with Type I retro-reflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. Majority of adult life jackets had retro-reflective material that was very worn. Prior to 02Apr2025, 60 adult and 12 children life jackets retro-reflective material must be replaced/repaired to operate/carry passengers on board the vessel. Once the 60 adult and 12 children life jackets are repaired, the vessel can ONLY carry that amount of passengers until the remaining life jackets are repaired. Owner/Operator must also provide a repair plan for repairing the remainder of the vessel's life jackets and how soon they will accomplish this task. 46 CFR 185.604(h)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. 02 of the 07 life float lights need a new battery installed. 46 CFR 180.175(d)

A vessel must carry one floating waterlight. The waterlight did not have a battery in it at the time of the inspection. Battery must be installed and operator must show that waterlight operates as intended. 46 CFR 180.70(d)

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Owner/Operator was conducting maintenance on vessel's survival craft, but was not conducting it IAW manufacturer's instructions nor were they documenting the maintenance. Owner/Operator must provide CG/MI with a way to document the maintenance. 46 CFR 185.702(d)

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Owner/Operator had the annual servicing tags on all extinguishers and was conducting monthly inspections of the extinguishers, but was not documenting the inspection. Owner/Operator must start documenting the inspection. 46 CFR 176.810(b)(ii)

The shutoff valve at the tank shall be operable from outside the tank compartment. Shutoff valves would not fully close when operated from outside the compartment.

Dry pipe vertical exhausts are permissible if suitably insulated. Exhaust pipe not full insulated.

The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Hailing port not marked.

(a)(2)All wiring and cable shall be suitable for marine service and for the particular installation used. In addition, flexible or portable cords such as SO, STO, and similar types cannot be used for permanently installed wiring. Pilot house has mini refrigerator and cell phone charger plugged into surge protector portable cord. Cite 46 CFR 183.10-20

(2)Cables entering boxes or fittings shall be protected from abrasion and shall conform with the following requirements: The cable armor shall be secured to the box or fitting. Fire pump junction box wiring is missing a stuffing tube. Cite 46 CFR 183.10-20(f)

(d) The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screen shall consist of a single screen of corrosion resistant wire of at least 30x30 mesh. The flame screen or flame arrester shall be of such size and design as to prevent reduction in the net cross-sectional area of the vent pipe and permit cleaning or renewal of the flame screens or arrester elements. Port and Starboard fuel vent pipe flame screen are 30% covered with white paint. 46 CFR 182.20-35

(b)(2)Fuel lines shall be accessible, protected from mechanical injury, and effectively secured against excessive movement and vibration by the use of soft non-ferrous metal straps with no sharp edges. Where passing though bulkheads, fuel lines shall be protected by close fitting ferrules or stuffing boxes. Port Main diesel engine high pressure fuel line bracket bolt is broken causing chaffing on metallic fuel line from vibration. Fuel line supply from from Port fuel tank to Port MDE is chaffing on metal engine room centerline catwalk. 46 CFR 182.20-40

Cite 46 CFR 183.01-15 (a) All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Port Main Diesel engine Jacket Water float switch cover was missing which exposed wiring.

Cite 46 CFR 185.20-35 As appropriate for the intended voyage, all vessels must carry adequate and up-to-date- (a) Charts, (b) Sailing directions, (c) Coast Pilots, (d) Light list, (e) Notice to Mariners, (f) Tide tables, and (h) all other nautical publications necessary. Vessel did not have currant Navigation Rules publication. Navigations Charts were not updated.

33 CFR 159.7 (C) When operating on a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each type III device in a manner which prevents discharge of sewage. Sewage was seeping from sewage PVC piping into bilge aft of Starboard sewage holding tank.

Cite 46 CFR 183.05-20 (C) Battery terminals shall be of the soldered type and all connections shall be made to the battery terminals with permanent type connectors. Spring clips or other temporary type clamps are not permitted. Engine room battery bank connections are made with wing nuts.

Placards containing instructions for donning life jackets must be posted in conspicuous places regularly accessible and visible to crew and passengers. Placard should be placed on the upper deck near the life jacket storage area.

Each vessel that operates on the high seas must have on board an EPIRB. The vessel's EPIRB had an expired battery.

The master of each vessel shall ensure that each EPIRB must be tested monthly and that test shall be logged or otherwise documented properly.

No vessel subject to inspection will be navigated unless it is under the direction and control of an individual who holds an appropriate license or officer endorsement on his or her MMC. The master did not have his original MMC onboard the vessel.

The COI must be posted under transparent material in a conspicuous place where observation by passengers is likely. COI was not posted in pax area.

Found concentrated pits on shaft packing aluminum spool, schedule 80, (.5 inch). water intrusion was noted from the deepest pitted area. On opposite side of the same spool there is another pit concentration of approximately 1/4 inch. Make repairs to satisfaction of attending MI.

Provide proof of merchant mariner credential.

Prove proof of enrollment in drug testing program.

Not to carry passengers until Coast Guard Marine Inspector verified operation of bilge pump, 46CFR182.510.

Affect temporary repair to pitting in forepeak with epoxy compound, and install electrical automatic bilge pump with audible indicator in pilot house no later than 24APR17. Permanent repair shall be completed at next haul out.
