All vessels shall have efficient guard rails or bulwarks on decks and bridges. Starboard side top guard rail adjacent from #3 hold found wasted and separated from stanchion. 40ac

The construction of the vessel shall be such as to minimize fire hazards insofar as its reasonable and practicable. Main engine high pressure fuel lines double jacketed found disconnected at main engine.

Boundary bulkheads separating accommodations and control stations from cargo and machinery spaces shall be of "A" class construction. Found door into engine room through an "A" class boundary unable to fully self close.

All enclosed spaces within the vessel shall be properly vented or ventilated. found ventilation trunks in pump room severely wasted. 30c.

The construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Found plastic bag tied around smoke detector in engine room workshop. 10c.

Prior to installation aboard ship, diagrams of the following systems must be submitted for approval; Bilge piping and sanitary drains. Found 02 double diaphragm pumps installed for permanent use in bilge and grey water systems. These installations are not part of an approved plan. 50c.

Each vessel must carry not less than 12 rocket parachute flares, and stow the flares on or near the navigation bridge. Vessel on has 06 rocket parachute flares on the bridge. 40ac.

At each inspection for certification and periodic inspection, the quarters, toilets, lockers, ETC. Shall be examined by the inspector to be assured that they are in a sanitary condition. Found soft greasy patch on potable water piping. 40ac.

Operating generators must provide a continuous and uninterrupted source of power for the ship's service load under normal operational conditions. The vessel allided with BNSF Taconite facility shuttles due to an inoperable number two ship service generator, which resulted in an overload and trip of the #1 and #3 ship service generators. The vessel shall complete repairs to satisfaction of class society and Coast Guard prior to departure from the port. Until deficiency is rectified, vessel's movement is restricted by conditions in COTP Order 03-2024.

In general, compliance with the standards established by the American Bureau of Shipping, will be considered as satisfactory evidence of the structural efficiency of the vessel. Vessel allided with BNSF Taconite facility shuttles. U.S Coast Guard inspectors attended the vessel and identified related damage onboard as follows: a 13 foot long pierced gouge inset approximately 30 feet above the waterline in way of the port windlass space. The damage extends across 7 frame bays. There are 2 distinct gouges, 1-2 inches wide, approximately 5 inches apart. The gouges penetrate through the hull within the first 4 frame bays and 2 frames are tripped.

Remote bilge level sensors, where provided, must be located to detect flooding at an early stage and to provide redundant coverage. 03 Bilge alarms found inoperable within engine room. 16ac

The steering-gear compartment must include handrails and either gratings or other non-slip surfaces to ensure a safe working environment if hydraulic fluid leaks. Vessel did not have any grating or non-slip surface to ensure a safe working environment. 50ac

Vessel grounded due to a crack in the Spring Air Piping on the Main Engine Exhaust Valve.

Heavy fuel oil steadily seeping from fuel injector valve on #6 cylinder to M/E.

Large amounts of heavy fuel oil pooled around #1 and #2 fuel pumps supplying the M/E.

The port lifeboat was damaged while transiting the Seaway, the fender was torn off and needs repaired.

Port radar is inoperative, does not display any picture.

5 inch fracture noted in fire main as well as soft patch 18 inches further down the line.

Final 18 inches of hose was bursted causing hose to be unusable during fire drill.

Valve packing on port side aft main deck hydrant had failed and was leaking profusely.

EEBD's were below the green zone. Minimum pressure required was 200 psi but one was at 175psi and the other at 125psi.

Overboard discharge valve was not secured and vessel was discharging grey-water.

Engine control room emergency escape door was misaligned and did't open.

VESSEL DID NOT HAVE MECHANICAL CLOSURE DEVICES ON POLLUTION CONTAINMENT. WOOD PLUGS WERE USED INSTEAD OF THE REQUIRED MECHANICAL CLOSURES. THIS DISCREPANCY WAS CORRECTED BEFORE DEPARTURE.

The fuel vent containment on both the port and starboard side of the vesselwere missing a mechanical means of draining.

SOLAS III: Each fire box on the vessel was missing the key used to open the box.

SOLAS III: Life rings on the Port and Starboard bridge wings and on the port and starboard side of the poopdeck wasted.
