A continuous leak was discovered in the aft stern-tube located in the #4 compartment shaft void (between FR's 40/42, approximately 2' aft of the stuffing box). Provide written repair proposal/plan and submit to MSU Duluth. Advance notification must be provided to MSU Duluth prior to moving vessel. Permanent repairs must be completed and approved by the USCG before carriage of passengers.

During test of the vessel's auto-deck sprinkler system it was noted that the check-valve located in the discharge line from the sprinkler pump was intermittently malfunctioning. Repair sprinkler system to full functionality and demonstrate satisfactory operation.

A continuous leak was discovered in the aft stern-tube located in the #4 compartment shaft void between frames 40 and 42. MSU Duluth OCMI has approved the operator's written proposal to affect temporary repairs via a 10" H x 19" W x 23" L (each bay) concrete box patch repair IWO the affected area. Permanent stern-tube repair must be completed, and approved, prior to the 2022 operational season. Vessel is prohibited from carrying passengers until the temporary repair is examined, and approved, by a USCG Marine Inspector.

Resolution: After diagnostic testing, it was determined that the loss of steering was a result of a faulty bridge selector switch. New selector switch has been installed and tested to the satisfaction of the attending Coast Guard Marine Inspector.
