Cable or wire serving vital systems listed in § 182.710 of this chapter or emergency loads must be routed as far as practicable from high risk fire areas, such as galleys, laundries, and machinery spaces. Wires for the fuel filter pumps were spliced next to the pumps not in an enclosure. A fire could result from fuel leaking onto the live wires. Wire connections must be relocated away from the pumps and placed in an vapor sealed enclosure. 46 CFR 183.340(e)

Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. 02 windows on the PORT side main passenger deck are cracked and need to be replaced. 46 CFR 177.1020

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. This subpart summarizes the responsibilities of credentialed mariners, marine employers, MRO, SAP and other chemical testing service providers in 49 CFR part 40. Vessel did not have an DAPI program, must provide proof of program. 46 CFR 16.113

The vessel owner or operator must pay the annual vessel inspection fee each year on or before the vessel's user fee anniversary date, unless the fee has been prepaid under § 2.10-105 of this subpart. Vessel annual fee for 2024 was not paid. 46 CFR 2.10-101(b)

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Monthly inspection & maintenance of portable fire extinguishers was not be conducted or logged, issue must be addressed to the satisfaction of the attending MI. 46 CFR 176.810(b)(1)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Cable access port in the pilot house allows water to pass through to the passenger area which could create an unsafe condition. 46 CFR 176.830(a)

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Provide proof that waste receptacles in the restroom are noncombustible or replace. 46 CFR 177.405(f)

Stability information (stability details indicated on the Certificate of Inspection, a stability letter, or a stability booklet), is required on certain vessels by paragraphs (b), (c), or (d) of this section. Enough stability information, including stability calculations and assumptions made to use them, must be provided to allow the master to be able to determine operating guidelines. Anti-freeze was found in the engine room and battery compartment bilges. This is in violation of vessel's Stability Letter which stating "bilges and voids shall be kept pumped to minimum content at all times consistent with pollution prevention requirement." 46 CFR 178.210(a)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Waterlight instruction requires that the battery be replaced every two years. Provide proof battery was purchased within the past two years or replace. 46 CFR 180.70(d)

Each lifejacket must be approved under approval series in subparts 160.002, 160.005, 160.055, 160.115, 160.176, or 160.255 in part 160 of this chapter, or other standard specified by the Commandant. Reflective material on multiple lifejackets require servicing/replacement. 46 CFR 180.71(c)

In systems and applications where flexible hoses are permitted to be clamped:(1) Double hose clamping is required where practicable. Alternate bilge pump flexible hose on the STBD MDE was not connected with double claps. 46 CFR 182.410(d)(1)

All electrical equipment and circuits must be clearly marked and identified. Circuits on the switchboard & distribution panel need to be identified. 46 CFR 183.220(d)

Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Cables in the compartment by the pilot house that stores the lifejackets and in the engine room were loose/hanging and need to be properly mounted. 46 CFR 183.340(b)(4)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Wires on the STBD MDE alternate bilge pump were not in an enclosure. 46 CFR 183.340(g)

If the batteries are not adjacent to a distribution panel or switchboard that distributes power to the lighting, motor, and appliance circuits, the battery lead must have a fuse in series as close as practicable to the battery. Battery breakers are located inside of the battery enclosure box and are hanging below the terminal connections for the batteries. The breakers being inside the battery enclosure it is meant to protect is not practicable & should be mounted outside of the enclosure. 46 CFR 183.350(g)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training and general description of the drill scenario. Drills have not been conducted or logged since 29Jul2024. 46 CFR 185.520(f)

The name of a vessel must be marked or painted in clearly legible letters and numbers on each life float and buoyant apparatus. 01 Life Buoy label damaged. 46 CFR 185.604(a)(2)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. 46 CFR 183.430

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. The flame screens shall consist of a single screen of corrosion resistant wire of at least 30x30 mesh. Vessel's fuel tank vent did not have a flame screen installed, must rectify. 46 CFR 182.20-35(d)

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Vessel is new to zone and did not have full crew for table-top discussion regarding abandon ship and man overboard drills. Vessel must have a full crew (01 Master and 02 Deckhands) to discuss abandon ship and man overboard drills/scenarios to the satisfaction of the attending MI. 46 CFR 185.520(a)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Vessel is new to zone and did not have full crew for table-top discuss regarding fire fighting drills. Vessel must have a full crew (01 Master and 02 Deckhands) to discuss fire fighting drills/scenarios to the satisfaction of the attending MI. 46 CFR 185.524(a)

Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel did not have logs with drills documented on board, must establish logs. 46 CFR 185.524(d)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Vessel's confirmation of enrollment consortium certificate expired on 30JUN2024, must provide MI with new certificate.

The lights prescribed in these Rules shall have an intensity as specified in Annex I to these Rules (33 CFR part 84), so as to be visible at the following minimum ranges of 3 miles. Stern light not illuminating. 33 CFR 83.22(a)(iii)

Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Main switchboard missing knock-out plates, allowing possible electrical shock to crew. Must be corrected. 46 CFR 183.200(b)

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. The amount of life jackets were not mark at any of the stowage locations, must be addressed. 46 CFR 185.604(f)

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck at the entranceway to that deck. Markings for upper deck passenger capacity were not present, must install. 46 CFR 185.602(g)

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel did not have logs with drills documented on board, must establish logs. 46 CFR 176.810(b)(ii)

Vital systems piping shall meet Sub F in accordance with 46 CFR 182.40-5. Non-metallic Material is present in fire/bilge piping. To be repaired at next drydock.

The space forward of the collision bulkhead does not have bilge suction connecting to the bilge piping system. Equipment muste be provided to remove water from the space.

The hinges on the door to the Lifejacket Stowage cabinet on the port side of the upper deck was unserviceable.

Electrical cables within swtichboard area was fastened by plastic fastener, require to replace with metallic version throughout the vessel.

Ring buoy light battery cracked. Replace prior to carrying pax. 46CFR185.726

Fuel line on port engine not covered with synthetic fubber or other suitable material. Replace hose with metallic hose or hose meeting SAE J-1942. 46CFR182.20-40

Lube oil lines on main engines not covered with synthetic rubber or other suitable material. Replace with hose meeting SAE J-1942. 46CFR182.20-40

Distribution pannel missing breaker/spare insert. Replace breaker or insert spare. 46CFR183.330

ELECTRICAL CABLES ARE SECURED WITH ZIP TIES. PROVIDE PROPERLY INSTALLED METAL CLIPS ON ELECTRICAL CABLES ALL THROUGHOUT VESSEL TO THE SATISFACTION OF MARINE INSPECTOR.

Raw water suction strainger was clogged with vegetation and starved raw water impellor with cooling water.

Vessel required to repair by crop and renew the areas of hull plating identified as wasted and pitted as identified in yard package.

Inspect and replace all bilge suction strainers, as needed. 46 CFR 182.510b

Beverage conduit in hold #4, starboard side is wasted and holed at elbow. Conduit is open on the other end in hold #2. Conduit must be made watertight to prevent progressive flooding. Repair to the satisfaction of the attending USCG Marine Inspector by 26 March 2004.

UNABLE TO TEST FORE PEAK HIGH BILGE ALARM DURING COI DUE TO INACCESSIBILITY OF SPACE. OPERATIONALLY TEST FORE PEAK HIGH LEVEL BILGE ALARM DURING NEXT DRYDOCK INSPECTION WHEN SPACE IS OPEN FOR INTERNALS, BUT NLT 28 FEB 2003.

Install operational compass.

Install rudder angle indicator.

Prior to carrying passengers, provide approved stability letter to OCMI.

DURING INSPECTION FOR COI IT WAS NOTED THAT STABILITY LETTER WAS NOT SIGNED. VSL REP. TO ACQUIRE SIGNED STABILITY LETTER AND POST ON BRIDGE.

MUST REPLACE OR MAKE PROPER REPAIRS NECESSARY FOR SATISFACTORY OPERATION OFTHE POWER OPERATED VENTILATOR, LOCATED IN THE MACHINERY SPACE.

POST THE VESSEL'S STABILITY LETTER UNDER GLASS OR OTHER TRANSPARENT MATERIAL IN THE PILOTHOUSE OF THE VESSEL.
