Vessels described by 46 CFR 175.110(c) (vessels operating on a coast wise route) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Marine inspector observed no interconnected smoke detector within machinery space. Vessel shall install a UL 217 approved interconnected smoke detector within the machinery space. 46 CFR 181.405 (c)

The alcohol testing devices need not be carried on board each vessel if obtaining the devices and conducting the required alcohol tests can be accomplished within 2 hours from the time of occurrence of the SMI. Marine Inspector observed vessel missing alcohol swabs on board. Vessel shall placed alcohol swabs on the vessel. 46 CFR 4.06-15(a)(3)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Marine Inspector observed paper chart of operating area not up-to-date. 46 CFR 4.06-15

Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Marine inspector observed exposed wires from generator to breaker box and surrounding equipment, to include electrical connections with wingnuts. Vessel shall ensure no exposed wires and electrical hazards are removed.

Electrical equipment used in the following locations must be dripproof: A machinery space. Marine inspector observed electrical outlets that were not drip-proof. Vessel shall replace all exposed outlets within machinery space. 46 CFR 183.210(a)

A generator or motor designed for 40 °C (104 °F) may be used in 50 °C (122 °F) ambient locations provided the generator or motor is derated to 80 percent of the full load rating, and the rating or setting of the overcurrent devices is reduced accordingly. Vessel shall derate their generator. 46 CFR 183.320 (b)(2)

The intent of a radio installation is to establish communications during emergencies. The vessel’s route allows the vessel to go out up to 100 NMs from shore. The vessels SSB radio is not equipped with a DSC alert; therefore, the vessel is unable to contact anyone during emergencies as the CG and vessels with GMDSS only rely on the SSB DSC to hear distress calls. Replace vessels SSB with a model containing DSC capability. In addition, the vessel does not have a reserve power supply. Install appropriate reserve power supply or as an acceptable alternative, provide a VHF FM handheld radio with appropriate charger. The FCC issuing authority for the Radiotelephony Certificate utilized an outdated “How to Conduct an Inspection of a Small Passenger Vessel” checklist. Provide new Radiotelephony Certificate using the updated checklist, revision 2021.02.24. 46 CFR 184.502

The stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. 46 CFR 176.306

A vessel must be equipped with bilge pumps that meet UL 1113 requirements. Replace bilge pumps to meet standard. 46 CFR 182. 520 (e)(1)

At each inspection for certification and at every other vessel inspection all observed unsafe practices. Marine inspector observed crack near ladder well to pilot house.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel has expired alcohol testing devices on board. Vessel is restricted to voyages that allow alcohol tests to be accomplished within two hours of a serious marine incident. 46 CFR 4.06-15(a)(1)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. 01 children’s lifejacket had deteriorated retroreflective tape. Vessel is restricted to carrying 02 children until corrected. 46 CFR 185.604(h)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Vessel missing clear separation between child and adult lifejackets. 46 CFR 180.78(a)(5)

Fuel lines must be annealed tubing of copper, nickel-copper, or copper-nickel having a minimum wall thickness of 0.9 millimeters (0.035 inch) except that:(i) Diesel fuel piping of other materials, such as seamless steel pipe or tubing, which provide equivalent safety may be used;(ii) Diesel fuel piping of aluminum is acceptable on aluminum hull vessels provided it is a minimum of Schedule 80 wall thickness; and(iii) when used, flexible hose must meet the requirements of § 182.720(e). 02 hoses on generator could not be verified as approved type. Provide evidence of proper replacement or evidence of approved hoses. 46 CFR 182.455

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Electrical connections on aft bulkhead of engine room need to be enclosed. 46 CFR 176.830(a)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Vessel had excess flammable liquids stored in accommodation space on board. Remove excess flammable liquids or store in designated flammable storage locker. 46 CFR 176.830(b)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Handrail fasteners were loose throughout vessel. 46 CFR 176.830(a)

The operator of each self-propelled vessel 12 meters or more in length shall carry, on board and maintain for ready reference, a copy of these Rules. Vessel was missing current Inland Navigation Rules. 33 CFR 83.01(g)

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation (COD) currently in effect for that vessel. Vessel was missing valid COD on board. 46 CFR 67.313(a)

All vessels must have navigation lights that are in compliance with the applicable sections of the international and inland navigation rules. STBD running light was oriented downward, not displaying correct light pattern. Repair or replace STBD navigation light.

Electrical equipment used in machinery spaces must be drip-proof. Electrical outlet in engine room found not drip-proof. Install/ fabricate drip-proof protection for outlet.

The Certificate of Inspection must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. COI and stability letter were found not posted. Post COI and stability letter.

Vessels on an exposed or partially protected route must be fitted with an 8 inch coaming on an opening that gives access to or is located in a cockpit or well, or an exposed location on a flush deck vessel. The aft door onboard was found without a sufficient coaming. Repair or replace coaming on door, prior to or during the next scheduled dry dock period. 46 CFR 171.124 (OLD)

Replace worn cutlass bearings on both shafts.

Repair cracked Starboard shaft.

Repair and install both propellers.

Repair 2 blisters in FRP on Port Bow.

Vessel is required to have an alcohol test kit. The test kit onboard was expired. Provide a valid alcohol test kit.

Vessel is required to have a 24", orange ring buoy with a waterlight and tending line. The ring buoy on board has faded in color, the continuous line around it was weathered and fraying, and the waterlight line is frayed and weathered. Replace or repair the ring buoy, and replace the lines on the ring buoy and waterlight.

Vessel is required to be marked with name and homeport on the stern, and vessel name on the port and starboard bow. No markings were on the vessel. Place the required markings on the vessel.

Generator designed for 40 degrees Celsius must be de-rated to 80% of the full load rating. De-rate generate to 80% of the full load rating.

MSD valve on Type 3 tank must be secured. Secure valve by removing handle, padlocking, or wire tie.

High water alarm must have audible and visual indicator at operating station. Provide working alarm for high water alarm in lazarette.

Provide supervisory training documentation for all crew with supervisory duties. 46 CFR 16.401

Display EAP resources in location visible to crew. 46 CFR 16.401

Provide background checks for employees as required per 49 CFR 40
