Any vessel of at least five net tons wholly owned by a citizen or citizens of the United States is eligible for documentation under this part. This includes, but is not limited to, vessels used exclusively for recreational purposes and vessels used in foreign trade. Vessel's COD is not current. 46CFR67.5

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. The vessel's safety cert had the wrong call sign. 46CFR80.903

A bilge pipe in a vessel of not more than 19.8 meters (65 feet) in length must be not less than 25 millimeters (1 inch) nominal pipe size. A bilge pipe in a vessel of more than 19.8 meters (65 feet) in length must be not less than 40 millimeters (1.5 inches) nominal pipe size. A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Vessel had no strainers at any of the bilge suction locations. 46CFR182.510(b)

No marine employer shall engage or employ any individual as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. The current regular captain that the owner has employed on the vessel, listed as MID # 124782 on the Confirmation of Enrollment from the Maritime Consortium, Inc. dated 15 May 2023 that was presented to the attending Marine Inspectors upon request of confirmation of compliance with the DAPI requirements, is listed as “HOLD – No Proof” under the “Status of Enrollment” column. *must be corrected prior to this individual serving onboard the vessel to meet the COI minimum manning requirements*

The owner, master, or managing operator shall instruct each crewmember, upon first being employed and prior to getting underway on a particular vessel, and at least once every 3 months, as to the duties that the crew member is expected to perform in an emergency. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Attending Coast Guard Marine Inspectors requested to review training and drill records, but no records could be provided for fire, abandon ship, and man overboard drills.

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. The bilge pick-up in the lazarette is missing the strainer.

A vessel must comply with the applicable requirements for any radio installations as set forth in 47 CFR Part 80. Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of 80.1101 (c)(2). VHF-DSC radio is not properly programmed – missing MMSI number and GPS input.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. The ventilation ducts for the machinery space that terminate near the port and starboard consoles are missing covers, which would allow the agent from the fire-extinguishing system to escape from the protected space, inhibiting the system’s effectiveness.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. The retroreflective tape on 8 adult and 1 child lifejackets are no longer in serviceable condition.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. In the forward storage area, adult and child lifejackets were found intermixed and without clear labeling indicating how each type were stowed separate from each other.

Each ring buoy on a vessel must be approved in accordance with 160.060 in subchapter Q of this chapter. The ends of the grab line shall be securely and neatly spliced together. The grab line shall encircle the buoy and shall be held in place by the backets. Ring lifebuoy line wasted and broken and requires replacement.

At each initial and subsequent inspection, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including: operation of the fire main system. At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. At the time of inspection, observed a dripping leak on the fire main near the cross-connect between the fire main and bilge piping when the fire pump and main was tested.

Each hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices to prevent its loss. The hatches for the engine room and port/starboard void spaces (near fuel tank) are missing securing devices (engine room hatch missing pin, void spaces hatches missing captive chains).

A vessel must carry either a first aid kit approved under approval series 164.041 or a kit with equivalent contents and instructions. The first aid kit available onboard at time of inspection was expired.

For a submersible electric bilge pump, a positive shutoff valve must be installed at the hull penetration. The overboard discharge pipe for the electric submersible bilge pump in the #2 void space is missing a shutoff valve.

At each initial and subsequent inspection of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure, including the following: inspection of all accessible parts of the exterior and interior of the hull. There are multiple instances of broken tabbing in the forepeak space. Provide a repair proposal to the Marine Inspector.

Remote fuel shutoff stations shall be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. The current marking for the fuel shutoff valves does not the above requirements.

At each initial and subsequent inspection of a vessel, the owner or managing operator shall have the vessel ready for inspection of machinery, fuel, and piping systems, including the inspection of all machinery essential the routine operation of the vessel. Dripless shaft seals found to be leaking at time of attendance.

A propulsion engine control system, including pilothouse control, must be designed so that a loss of power to the control system does not result in an increase in shaft speed or propeller pitch. The vessel was recently repowered with electronically controlled engines. In accordance with CG-CVC Policy Letter 17-07, testing shall be conducted in accordance with the Marine Safety Center (MSC) accepted and reviewed testing protocol (e.g. QFA and DVTPs).

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. The vessel’s DC electrical systems has reportedly been recently re-wired / re-done, without prior notification to the OCMI or submission of plans for the work to be performed. Submit electrical plans to the Marine Safety Center (MSC), with follow-up verification by attending CG Marine Inspector as needed.

Each hatch exposed to the weather must be watertight. Watertight hatches throughout the weather deck are not closing watertight. 46CFR171.124(a)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. The operating station has no visual indicators for installed automatic bilge pumps. 46CFR182.530(c)

Vessels carrying more than 49 passengers and =65ft in length shall be provided with 1 portable hand pump and 1 25GPM fixed power bilge pump which shall be self-priming and be permanently connected to the bilge main and may also be connected to the fire main. If of sufficient capacity the power bilge pump may also serve as a fire pump. A submersible electric bilge pump may be used as a power bilge pump only on a vessel of not more than 65 feet in length carrying not more than 49 passengers. Fixed power bilge pump and bilge main non-operational. 46CFR182.25-10(a)-(b) 46CFR182.520(e)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. No alcohol testing devices maintained on-board. 46 CFR 185.212 46 CFR 4.06-15(a)

A power-driven fire pump shall be installed on each mechanically propelled "S" vessel which is permitted to carry more than 49 passengers. Fire pump inoperable. 46CFR181.10-1(a) Old 'T'

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station:(1) A light to indicate discharge; (2) An audible alarm that sounds upon discharge. Audible alarm on pre-engineered fixed gas fire extinguishing system inoperable. 46CFR181.420(b)

The discharge ends of vent pipes shall terminate on the hull exterior...or, they shall terminate in U-bends as high above the weather deck as practicable... The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. Diesel fuel vent pipe on starboard side weather deck excessively wasted below u-bend and flame screen. 46CFR182.20-35(c)-(d) Old 'T'

Penetrations and openings in watertight bulkheads must have a means to make them watertight. Port exhaust piping penetration in the watertight bulkhead between port void and engine room cannot be made watertight. 46CFR171.114(a)(2)

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by Federal Communications Commission, as set for the in 47 CFR part 80. FCC Safety Radiotelephony Certificate expired and Master FCC Marine Radio Operator Permit not available for inspection. 46 CFR 184.502

Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members. Provide repair proposal for deck soft spot in way of passenger embarkation area and fueling hatch cover. 46 CFR 176.700(a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. 26 passenger seats identified in need of repair and marked. 46CFR176.830

An adult life jacket must be provided for each person carried on board a vessel. 04 adult and 03 child life jackets observed in unserviceable condition and removed. Passengers reduced to 122 until life jackets replaced. 46CFR180.71(a)

Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather deck. So that the marine inspector may satisfy himself as to the construction of the hull structure. For the following submit a repair proposal to the satisfaction of the attending marine inspector IAW NVIC 7-95. Inspector noted the 2nd and 3rd frame in the first compartment from the port side were sistered together with deck screws and did not extend to the most forward bulkhead. 46 CFR 176.25-5

On passenger decks of vessels engaged in ferry or excursion operations, rails shall be at least 42 inches high. The space below the rail shall be fitted with bulwarks, chain link fencing, wire mesh, or the equivelant. For the following submit a repair proposal to the satisfaction of the attending marine inspector. Marine inspector noted missing course on railing. Replace missing bottom wire course on the vessel aft, port and starboard sides prior to conducting excursion operations. 46 CFR 177.35-1

Rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crews. For the following submit a repair proposal to the satisfaction of the attending marine inspector. Marine inspector noted starboard railing had missing fasteners. 46 CFR 177.35-1

In systems and applications where flexible hoses are permitted to be clamped, double hose clamping is required where practicable. Port engine cooling water intake is missing one hose clamp. 46 CFR 182.410(d)(1)

A power-driven vessel underway shall exhibit a stern light. Stern navigation light is inoperable. 46 CFR 183.420 33 CFR 83.23(a)(i)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions are not posted at the operating station or on board the vessel. 46 CFR 185.510(a)

Each hatch exposed to the weather must be watertight. Engine room, port void space, and starboard void space hatch covers are not sealing water tight, and water is accumulating in starboard void bilge. 46 CFR 171.124(a) Old 'T'

The installation and location of portable fire extinguishers must be to the satisfaction of the Officer in Charge, Marine Inspections. Two portable fire extinguishers loosely stored in forward-starboard compartment without installed mounting. 46 CFR 181.520

Each child size lifejacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size lifejackets are not mistaken for adult life jackets. Child size life jackets are intermingled with adult size life jackets without markings for their stowed location, allowing sizes to be mistaken for each other. 46 CFR 180.78(a)(5)

A vessel must carry adequate and up-to-date charts, U.S. Coast Pilot [name withheld], and Tide & Current Tables for intended voyage. Chart plotter has not been updated since 2016 and no other navigational publications or charts are maintained on-board the vessel. 46 CFR 184.420(a)

Each life jacket and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. 8 life jackets and the ring life buoy were not adequately marked with vessel name. 46 CFR 185.604

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel reported a casualty to their starboard engine. Cause of casualty is unknown at this time. Engine is currently inoperable. Vessel is not authorized to carry passengers until after repairs have been completed and sea trials are conducted in the presence of an attending Coast Guard Marine Inspector.
