MI identified unprotected wire penetration into the conduit run from the ventilation fan in the aft locker on the main deck. MI discovered a dead end wire to the left of the entrance in the auxiliary machinery space (i.e. stack space). Wire to be removed or terminated in an appropriate junction box. At least one ring buy must be fitted with a floating waterlight; Each floating water light must be approved in accordance with 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, but not limited to, approval series 160.110. MI found the in service waterlight to have an expired battery. Battery to replaced to the satisfaction of the attending Marine Inspector. 46 CFR 117.70 (d)(1)

Each life float must be fitted with a light. MI discovered 06 of 08 life floats had attached lights with expired batteries. Batteries are to be replaced to the satisfaction of the attending Marine Inspector. 46 CFR 117.75 (d)

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. MI identified multiple bulkhead and deck penetrations without proper fire stops in the second deck bar stores. "old T" 46 CFR 177.10-5(a)

Doors in A-Class fire control boundaries must meet the following requirements; Holdback hooks are not allow. MI discovered a manual hold back hook installed in addition to the automated holdback arrangement 1st deck aft stairs. Manual holdback hook to be removed to the satisfaction of the attending Marine Inspector.

An Emergency instruction placard containing the following information; If a fire is in the machinery spaces, shut off fuel supply and ventilation and activate fixed extinguishing system if installed. Posted emergency instruction placards shall be updated to appropriately reflect utilization of the new CO2 system to the satisfaction of the attending Marine Inspector. 46 CFR 122.512 (a)(3)(iv)

The owner, charterer, master, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least every three months, as to the duties that the crew member is expected to perform in an emergency; Training must address proficiency in location and use of firefighting appliances. During the fire drill multiple crew members did not know the location or proper local operation of the newly installed fixed CO2 system. Formalized training to be created and conducted to ensure all crew are familiar with duties related to using the fixed CO2 system followed by, completion of a satisfactory fire drill to the satisfaction of the attending marine inspector. 46 CFR 122.420 (a)(b)(i)

Doors in A-Class fire boundaries must meet the following requirements: A door in a bulkhead required to be A-15 Class must be of solid or hollow steel or equivalent material construction capable of meeting the requirements of an A-15 Class bulkhead. MI identified 02 missing doors on th main deck galley space port and starboard. We reviewed the Fire Zone Plan (07081-001-635-0 / dated 1/27/09) approved by MSC letter H2-0901149 that appears to show port and starboard doors forming the boundary between the category 5 low risk accommodation space and category 9 galley. Per the bulkhead boundary tables in 116.415 (b), the required bulkhead rating between a category 5 space and a category 9 space is A-15. Provide a proposal to the local OCMI on how to bring the onboard arrangement into compliance with the applicable SFP requirements. 46 CFR 116.435 (c)(2) 46 CFR 116.415

A door must be of the self closing type capable of closing against a 3.5 list of the vessel.

Hydraulic leak in Engine Room.

Crank case breather piping for previously installed diesel engines protruding through the hull in the engine room, needs to be made permanently watertight.

Exhaust piping on port generator needs to be gastight throughout IAW 46CFR82.15-20(a)

High pressure injector tubing on stbd MDE worn in way of bracket. Renew. IAW 46CFR182.20-40
