Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. The approval number requirtes four rocket projectiles. Vessel had one onboard. provide three additonal projectiles in accornadance with 46 CFE 160.040

A vessel...must undergo a drydock and internal structural examination at least twice every 5 years, with not more than 36 months between examinations. Vessel is overdue for a drydock and internal structural examination. Action 705: Prior to commercial operations

Repairs and minor alterations to existing towing vessels must be made in accordance with this part. New installations that are not replacements in kind must comply with the requirements of subpart C of this part, if applicable. Vessel conducted repairs without notification or verification for compliance. Action 705: Prior to commercial operations.

Except as otherwise noted, medical certificates will be issued for the following periods of time: (2) Medical certificates issued to all other mariners will be issued for a maximum period of 5 years. Master could not produce a valid medical certificate. Last valid medical certificate in MISLE expired on 09JAN2024. Provide an updated medical certificate for current Master or provide another Master of Towing vessels with valid medical certificate prior to conducting towing operations. 46 CFR 10.301(b)(2)

An individual may not be engaged or employed, including self-employment, on a vessel in a position as master, operator, or person in charge for which a credential is required by law or regulation unless all crewmembers covered by this section are subject to the random testing requirements of this section. Please provide an updated list of covered employees currently enrolled in your drug and alcohol consortium. 46 CFR 16.230(K)

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Original COD was not onboard the vessel. O/O provided a receipt and NVDC shows the COD was issued but it has not been received by O/O. 46 CFR 67.313

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel did not have a copy of the consortium enrollment letter onboard. 46 CFR 16.220

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. Vessel had logs but not all the required information listed above was properly records. 46 CFR 140.915

Each towing vessel must be equipped with navigation lights, shapes, and sound signals in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84 as appropriate to its area of operation. Several of the required white light lenses were discolored and showing an orange/amber color instead of white. 46 CFR 140.720

Replace all expired items in the first aid kit.

Provide means to prevent personal from coming into contact will all pipes in the engine room that reach 150 degrees or higher

Properly secure battery box on the port generator to prevent it from moving.

Make permanent repairs to the port generator cooling water pipe.

AIS on board, but not working. Vessel in Alaska. Mate reports AIS is working as of 15 January 05.

VSL DID NOT HAVE A PUMP AND FIXED PIPING SYSTEM FROM THE MACHINERY SPACES TO THE WX DECK FOR THE DISCHARGE OF OILY WASTE. ALSO, THE VSL DID NOT HAVE A STANDARD SHORE DISCHARGE CONNECTION FOR IT'S WASTE DISPOSAL SYSTEM.
