The plan “Foundation for Crew-boat on Aft Propulsion House” A 6065-2291-1 is dated 27 June 1986. The Fire & Safety Plan 0918-832-001 21 July 2015 shows a boat located on the starboard side main deck between frames 78-81, but nothing on top of the Z-Drive spaces. This was brought to the attention of the local ABS Survey who found that it was not included. As calculations must account for the weight of all loads carried aboard the vessel and being there is no proof that the location of the work boat can be verified, the work-boat may not be carried until a new stability test is conducted and the results allow for the carriage of the work-boat.

In the pump room bilge, starboard side outboard,IWO fire pump remove the blue 5 gallon jug (unknown use) as bilges in the machinery spaces shall not have items which might create a fire hazard.

The ship has newly installed grating provide proof that the grating meets the guidance as set out in NVIC 9-97. The label may be molded into the grating, or included on a permanently attached label.

Properly install the international shore connections to the fire main with suitable connections for shore side fire fighters (stop valve should be before the flange)

Fire hazards shall be keep to a minimum, remove all the gear/stores from the AC Space as it is not designed a store room (a sign is also posted "No storage")

Each panel board must meet section 17.1 of IEEE 45-2002. Equipment for interior locations not requiring exceptional degrees of protection must be manufactured to at least NEMA 250 Type 1 with drip shield or IEC 60529 IP 11 as specified in IEC 60529. Each transformer winding must be resistant to moisture, sea atmosphere, and oil vapor, unless special precautions are taken, such as enclosing the winding in an enclosure with a high degree of ingress protection. 220/120V Breaker Panel and 440V Transformer in the Starboard Stern Store Room was observed to have severe water damage/corrosion, and the 440V transformer was not secured to the deck. Renew panel, wiring and inspect the internals of the transformer and secure it to the deck. Pictures of repairs can be accepted.

On January 18, 2023, the vessel was underway off the coast of Cape Hatteras in the Atlantic Ocean when the vessel experienced a loss of electrical power. Provide proof of notification to ABS.

Suitable hand rails shall be installed above the starboard side Z-drive room provide safety rails to protect the crew when there is need to close the vent covers.

Drawing #0918-832-01 Fire/Safety Plan dated 7/21/2015 shows what to appears to be a crew boat on the main deck port side 78-81 QTRS 01 Deck. Presently the crew boat is stored across the Z-Drive Rooms. Provide documentation of the installation of the crew boat cradles and storage to this new location. The dredging operation will last approximately 15-20 days upon arrival to the area. The #5 port void must be inspected by ABS surveyor every 30 days to ensure the temporary patch is free of water. The inspection must continue until permanent repairs are completed. Once dredging operations have concluded, the vessel must proceed to Bollinger Shipyard located in Morgan City, LA to complete permanent repair of #5 port void by NLT 15 April 2021. During vessel's annual inspection it was discovered that the the vessel has no PSTP which are required by regulation. Vessel must submit to Marine Safety Center (MSC) for approval DVT/PSTP that meet all relevant parts of applicable regulations. 24Feb2021 submitted to MSC.

The RN Weeks is in Sector New York's fleet of responsibility and records indicated that the vessel is PAST DUE for the 3rd Annual/Periodic inspection (LAPSED(3rd inspection sub activity was not opened between 06/05/2020 and 12/05/2020.) Vessels must undergo a periodic inspection within 3 months before or after the second or third anniversary of the date of your vessel’s Certificate of Inspection. You must provide a date and location that the periodic will take place. Once a date is confirmed there will be no extensions made the inspection must take place by 22 January 2021

Vessels life raft launching appliance falls were last replaced in 06JUN15. Vessel is required to replace falls and conduct a weight test in the presence of a marine inspector.

Vessel was replacing one of the generators that broke on the vessels last voyage. Vessel was unable to finish the install prior to departure. Vessel is reguired to finish the install within 14 days and notify the nearest OCMI for inspection.

The hospital is being used as a accommodation space. The space in addition to normal hospital equipment has additional lockers, workstation, TV and the bed is being used. The hospital is to be used as a treatment space for the sick and no others.

There shall be at least two means of escape for the crew. The starboard side storeroom aft emergency exit was not able to be opened.

Means shall be provided to secure all the ventilation from outside the space. The forward house ventilation fire dampener is not able to be closed.

Vessel #2 generator is not operational due to a faulty fuel oil pump and cannot demonstrate safety controls. Vessel must repair the engine and prove proper operation of the safety controls to a USCG Marine Inspector or request ABS verification.

Coast Guard Sector NC was notified by vsl rep that during maintenance to equipment in fwd ballast tank a minor fuel smell and sheen was noticed. Remove fuel from adjacent tank and schedule a marine chemist to certify both tanks safe for entry. Vessel to be attended by CG Marine Inspector prior to movement.
