46 CFR 182.100 All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI observed hydraulic leaks on both steering rams. Enact permanent repairs to the satisfaction of USCG Marine Inspector.

46 CFR 182.455 (b) (4) The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. MI observed that valves were not completely closing. Enact permanent repairs to the satisfaction of USCG Marine Inspector.

46 CFR 181.405 (c) Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. MI observed that detector in engine room was not operational, and that a detector was not installed in the pilothouse. Enact permanent repairs to the satisfaction of USCG Marine Inspector.

46 CFR 180.210 (a) A vessel of more than 19.8 meters (65 feet) in length must carry at least one rescue boat unless the cognizant OCMI determines that the vessel is sufficiently maneuverable, arranged, and equipped to allow the crew to recover a helpless person from the water. During a demonstration of helpless person recovery, the Marine Inspector observed that the recovery method was unsatisfactory. The vessel shall propose procedures and demonstrate a safe and effective method for recovering a helpless person from the water.

46 CFR 176.810 (b) (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel logs indicated that fire extinguishers were not being inspected monthly Corrected on the spot.

46 CFR 185.728 (a) Each EPIRB, other than an EPIRB in an inflatable life raft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. Vessel logs indicated that EPIRB was not being tested monthly. Corrected on the spot.

Exhaust gas must not leak from the piping or any connections in the exhaust system. Both port and stbd exhaust pipes were leaking cooling water from the flexible coupling just before passing through the aft bulkhead. Make repairs to the Coast Guards satisfaction. 46 CFR 182.430

Vessel on a Oceans route must carry a total of 12 distress signals, 6 red, 6 smoke, or 12 red. All distress signals were 1 month expired. New distress signals were purchased and on board prior to completing the inspection. 46 CFR 180.68

At each inspection each item of fire protection equipment shall be ready for inspection. The 5th, extra, fire extinguisher in the E/R had no charge. Service, replace, or remove to the satisfaction of the Coast Guard.

A visual and audible alarm must be provided at the operating station to indicate high water. The visual light for compartment 1 was not working. Repair to the satisfaction of the Coast Guard.

Deck material wastage discovered beyond 25% within fuel tank room. Repair to the satisfaction of the CG.

Excessive shaft movement discovered on port and starboard shafts. Remove and inspect. Provide objective evidence to the satisfaction of the CG shafts are not damaged or worn.

46 CFR 182.100 Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Vessel reported port engine water pump failed. Vessel shall enact permanent repairs to the satisfaction of USCG Marine Inspector.

All engine exhaust pipes must be water cooled. MI found the starboard engine exhaust cooling water hose to be deteriorated needing replacement. 46 CFR 182.425

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Junction box for the DC system located forward of the door on the port side to the accommodation space has no means of closure exposing the wire connections and breakers. 46 CFR 183.340(g)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. MI observed the guard for the belt on the vessel's generator to be missing. 46 CFR 183.340(g)

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. MI found the connection joint of the starboard side ram of the steering gear to be loose causing play in the system. 46 CFR 176.814

Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location of the vessel and the authorized route of the vessel. MI observed two broken windows, one located in the wheel house and one on the galley door. 46 CFR 177.1020

A lighting fixture must be installed as follows: Each fixture must comply with 183.200. MI observed the lighting fixture in front of the wheelhouse to be missing the bulb exposing the socket. 46 CFR 183.410(c)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel shall install system to the satisfaction of attending USCG Marine Inspector. 46CFR185.405(c)

Flammable items not otherwise covered by the regulations of this subchapter, such as rechargeable batteries, including lithium ion batteries utilized for commercial purposes, must be handled, stored, and operated in a way that mitigates the risk of hazardous conditions. Vessel shall develop plan to the satisfaction of attending USCG Marine Inspector. 46CFR185.364

Propulsion machinery must be suitable in type and design for propulsion requirements and capable of operating at constant marine load under such requirements without exceeding its design limitations. Vessel experienced power load failure and lost propulsion capabilities as a result. Repair failed equipment and demonstrate proper operation of machinery to attending USCG Marine Inspector prior to carrying passengers. 46 CFR 182.200(a)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI found the stern light extinguished. The vessel is limited to daylight without reduced visibility operations. 46 CFR 183.420

At each initial inspection for certification of a vessel the owner or operator shall have the vessel ready for inspection all machinery essential to the routine operation of the vessel. MI found the ships service diesel generator (SSDG) leaking fuel from the fuel manifold. 46 CFR 176.804(c)

The enclosures of all equipment shall be permanently grounded to the metal hull of the vessel by the mounting bolts or other means. MI found metallic enclosures ungrounded. 46 CFR 183.10-20(g)

A dual voltage generator installed on a vessel shall be of the grounded type, where the neutral of a dual voltage system must be solidly connected at the switchboard's neutral bus. MI found no neutral to ground. 46 CFR 183.324(a)(1)

Ground detection must be provided for a dual voltage alternating current system. The dual voltage AC electrical generator is missing ground detection at the switchboard. 46 CFR 183.324(c)(1)

On a metallic vessel, a grounded alternating current system must be grounded to the hull. MI fund the 110 volt AC appliances ungrounded. 46 CFR 183.376(d)

During a fire in the engine room causality the vessel shall follow the emergency checklist including closing air supply by closing ventilators. MI found in the engine room aft port overhead a ventilation duct without a dampener. 46 CFR 185.25-5(c)

On a vessel of at least 26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. MI found the bilge high level alarms OOC. 46 CFR 185.530(a)

The neutral of each grounded generation and distribution system must be grounded at the generator switchboard. MI found the neutral ungrounded. 46 CFR 183.376(c)

Each vessel shall be provided with safe, readily available and sufficient firefighting equipment necessary to extinguish a fire. MI found leaking fire main pipes above the main fire pump in the engine room. 46 CFR 181.01-5

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel, shall be completed to the satisfaction of the cognizant [name withheld] found multiple deficiencies in the electrical system to include: missing grounding conductors for all AC items onboard, missing ground bus bar in the distribution panel, missing neutral to ground at the switchboard, and missing ground detection at the switchboard. The vessel shall submit electrical repair plans to MSC for approval. 46 CFR 183.115(c)

Each portable fire extinguisher on a vessel must be of an approved type. MI found a 40-B:C fire extinguisher waisted from corrosion. Rectify within 7 days. 46 CFR 181.500(a)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. MI found wiring in the AC compartment not properly affixed to metal brackets. Rectify within 7 days. 46 CFR 183.340(b)(4)

All vessels must have permanent draft marks at each end of the vessel or have permanent load markings placed on either side of the forward, amidships, and aft to indicate the maximum allowable draft and trim. MI found the draft makings required by the vessels stability letter missing from the hull. Repair during next drydock. 46 CFR 185.30(b)(1)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than (65 feet) in length must also have navigation lights that meet UL 1104, Standards for Marine Navigation Lights. MI found navigation lights including, port side light, starboard side light, and mast light lenses severely corroded. Rectify within 14 days. 46 CRF 183.420

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name. MI found ring life buoys markings faded and unreadable. Rectify within 7 days. 46 CFR 185.604(b)

Each ring life buoy on a vessel must be approved in accordance with 46 CFR 160.050 in subchapter Q of this chapter. MI found the starboard aft life ring buoy to be unserviceable due to a cracked water tight plug in the body of the buoy. Rectify within 7 days. 46 CFR 180.70(b)(1)

The fire pump outlet shall be fitted with a pressure gauge. The MI found the fire pump pressure gauge unserviceable. 46 CFR 181.10-1(c)

Each penetration of a watertight bulkhead shall be Accomplished by using an approved method. MI found missing packing encompassing a cable run from the fuel space to the engine room. 46 CFR 182.40

Where flexibility is necessary, a section of flexible metallic hose may be used and is double clamped where practicable. MI found 5 hose clamps missing from the wet exhaust system. Rectify within 7 days. 46 CFR 182.430(e)

The platform must be arranged so that under normal stowed conditions, it can be retained in the center of the float and can be readily released from this position for use. MI found the life float basket affixed using an unapproved nylon cord. Rectify within 7 days. 46 CFR 160.027-3(e)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. The PA speaker conductor connections on the main deck were not made within an enclosure and conductors are exposed to the elements. 46 CFR 183.340(g)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart. The conductors on the main deck located behind where the life jackets are stored on port/stbd upper shelves are not supported. The conductors are tangled with the life jackets and create a hazard to passengers/crew when removing life jackets. 46 CFR 183.340(a)(4)

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessels Starboard engine failed while the vessel was underway with passengers. After troubleshooting and engine is operational, conduct Sea trials in the presence of Coast Guard marine Inspector.

A vessel must comply with the applicable requirements for any radio and EPIRB installation as set forth in 47 CFR part 80. The vessel's MF-DSC single side band radio is inoperable. The vessel is restricted to voyages no further than 20 miles from shore. 46 CFR 184.502

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without approval of OCMI. Identified heavy pitting corrosion on two transverse frames in the engine compartment. Engine room frames #1 and #2 (numbered from aft to fwd) were found pitted. Crop and renew frames at the next scheduled dry dock. 46 CFR 176.700

The exhaust piping shall be so arranged as to be gas tight throughout its length. Identified corroded exhaust piping in the lazarette for the ship's generator. The corrosion is located at the rubber hose connection for flexibility and permits the flow of exhaust vapor into the space. Make satisfactory repairs and contact MI for re-inspection when complete. 46 CFR 182.20-20

The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. Following main propulsion rebuild and steering gear servicing, conduct sea trials in the presence of a marine inspector. Prior to Credit Dry-dock. 46 CFR176.700(d)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. MI observed unapproved nonmetallic flexible hose at discharge end of bilge system from auxiliary pump. Replace unapproved hoses with approved bilge piping, as listed below, and provide satisfactory operational test. Approved Bilge piping materials: 1.) Metallic (Aluminum) 2.) Rigid Nonmetallic (PVC) 3.) SAE J-1942 Nonmetallic Flexible Hose w/ approved 1475 fittings. 4.) SAE J-1527 or J-2006 Nonmetallic Flexible Hoses, meeting the following additional requirements: - Length of hose must be less than 30 in. - System pressure must be less than 5 psig. - Where practicable, have 2 hose clamps at both ends. IAW: 46 CFR 182.25-1, 46 CFR 182.40-15, 46 CFR 182.720 (e)

In systems and applications where flexible hoses are permitted to be clamped, double hose clamping is required where practicable. MI observed missing hose clamps on multiple generator hoses. Repair. IAW: 46CFR182.410 (d) (1)

Where life preservers are stowed so that they are not readily visible to passengers, the containers in which they are stowed shall be marked "life preservers". MI observed faded/illegible life preserve stowage markings in accommodation space. Repair. IAW: 46 CFR 185.30-15

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed tear/hole on inside grab line of life-float. Repair. IAW: 46 CFR 176.808 (b)

All escape hatches and other emergency exits shall be marked on both sides using 1-inch letters: "Emergency Exit, Keep Clear". Escape hatch in engine room was not properly labeled. Label escape hatch with proper wording in no less than 1-inch letters. IAW: 46 CFR 185.30-15

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. MI observed forward rope deck pipe missing cover. Replace. IAW: 46 CFR 16.800 (a)

Install 2 life floats in kind and reduce manning to 40 pax and 4 crew iaw 46CFR180.10-5

Provide GPS input to VHF radio containing MMSI number for DSC. 46 CFR 184.502

Replace missing gasket on aft watertight hatch; prove watertight. 46 CFR 179.360

Replace missing lanyard on forepeak hatch. 46 CFR 176.360(b)(2)

Replace three inoperable lighting fixtures in accommodation space. 46 CFR 183.410

Port Engine was reported inoperable. Make proper repairs and prove satisfactory operation of engine. Deficiency is in effect once vessel is dockside. 46CFR 182.200
