Each vessel that relies on electricity to power vital systems, interior lighting, communication systems, and navigation equipment and lights must be arranged so that the loads can be energized from two sources of electricity. Vessel reported a failure of their starboard generator while underway with passengers. Determine cause of generator failure and repair as appropriate. 46 CFR 120.310

46 CFR 56-50-50(g) - Each individual bilge suction shall be fitted with a suitable bilge strainer having an open area of not less than three times that of the suction pipe. Multiple bilge pickups were missing suitable strainers.

46 CFR 116.405 - The general construction of the vessel must be such as to minimize fire hazards insofar as it reasonable and practicable. There was missing fire insulation in the soda room.

46 CFR 120.410(a) - Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. There were multiple lights missing guards.

46 CFR 117.70(d)(2) - Each life ring buoy with a floating waterlight must have a lanyard of at least 3 feet in length but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Waterlight did not have a lanyard attached.

46 CFR 116.700(a) - A crew accommodation space and a work space must be of adequate construction and with suitable equipment to provide for the safe operation and accommodation of the crew in a manner practicable. Metal stairway into galley space is wasted and heavily corroded. This is to be addressed at the next scheduled dry-dock 1/31/2025.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Life ring on bridge not connected to waterlight. 46CFR180.70(d)(2)

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Missing or inapropriate strainers on all bilge piping. 46CFR182.510(b)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Piping at fire pump has unused section that needs to be capped off. Possible flooding hazard. 46CFR182.100

The neutral of each grounded generation and distribution system must be grounded at the generator switchboard and have the ground connection accessible for checking insulation resistance of the generator to ground before the generator is connected to the bus, except the neutral of an emergency power generation system must be grounded with: (1) No direct ground connection at the emergency switchboard; (2) The neutral bus permanently connected to the neutral bus on the main switchboard; and (3) No switch, circuit breaker, or fuse in the neutral conductor of the bus-tie feeder connecting the emergency switchboard to the main switchboard. Ground test lamps indicate a fault for the #1 generator. 46CFR183.376(c)

New installations of electrical equipment and material, and the repair or replacement of wire and cable, on an existing vessel must comply with this part. Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel must provide report from certified marine electrician attesting to electrical system, as pertains to galley circuit breaker, is intact and unaffected by galley fire on 10JUN2022 IVO tilt skillet. Until report is reviewed and approved by MI, vessel is to isolate/ensure galley circuit breaker inoperable when passengers onboard. Vessel must also ensure that fire watchstander is posted in galley to monitor for/respond to signs of fire at all times when passengers onboard. 46 CFR 120.115(c) 46 CFR 120.200(b)

Vessels which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H of this chapter. Structural fire protection was observed missing from multiple locations throughout the vessel, including above main deck air handler, second deck forward stores, and engine room bulkheads. All electrical installations shall be to the satisfaction of the [name withheld] wiring was observed near forward galley bulkhead and equipment was observed being stored on vessel's electrical cable run in second deck forward storage space. "Old T" 183.01-15(a)

A fire control plan must be posted on the vessel showing the location of fire protection equipment. Multiple fire extinguishers were observed of improper size, off-station, or missing from locations listed on fire control plan. 46 CFR 116.115(b) 46 CFR 116.530

Doors in stairway enclosures shall be of the self-closing type. MI observed second-deck self-closing doors not of adequate closing speed. "Old T" 117.10-5(a)

Vertical exhaust systems shall be lagged with fire resistant material. During inspection missing lagging was observed on STBD engine exhaust. Replace in kind. "Old T" 182.20-15(a) "Old T" 182.15-20(f)

Portable fire extinguishers shall be installed to the satisfaction of the [name withheld] portable fire extinguishers observed onboard missing suitable marine-type mounts. "Old T" 181.30-10(b)

A fire door in a stair tower must be self closing. The fire door leading to the galley did not close all of the way upon actuation.

The fire pump located port side engine room was missing the junction box cover, improper connections with screw on connectors as well as uninsulated connections were observed.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Pump located in the port side of the compartment aft of the engine room was not mounted and electrical cable clamp for the power source was loose.

Fire protection shall be such as to minimize fire hazards insofar as reasonable and practical. Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 or Subchapter H of this chapter. Manual hold back was installed on the door leading to the stairway leading down to the machinery spaces.

All cable and wire must be installed with metal supports spaced not more than 24 inches apart, and in such a manner as to avoid chafing and other damage. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date on your COI. You failed to complete your annual inspection within the required timeframe. Failure to complete your inspection by August 31, 2020 will result in your deficiency being upgraded to Prior to Carrying Passengers.

A vessel on LBS route must carry 3 Red Flares and 3 Orange smoke or 6 Red flares. 6 orange smoke flares on board. Provide at least 3 Red flares approved distress signals

Provide diagram/Instructions for operation of each bilge pick-up line. Crew unable to satisfactorily drain water tight compartments.

Prove proper operation bilge pump and proper suction from each fixed bilge pick up.

Bilges must have no excessive accumulation of oil, trash, debris or other matter that might create a fire hazard or clog bilge pumping system. Properly clean excessive grease accumulation in galley bilge.

Provide this office with a Repair Proposal for conducting permanent repairs to the affected area of the keel cooler. Ultrasonic Testing (UT) must be included as part of the Repair Proposal.

Ultrasonic Testing IWO the entire hull plating associated with both Keel Cool Systems. Any additions or updates to the proposal should include a schematic identifying all affected/wasted areas and a plan for conducting permanent repairs for all wasted areas identified. A copy of the UT Report must be provided to this office.

Properly secure main electrical supply cable (that feeds the main switchboard) located in the engine room

Remove all excessive electrical extension cords and surge protectors (“Daisy Chains)

Replace missing cover for junction box located the galley

Remove portable space heaters located throughout the vessel

Vessel has 02 pinhole leaks in the hull bottom of the keel cooler system in between frames 23-24 IWO of the galley compartment. Provide temporary repair proposal to the satisfaction of the local [name withheld] approved, conduct repairs to the satisfaction of the local OCMI.

Provide next available dry dock dates and facility to local OCMI for external hull survey and perm. repair of hull bottom.
