A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. Prove proper operation as described by the regulation and manufacturer’s instructions in the presence of the attending marine inspector. 46 CFR 181.420(b)(3)

Drill not conducted due to the failed stbd pre-engineered fixed gas fire extinguishing system reset device. Conduct a satisfactory man overboard drill in the presence of the attending marine inspector upon completing repairs to the failed stbd pre-engineered fixed gas fire extinguishing system reset device. 46 CFR 176.810(d)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Drill not conducted due to the failed stbd pre-engineered fixed gas fire extinguishing system reset device. Conduct a satisfactory man overboard drill in the presence of the attending marine inspector upon completing repairs to the failed stbd pre-engineered fixed gas fire extinguishing system reset device. 46 CFR 176.808(g)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Observed a suspected crack on the stbd crossbeam vertical station weld propagating to the end plate of the crossbeam. Remove paint to expose the aluminum for reinspection by the attending marine inspector by March 4, 2025. 46 CFR 176.840

A vessel of 12 meters or more in length shall be provided with a whistle. Vessel missing whistle. Provide a whistle with a range of 0.5 miles as described in Table 33 CFR 86.01(c) to the satisfaction of the attending marine inspector by March 4, 2025. 33 CFR 83.33(a)

An emergency instruction placard containing the following information will satisfy the requirements of § 185.510. Vessel emergency instructions missing flooding. Amend vessels emergency instructions to include flooding as described by the regulation to the satisfaction of the attending marine inspector.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. The radio inline fuse is of a blade type. Replace with same amperage UL approved cartridge type fuse to the satisfaction of the attending marine inspector. 46 CFR 183.380(l)

Except as noted in § 176.114 of this part, each vessel inspected and certificated under the provisions of this subchapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. Vessel has Sector Honolulu WI-31 on the COI routes permitted and conditions of operation. The portable oxygen bottle required by WI-31 has passed its hydrostatic test date. 1) Replace with a new oxygen bottle or 2) Hydrostatic test the overdue oxygen bottle to the satisfaction of the attended marine inspector.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The ring life buoy has a strip of wasted retroreflective tape. Renew in-kind to the satisfaction of the attending marine inspector. 46 CFR 185.604(i)

Except as noted in § 176.114 of this part, each vessel inspected and certificated under the provisions of this subchapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. Vessel has Sector Honolulu WI-31 on the COI routes permitted and conditions of operation. The current WI-31company policy does not meet Sector Honolulu WI-31 policy requirements. Revise the current WI-31 company policy to include the specific information missing for review and approval by the attending marine inspector to retain the reduced manning endorsement. 46 CFR 176.100(b)

The master of a vessel, except a vessel listed in § 185.502(a) of this part, shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. The passenger count shall be available to the Coast Guard upon request. No process to meet the above regulation is implemented. Implement the above regulation as described to the satisfaction of the attending marine inspector. 46 CFR 185.504

Spaces containing machinery shall be fitted with at least two ducts to furnish natural or mechanical sup-ply and exhaust ventilation. One duct shall extend to a point near the bottom of the compartment, so installed that the ordinary collection of water in the bilge will not trap the duct. Where forced ventilation is installed, the duct extending near the bottom shall be the exhaust. The total inlet area and the total outlet area of ventilation ducts shall each be not less than one square inch for each foot of beam of the vessel. These minimum areas shall be in-creased as necessary when such ducts are considered as part of the air supply to the engines. One of the two Stbd engine room intakes was decommissioned by blanking off with a piece of wood due to the vent being damaged. Restore this stbd engine intake back to original to the satisfaction of the attending marine inspector. 46 182.20-45(c) Old "T"

All vessels must have navigation lights that are in compliance with the applicable sections of the Navigation Rules. Vessel did not have operational anchor light. Provide navigation lights that are in compliance with the Navigation Rules. 46 CFR 183.420

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Bilge pump visual indicators did not turn on when pump was active. 46 CFR 182.530(c)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. Nine life jackets were not marked with appropriate retroreflective material. Provide appropriate markings to the satisfaction of the attending marine inspector. Passenger count reduced to 41. 46 CFR 185.604

Machinery and fuel tank spaces must be separated by boundaries that prevent the passage of vapors. Penetrations in stbd deck, existing vent openings both sides, and port aft bulkhead allow passage of vapors. Provide a proper seal to prevent the passage of vapors. 46 CFR 177.405

Except as otherwise provided in this section, rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crew. Stanchion on port forward deck rail is loose at deck mounting. Secure stanchion deck mounting point. 46 CFR 177.35-1(a) OLD T

Each vessel inspected and certificated under the provisions of this subchapter must, when any passengers are aboard during the tenure of the certificate, be in full compliance with the terms of the certificate. Vessel's required deckhand enters the water during in water snorkeling activities and the vessels AED pads are expired IAW Work Instruction 31. Replace AED pads IAW Work Instruction 31. 46 CFR 176.100(b)

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The markings required by this section, which may be made by the use of any means and materials which result in durable markings, must be made in clearly legible letters of the Latin alphabet or Arabic or Roman numerals not less than four inches in height. Vessel's name and hailing port markings are less than four inches. Replace with markings of appropriate size.

Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other than steel construction will specify if a flame shield is required. Installed secondary port and stbd main engine fuel filters not an approved type. Replace with approved fuel filter. 46 CFR 182.455(b)(6)

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. Marine employer is DER and enrolled in the company's random testing program. A person subject to testing cannot receive their random selection notice and still meet the definition of “unannounced” as it pertains to random tests. Resolve by providing documentation on how the random testing process will work for the owner to the satisfaction of the attending MI. 46 CFR 16.113(a)

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. Vessel shares crew with another company, and no crew sharing agreement is in place. Provide evidence of crew sharing agreement to attending MI. 46 CFR 16.113(a)

A submersible electric bilge pump may be used as a power bilge pump provided that the pump is listed by an independent laboratory as meeting the requirements in UL 1113. Vessel was unable to prove that installed submersible electric bilge pumps are UL 1113 rated. Provide evidence of existing pump UL 1113 rating, or replace with approved bilge pumps. 46 CFR 182.520(e)(1)

Vessels constructed of wood must provide bilge level alarms in all watertight compartments. Vessel did not have bilge level alarm in port forward space. Install bilge level alarm in port forward space. 46 CFR 182.530(b)

Suitable covers, guards, or rails shall be installed in a way of all exposed and hazardous places such as gears, machinery, etc. Port and Starboard Main Diesel Engines do not have guards over serpentine belts. 46 CFR 177.35-15 (a) (Old T)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Number of child size life jackets are marked incorrectly. 46 CFR 180.78 (a) (5)

A pre-engineered fixed gas fire extinguishing system must: Be installed in accordance with the manufacturer's instructions. Cable run between port and stbd engine room and port and stbd aft lazarette not properly sealed, allowing fixed fire extinguishing medium to communicate through bulkhead. Make vapor tight to ensure quantity of extinguisher is sufficient. 46 CFR 181.420 (a) (4)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. Forward port compartment high bilge alarm inoperable. 46 CFR 182.530 (a)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position or perform the duties and functions of a position, required by the vessel's COI. Marine employers may form or otherwise use sponsoring organizations, or may use contractors to conduct the random chemical testing programs. Vessel's consortium enrollment paperwork outdated and missing pre-employment drug testing. 46 CFR 16.230(a)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel's life ring buoy reflective tape is worn. Replace life ring buoy reflective tape. 46 CFR 176.808(b)

An adult life jacket must be provided for each person carried on board a vessel. Vessel had 7 life jackets not in serviceable condition. Vessel is restricted to 43 persons total due to lack of properly outfitted life jackets. 46 CFR 180.71

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing correct MMSI for DSC operation. Update MMSI number in accordance with FCC Radio Station Authorization certificate. 47 CFR 80.905

Inspect all railings and bulwarks and their attachment to the hull structure. Two fractured welds on fwd upper stanchions for the canopy. Two fractured welds on the port and stbd passenger seating stanchions. Scarf and reweld fractures to the satisfaction of Marine Inspector. 46 CFR 176.25-10 Old-T

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of a vessel. The vessel has a fracture where the transverse frame meets the inboard bulkhead in the aft portion of the engine compartment and at the transverse frame that meets the outboard bulkhead in the battery compartment. Submit a repair proposal to be reviewed and approved by the attending USCG Marine Inspector. 46 CFR 177.10-1(a) Old-T

Provision shall be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Vessel has no means of securing intake or exhaust ventilation.

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. Holes in the forward engine room bulkhead on the port side and free communication between the port and starboard hulls allow the fixed fire extinguishing medium to communicate outside the space it is intended to protect. Submit repair proposal to be approved by the attending USCG marine inspector.

The COI issued to a vessel describes the vessels minimum manning requirements and any equivalences accepted or authorized by the cognizant OCMI. The vessels Master does not hold a valid MMC. Vessel is required to be operated by, and shall not operate without, a Master holding a valid MMC with applicable endorsements.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. All 5 of the required child size PFDs and 11 of the adult size life jackets are deteriorated and shall be replaced. The vessel is not to carry any children or more than 35 adult passengers until these PFDs are replaced.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. These records shall identify the total number of individuals chemically tested annually for dangerous drugs in each of the categories of testing required by this part including the annual number of individuals failing chemical tests and the number and types of drugs for which individuals tested positive. Vessel was unable to provide a report for total number of employees tested.

As appropriate for intended voyage, a vessel must carry adequate and up-to-date charts. Vessels charts have not been corrected since 2006.

The master shall ensure that one or more placards meeting the requirements of this section are displayed in prominent locations. Garbage placard is missing.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Vessel has areas of angel hair wood rot in compartment aft of the starboard rudder. Remove loose angel hair to facilitate further inspection and submit repair proposal to be approved by attending USCG inspector.

Vessel must be provided with satisfactory arrangement for draining any watertight compartment. Vessel was unable to demonstrate operation of the starboard fixed hand pump.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel does not have an AED on board as required by Work Instruction 31. Operations under Work Instruction 31 are not to be carried out until an AED is on board the vessel.

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Missing double hose clamps on stbd MDE oil filter hose.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Vessels remote fuel shutoff station not labeled.

Each connection to a conductor must be made within an enclosure and have a proper connection. The positive DC cable for the starboard main diesel engine was secured with an improper conductor. Secure the cable in an enclosure with an approved conductor.

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. The electrical connector controlling the port engine is damaged and shall be repaired to prevent loss of connection.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. MI observed a temporary repair to the stbd MDE heat exchanger and determined there to be a leak at this location. Submit repair proposal to the satisfaction of attending inspector.

Vessel required to have one 20" or 24" ring lifebuoy fitted with 60' lifeline and floating water light attached with a corrosion resistant clip to allow quick disconnect. Ring lifebuoy’s water light no longer in serviceable condition. Provide proper serviceable water light.

A hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. The lever securing the hatch cover on the port forward compartment is inoperable and shall be repaired.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. The port and starboard fuel tank vents do not have the correct size flame screens installed.

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used. When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. The fuel fill hose on the port side is missing the required bonding wire.

The owner or managing operator shall make a vessel available for drydock examination. Vessel is overdue for a credit drydock.

Independent fuel tanks must be designed and constructed as described in this paragraph (a). Aluminum diesel fuel tank has a hole on the weld due to corrosion.

Each officer employed on any vessel subject this subchapter must have his or her license onboard and available for examination at all times when the vessel is operating. Vsl master did not have a medical certificate. Provide a current medical certificate.

An adult life jacket must be provided for each person carried on board a vessel. Vessel is restricted to 19 persons total due to lack of properly outfitted lifejackets. 16 L/J did not have required retroreflective tapes, 8 L/J were still inside the original packaging with no vessel name, and 7 were unservicable due to damages.

Child size life jackets equal to 10% of the number of persons permitted on board must be provided. Vessel needs provide required number of child size life jackets due to not being servicable.

Every vessel must be fitted with a suitable magnetic compass designed for marine use, compass must be illuminated or restricted to daytime operation. No light for the compass.

A means to reset devices used to automatically shut down ventilation systems and engines. Stbd engine room Fireboy fixed system override switch does not operated as designed. To start the engine, override switch must be pressed during the engine start. The engine should start without holding the override switch.

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. Repair a fracture at the base of bow center stanchion.

A hatch cover must have securing devices and be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss.

Provide repair proposals for stbd fwd compartment with a diagonal longitudinal member with multiple location wood rot, below the hatch coaming facing forward wood rot.

Provide repair proposals for port fwd compartment with a longitudinal stringer penetrating fwd bulkhead with multiple wood rot locations.

Nozzle, flanges, or other fittings for pipe connections to a metal tank must be welded or brazed to the tank. Properly install fuel return line to fuel tank for both port and stbd fuel tanks.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have a light to indicate discharge; Starboard visual indicator for FireBoy does not activate to indicate discharge. Repair visual indicator.

At least one ring life buoy must be fitted with a lifeline; Lifeline on ring buoy is wasted. Replace lifeline on ring buoy.

An adult life jacket must be provided for each person carried on board a vessel; Vessel has 42 out of required 50 life jackets. Replace wasted life jackets. Total persons allowed on board reduced to 42 persons.

Each hydrostatic release unit, other than a disposable unit, must be service within 12 months of its manufacture and within 12 month of each subsequent servicing; Hydrostatic release unit has not been serviced since 2017. Replace hydrostatic release unit for EPIRB.

Each ring life buoy must be marked in clearly legible block capital letters with the vessel's name; The markings on the vessel's ring life buoy are no longer legible. Add vessel's name to ring life buoy iaw 46 CFR 185.604.

When the operator is on duty and in charge of transmitting systems, or performing service, maintenance or inspection functions, the license or permit document, or a photocopy thereof, or a copy of the application and PPC(s) received by the FCC, must be posted or in the operator's personal possession, and available for inspection; Operators permit was not available on board vessel. Provide operators permit for inspection.

Repair rotted stringers in port and starboard aft compartments. Submit repair proposal for Coast Guard approval prior to commencing repair.
