Portside Life Buoy faded. Replace retro.

Portside lifejacket box on the 01 deck – Found 10 child lifejackets that are in need of repair.

Ensure that all child life jackets are stowed in a manner to keep them separated from the adult lifejackets. The boxes on the 01 deck, have child life jackets stowed without separation.

Fire extinguishers. Provide documentation for monthly testing of portable fire extinguishers. This is the visual check of the extinguisher on a monthly basis. This could be recorded on each tag or on any other kind of maintenance log.

Lifebuoy Light – One of the lifebuoy lights was damaged during the man overboard drill. Provide a new light for the buoy.

Fuel Shutoffs – Not labeled – Label as appropriate.

Floor Heater – Portside 01 deck forward. Heater found damaged. Repair as necessary.

Vessel suffered a marine casualty. Vvulcan coupling seperated on stbd main engine.

Vessels sprinkler system on car deck was not properly working at all the sprinkler heads.

Crewman was confused over instructions and stop valve numbering.

Required number of child life jackets were not in designated areas. Child life jackets were found amongst adult life jackets. Ensure proper amount of child lifejackets are in designated areas.

There was evidence of an exhaust leak on stbd exhaust. REpair/replace as necessary.

Aft stbd fire hose could not be connected to other hoses. REpair/ replace as necessary.

After loading and prior to departure, and at all other times necessary to assure the safety of the vessel, the master shall determine that the vessel complies with all applicable stability requirements in the vessels stability letter. The Captain and Senior Deck Hand were unable to tell the Marine Inspector what the draft limitations of the vessel were, nor could they find them when directed to look at their stability letter. Furthermore, they were unable to explain how to read the draft marks. Provide immediate training to Captain and crews on the vessels Stability Letter limitations and how to read draft marks.

A vessel that transports a hazardous material, listed in 49CFR172.101, in commerce, shall ensure the material is handled and transported IAW 49CFR parts 171 through 176. Captain was unable to tell us the exact quantities of propane carried on the vessel and was unaware of the 49CFR requirements. Determine the quantities of hazardous material transported on the ferry, determine applicability of 49CFR and come up with training and plan as necessary.

The owner, master or managing operator shall instruct each crewmember at least once every three months, as to the duties that the crewmember is expected to perform in an emergency, ..... Training shall be logged for review by the Coast Guard upon request. Vessels log book had no fire drills for calendar year 2023 and only one drill of any kind for 2023 calendar year. Vessel crew knowledge for firefighting was substandard. Crewman could not identify location of fire axe. Fire axe was finally found in a cabinet by bridge. Vessel has both CO2 and dry chemical fire extinguishers. Crewman was not aware of the difference, and didn't know what they would choose in the case of a fire. The vessel crew also had a difficult time identifying what the different damper pull stations were for. (which dampers were for stack and which were for ER.). Provide immediate training to all captains and crew prior to the first time they get underway for the weekend. All training should be logged in ships log.

A Fire Control Plan must be posted on the vessel in a location that is accessible to all passengers.... Vessel could not provide a Fire Control Plan. Plan should show the location of all fire protection/emergency equipment. Immediately provide a Fire Control Plan and post on the vessel.

Fire door on port side engine room did not close upon activation.

Crew could not find fire axe. Properly mount in a visible area.

Lazarette hatch did not have appropriate lanyard to keep it from sliding away. Provide lanyard for hatch.

Port shaft parted from red gear coupling.

Main generator shut down while operating.

Various life jackets need retro replaced. Child lifejackets not properly segregated. Go through all lifejackets, repair/replace as needed and ensure proper segregation of lifejackets

During U/W drills, there was no crewman working on the main deck. COI specifically calls for a trained crewman to be on main deck at all times.

During drills, crewman could not explain to CG how to operate ER CO2 system. Provide training and or labelling of system to ensure crew can properly operate.

During drills, crew was unsure how to fight a car fire on the main deck.

Two fire dampers failed during fire drill.

During drills, crew could not communicate with pilot house. Provide proper methods for comms.

Captains of vessel have not been providing proper passenger counts to shore side facility.

Captain could not provide FCC Radio Operators Permit.

Aft Generator has a dry rotted hose coming from keel cooler. Replace hose.

On forward generator, could not find proper approval numbers on fuel hose at engine filter. Provide proper documentation for hoses or replace as necessary.

Vessel must be manned in accordance with its 05Mar2021 amended COI no later than 05Apr2021.

Fuel leak from port main engine.

The vessel is currently broadcasting (1) Zero Length and Beam. (2) Zero Draft,(3) Improper Ship Type. Ship Type should be 60, vise just 6 per the official encoding guide.

FRP patch found on piping system. Temporary repairs not authorized on vital systems.

Sections of hose found in bilge system. Provide proof that the system is unaltered from approved drawings.

Reduced total persons allowed to 349 (344 PAX/5 crew), limited upper bridge deck to 129 total persons and upper 2 decks to 259 total persons pending review/approval of stability calculations by MSC for compliance with new AAWPP of 185 lbs.

Hoses on Starboard Maine Diesel engine: high pressure fuel oil, fast lube oil recovery hose and oil drain hose from the oil sump showed slight wear from contact with other hoses/engine parts. Require chaffing gear to prevent further damage to hoses.

Secure support brackett for CO2 system in Engine Room.
