Each life preserver carried on avessel must have at least 200 sq. cm(31sq. in.) of retroreflective material at-tached on its front side, at least 200 sq.cm on its back side, and at least 200 sq.cm of material on each of its reversiblesides.(b)Retroreflective material requiredbythis section must be Type I materialthat is approved under subpart 164.018of this chapter. 02 Lifejackets and 01 life rings need retro tape replaced.

A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station. Vessel was missing the Oil Dischage Placard.

On a Small Passenger Vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. The bilge high level alarm audible alarm was operating intermittently. Make repairs and demonstrate proper operation of the bilge high level audible alarm sounding continually in the presence of an attending Coast Guard Marine Inspector prior to carrying passengers.

A pre-engineered fixed gas fire extinguishing system on a Small Passenger Vessel must Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. During a test of the system, the engines would not shut down thereby making the system ineffective to extinguish an engine room fire. In the presence of a Marine Inspector, have the fire servicing company demonstrate the proper operation of the fixed fire extinguishing system to include alarm, engine shutdowns, and manual override at the operating station prior to carrying passengers.

The movement of a small passenger vessel shall be under the direction and control of the master or a licensed mate at all times. The Owner/Master of the vessel did not have a current and valid Masters license. The Master's license expired on May 29, 2024, but continued to operate on an expired license. Prior to getting underway with passengers, provide an appropriately licensed captain on board. In addition, provide a copy of their license and proof of enrollment in a random drug testing program to the Coast Guard Marine Inspector. The licensed captain must be in direct supervision of anyone operating the vessel.

On a Small Passenger Vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. The bilge high level alarm in the engine room was inoperable and did not provide any warning at the operating station of flooding in that space. Make repairs and demonstrate proper operation of the bilge high level alarm in the presence of an attending Coast Guard Marine Inspector prior to carrying passengers.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. The vessels current Certificate of Documentation expired on August 31, 2024. Provide a Certificate of Documentation for the vessel and ensure the original is on board at all times whenever the vessel is underway.

An adult life jacket must be provided for each person carried on board a Small Passenger Vessel. The vessel had two, Type II Lifejackets mingled with the other lifejackets stowed in the aft life jacket locker. Provide two (02) additional Type I life jackets for a total of 51 Adult Life Jackets in accordance with the Certificate of Inspection. Vessel is reduced to the carriage of 46 passengers plus 3 crew until the replacement jackets are on board.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. During a test of the electric bilge pumps, the indication light for the bilge pump in compartment #1 was not operational. Make repairs and demonstrate proper operation of the light in the presence of an attending Coast Guard Marine Inspector

(b) All other vessels must undergo an inspection for certification as specified in § 176.404 and annual inspection as specified in paragraph (b)(1) of this section. (1) Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel missed its 2024 inspection and has to conduct a new COI inspection before operating with passengers onboard.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. There were no Fire Buckets at the time of the inspection. Provide 3 fire buckets as required, properly mark and provide lanyards.

Crop & Renew one wasted deck stiffener located in the forepeak tank (Void #1), port side, second stiffener from the forward bulkhead. Stiffener should be cropped from the keel bar to the next longitudinal frame. All repairs must be to the satisfaction of an attending Coast Guard Marine Inspector.

Either Crop and Renew wasted natural goose neck air ventilation pipe on the port side, midship or crop out area of the deck around where the air ventilation pipe was. The minimum plate insert size should be no smaller than 18 inch by 18 inch. All repairs must be to the satisfaction of an attending Coast Guard Marine Inspector.

The owner has requested to make a temporary repair with a wrap around piece and intends to make permanent repairs no later than May 31, 2024 where the vessel will be hauled out again for a subsequent dry dock. No later than May 31, 2024, crop out the cracked section of that corner and insert a new plate with a minimum size of 18 inches by 18 inches. All the work completed must be completed and tested to the satisfaction of an attending Coast Guard Marine Inspector.

Unless otherwise specified, vessel owners must pay the fees required by this subpart before inspection or examination services are provided. 2.10-20(a) General requirements. User Fees not paid.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Main engine shows signs of excessive oil leaks that is creating a excessive accumulation of oil and main engine exhaust is oil soaked and saturated and creates a fire hazard. 46CFR182.200(a)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil that might create a fire hazard. Compartment 2 (Engine Room) has excessive accumulation of oil that might create a fire hazard. 46CFR176.830(a)

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Insulation for the main engine exhaust piping is oil soaked and saturated and creates a fire hazard.

An adult life jacket must be provided for each person carried on board a vessel. 2 Adult Life jackets found unserviceable. Vessel is restricted to the carriage of not more than 46 passengers plus 3 crew until two type I PFD's are provided on board. 46CFR180.71(a)

All equipment, including switches, fuses, lamp holders, etc., must be suitable for the voltage and current utilized. Inspector observed evidence of melting cable to what appeared to be the fuel filter pressure switch, operator must identify cause and replace if needed.

The master of a vessel, except a vessel listed in § 185.502(a) of this part, shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel’s normal berthing location or with a representative of the owner or managing operator of the vessel. The passenger count shall be available to the Coast Guard upon request. Operator unable to show availability of written count ashore at the vessel’s normal berthing location or with a representative of the owner or managing operator of the vessel.

The Coast Guard shall evaluate emergency drills to ensure that each crew member is familiar with his or her duties in case of emergencies including response to fires, man overboard and abandon ship in accordance with 46 CFR 185.520 and 46 CFR 185.524. Because of material deficiencies on vessel, the Coast Guard was unable to witness emergency drills during this annual inspection. Owner must make crew available for drills.

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of Steel hull vessels. Repair washed out weld on strut. 46CFR177.300(b)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. No operable portable battery light at the access to the propulsion machinery space. 46 CFR 183.430

A vessel of 12 meters or more in length shall be provided with a whistle. Whistle non functional. 33 CFR 83.33

Lighting fixtures: Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Guards were missing on outside deck lighting and Compartment 1 lighting on starboard side. The vessel will be restricted from carrying passengers after October 8, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 46CFR183.410(a)

Unsafe practices: At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil that might create a fire hazard. Compartment 2 (Engine Room) needs to be cleaned of oil. The vessel will be restricted from carrying passengers after October 8, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 46CFR176.830 (b)

Unsafe practices: At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive trash, debris, or other matter that might create clog bilge pumping systems. Compartment 3 need to be cleaned of debris. The vessel will be restricted from carrying passengers after October 8, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 46CFR176.830 (b)

General requirements. In systems and applications where flexible hoses are permitted to be clamped, Double hose clamping is required where practicable. 2 locations on Keel cooler inlet port side need double hose clamps. Marked in blue tape. The vessel will be restricted from carrying passengers after October 8, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 46CFR182.410(d)(1)

General design, installation, and maintenance requirements: Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Cable entering battery charger box is not protected from abrasion. The vessel will be restricted from carrying passengers after October 8, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 46CFR183.200 (b)

Protection from wet and corrosive environments: Electrical equipment used in a machinery space must be dripproof. Breaker and terminal on port side aft ER bulkhead is not drip proof. Dripproof means enclosed equipment so constructed or protected that falling drops of liquid or solid particles striking the enclosure at any angle from 0 to 15 degrees downward from the vertical do not interfere with the operation of the equipment. A National Electrical Manufacturers Association type 1 enclosure with a dripshield is considered to be dripproof. The vessel will be restricted from carrying passengers after October 8, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. 46CFR183.210(a)(1)

A vessel on a lakes, bays, and sounds, or rivers route must carry (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Provide Vessel 2 hand orange smoke or 2 red had flares per 160.021. 46CFR180.68

Lifesaving equipment markings 185.604(f) The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Enable DSC VHF capabilities past the baseline per 46CFR80.905.

Provide a marine operators permit or higher class license to all persons who operate the marine radio per 47CFR80.159(e)(1).

01-Demonstrate proper operation of all bilge high level alarms prior to carrying passengers and to the satisfaction of an attending Coast Guard Marine inspector in accordance with 46CFR182.530

02-Demostrate proper operation of the emergency fuel shut offs prior to carrying passengers and to the satisfaction of an attending Coast Guard Marine inspector in accordance with 46CFR182.455(b)(4)

03- Properly install a forward mast head light with an arch of visibility of 225 degrees and a stern navigational light with a arch of visibility of 135 degrees in accordance with 33CFR83. 22 and 23. The all around light is only acceptable for a vessel less than 12 meters (39.4 feet) in accordance with 33CFR83.23(d)

04- Install both port and starboard running lights in a vertical position in accordance with 33CFR84.02. Running lights are currently positioned in a horizontal arrangement.

05- Properly post all pages of the COI in a conspicuous place so that passengers can see it in accordance with 46CFR176.302.

06-Properly post stability letter dated April 23rd, 2007 in the operating station so it can be viewed by the master in accordance with 46 CFR 176.306

07- Provide a safety radio certificate in accordance with 40CFR80

08- Provide and post emergency instructions that meets the requirements of 46CFR185.510 & .512

09- Provide and post a life jacket placard for every type lifejacket onboard or need to provide 4 of the York type PFD's

10- Remove all power strips in the operating station in accordance with 46CFR183.200.

11- Provide suitable covers for all exposed lighting in the engine room 46CFR183.200.

Remove all oily water mixture from the bilges in the machinery space and properly dispose of the same. Bilges should be pumped to a minimum content at all times in accordance with the vessel's stability letter.

Make repairs and demonstrate proper operation of the float activation switch in way of the electric submersible bilge pump in the machinery space.

Properly repair wasted through hull pipe in the foward space that penetrates the bulbous bow to the satisfaction of an attending Coast Guard Marine Inspector.

Vessel must have 6 orange smoke flares. 46 CFR 185.614

Lifebuoy light lanyard is wasted and must be renewed. 46 CFR 180.70(d)(3)

One of the two required lifefloats is in unservicable condtion. 46 CFR 180.175

Life jacket stowage was not marked in accrodance with regulations. 46 CFR 185.604

Several items in first-aid kit were beyond expiration date.

Lanyards on floatlights for both life floats were deteriorated and needed to be replaced. Vsl restricted to L, B, & S route until this is corrected.

Hydrostatic release for EPIRB was beyond expiration date. Vsls route restricted until new release is installed.

The generator does not shut down when system is activated, the main engine does.

Vessel did not have a fixed fire extinguishing system in the engine room as required.

Waterlight did not have a lanyard attached.

Storage areas for lifejackets were not properly marked.

Vessel missing 3 red flares and 3 orange smokes for the route requested.

Fuel tank vents did not have flame screens installed.

Portable extinguisher near exit of passenger space not properly mounted.
