Employers must maintain records of chemical tests. Vessel did not have current record of drug and alcohol consortium enrollment or proof of chemical testing for crewmembers, or current (2024) MIS report data from consortium. 46 CFR 16.260

Each portable fire extinguisher on a vessel must be of an approved type and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG-4). All portable fire extinguishers and pre-engineered fixed gas fire extinguishing system onboard were overdue for annual servicing (expired JUN 2025). 46 CFR 181.500(a) 46 CFR 181.420

A vessel must comply with the applicable requirements for any radio and EPIRB installations required by the FCC. NOAA registration certificate and decal for installed EPIRB was expired (SEP 2024). EPIRB hydrostatic release unit was also expired (DEC 2024). Vessel is restricted to a Lakes, Bays, and Sounds route until rectified. 46 CFR 184.502 46 CFR 185.740(b)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel's starboard running light was inoperable. Vessel is restricted to daytime operations only until light is working. 46 CFR 183.420

Each life float must be fitted with a light. Floating marker light for vessel's life float was inoperable. Vessel is restricted to daytime operations only until light is working. 46 CFR 180.175

Each life float must be marked with the vessel's name and the number of person capacity, with lettering of at least 2 inches high. Vessel's life float was not properly marked. 46 CFR 185.604

A visual and audible alarm must be provided at the operating station to indicate a high water level. High water level alarm in vessel's lazarret was not working. 46 CFR 182.530

Vessel's fuel shutoff valve was functional, but the valve handle was severely corroded and brittle. Replace valve handle. 46 CFR 182.455(b)(4)

The required fire extinguisher maintenance (monthly checks) shall be logged. MI observed insufficient proof that maintenance was being performed. 46 CFR 176.810(b)(1)(iv)

Each EPIRB must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative, and shall be logged. MI observed insufficient proof that maintenance was being performed. 46 CFR 185.728(a),(c)

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: For fixed-gas fire extinguishing systems, the inspections and tests re- quired by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility may be required to perform the required inspections, maintenance procedures, and hydrostatic pressure tests. Vessel's fixed firefighting system cylinder in engine compartment was in need of re-charge. Provide servicing report showing that cylinder has been charged and the engine shutdown override has been tested. 46 CFR 176.810(b)(2)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Vessel's high water alarm in aft void was not working. 46 CFR 182.530(a)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. The orange color and retro on the life float was faded. 46 CFR 180.175

A vessel to which this part applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel was due for credit dry dock inspection on 31May24. Vessel shall not operate with passengers until credit dry dock inspection is completed. 46 CFR 176.100

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] observed broken fitting in the deck up forward, which created an opening in the deck. Vessel rep removed the fitting and installed a metal plate as a temporary repair. Vessel rep must submit a repair proposal for approval and subsequently complete a permanent repair prior to next credit drydock. 46 CFR 176.700(a) 705c (prior to credit drydock)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. MI observed that the audible/visual alarms for all three installed high level alarms were inoperable. 46 CFR 182.530(a)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed life ring buoy was fitted with a grab line and retroreflective tape that were failing and required replacement. 46 CFR 176.808(b)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. MI observed master did not have a radio operator permit. Vessel is limited to operating within 1000 feet from shore. 47 CFR 80.159(e)(1)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed that the international orange color of the lifefloat was obscured by grime and the retroreflective tape was failing. Vessel route is restricted to Lakes, Bays, and Sounds. 46 CFR 176.808(b)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. MI observed hydrostatic release unit for the EPIRB was not marked with an expiration date. Vessel route is restricted to within 3 miles from shore. 46 CFR 185.740(b)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. MI observed the aspirin, a required item per the regulations, was expired. 46 CFR 184.710

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] observed that the opening in the deck up forward to permit the use of a secondary anchor line was missing its cover. 46 CFR 176.700(a)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Coast Guard Light List. MI observed vessel did not have a copy of the Light List onboard. 46 CFR 184.420(a)(4)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its design limitations. The operator notified Sector St. [name withheld] Division for a marine casualty, main engine failure, on the DIVERSION. Make satisfactory repairs to the main engine and contact a USCG MI to verify repairs. 46 CFR 182.00 and 46 CFR 176.700

New installations of electrical equipment and material must comply with 46 CFR 183. USCG inspector identified "new" electrical installations that do not match original approved plans for the vessel after receiving a drawing of the new DC fuse panel installations. The operator shall submit a new one line electrical diagram and load analysis for DC electrical systems to the Marine Safety Center for review and identify new electrical installations in the submission that differ from the original approved electrical plans. Contact USCG Sector St. [name withheld] Division once plans are reviewed/approved and ready for OCMI inspection. 46 CFR 183.115(c)

Fuses must be of the cartridge type only and be listed by the Underwriters Laboratories or another independent laboratory recognized by the commandant. Replace blade fuses with UL cartridge type fuses. 46 CFR 183.380(l)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Negative conductor from battery to engine mount found severed at the negative battery terminal. 46CFR183.100(b)

The materials and construction of fuel lines, including pipe, tubs, and hose, must comply with 182.455. Compression fitting on fuel injector #3 found leaking fuel on engine block. 46CFR182.455(a)

02- The owner of a vessel must present title evidence in accordance with one of the methods specified: When the ownership of a documented vessel changes. pProvide proof of updated COD reflecting new ownership.

47 CFR 80.401- Radiostations are required to have current station documents as indictated in the table in 80.401. Provide current call sign on radio safety certificate and EPIRB registration.

46CFR 180.175 (a), (f) (1)(2)(3), The painter line on the lifefloat must have a breaking strength of 1,500 lbs, be at least 100 ft, and be of dark color. The lifeline and pendants must meet the requirements of 160.010. The light must be a floating waterlight under approval of 160.010. Each item of survival craft equipment must be in good quality and efficient for the purpose intended to serve. Replace lifeline, light, painter, and paddles to meet requirements and be in satisfactory conditions.

46 CFR 182.70- A vessel must carry one floating waterlight, which must be in accordance with 46CFR 161.010. Watertight batteries are expired and must be replaced.

46CFR 176.810- Portable Extinguishers must be inspected and maintained in accordance with NFPA 10. Portable extinguisher for machinery space missing pin. replace to meet maintenance standard.

46CFR 179.360- A hatch exposed to the weather must be watertight, replace gaskets to make watertight.

46CFR 180.205 (e)- Each vessel certificated to operate on limited coastwise route must have an EPIRB installed to float free and activate. EPIRB mount needs to be replaced to good condition.

Electrical equipment on a vessel must be installed and maintained to provide necessary safety. Electrical equipment used in a location exposed to the weather must be watertight. Battery containment is holding water

46 CFR 181.420 (b) (1)-A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: a light to indicate discharge and an audible alarm that sounds upon discharge. Vessel does not have audible and visual alarm.

46 CFR 176.810 (b) (1)- Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Portable fire extinguisher on starboard side had broken hose clamp and was unable to be used.

46 CFR 185.728 (b)-The EPIRB's battery is replaced after it is used or before the date required by FCC regulation, whichever comes sooner. EPIRB battery is expired and needs to be replaced.

46 CFR 180.175 (f) (1)-Life float lifeline and pendants must be as furnished by the manufacturer with the approved life float. Life float lines are deteriorated and need to be replaced.

46 CFR 180.70 (b) (1)-Each ring life buoy must be approved in accordance with subchapter Q. Ring Buoy is cracked and has frayed line.Ring Buoy needs to be replaced.

46 CFR 180.68 (a) (1) (2)-A vessel on a limited coast-wise route must carry six red hand flare distress signals and six orange smoke distress signals.

46 CFR 67.7- Any vessel of less than five net tons must have a Certificate of Documentation. COD expired 31 march 2019.

46 CFR 184. 420 (a)-As appropriate for the intended voyage a vessel must carry adequate and up-to-date charts, coast pilot, light list, and tide tables.

47 CFR 80.905 (a) (1)-Vessels operated solely within 20 nm of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of 180.1101(c)(2).

46 CFR 16.210 (a)-No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. Employees have not gone through pre-employment drug testing.

46 CFR 4.06-3 (a) (1) (i)-Alcohol testing following a serious marine incident must be conducted within 2 hours of when the SMI occurred. Alcohol strips were expired and couldn't be used to test.

46 CFR 183.340 (b) (4)-All cable and wire must be installed with metal supports spaced not more than 24 inches apart. Cable coming off of battery leading to engine on starboard side was hanging in bilge.

46 CFR 182.530 (a) -On a vessel of at least 26 feet a visual and audible alarm must be provided at the operating station to indicate high water in the space. The visual indication for the engine room and steering space was inoperable.
