Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Vessel's life floats was not marked with Type II retroreflective tape. Mark life floats with Type II retroreflective tape IAW with IMO Resolution A.658(16). 46 CFR 185.604(i)

Each ring life buoy on a vessel must: Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150; Ring life buoys plugs are deteriorated causing an open hole in the two life buoys. There is a potential for water intrusion which reduces the design capabilities of the ring life buoys. 46 CFR 180.70.b.1 16 C

Cable and wire for power and lighting circuits must: (1) Meet Section 310-13 of NFPA 70 (incorporated by reference; see 46 CFR 175.600) except that asbestos insulated cable and dry location cables may not be used; (2) Be listed by Underwriters Laboratories (UL), as UL Boat or UL Marine cable; or (3) Meet § 111.60-1 in subchapter J of this chapter for cable, and § 111.60-11 in subchapter J of this chapter for wire. Vessel was found with ROMEX wire installed powering the air conditioning system Remove ROMEX wiring. If vessel intends to utilize system, submit electrical plans as follows. Either 1) Documentation to attending USCG marine inspector showing restoration to original approved electrical configuration or 2) Provide plans to MSC showing bill of materials, load analysis, single line diagram and overcurrent protection for modified system.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Vessel did not have a complete first aid kit available.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. The records shall be sufficient to: (1) Satisfy the requirements of §§ 16.210(b) and 16.220(c) of this part. (2) Identify the total number of individuals chemically tested annually for dangerous drugs in each of the categories of testing required by this part including the annual number of individuals failing chemical tests and the number and types of drugs for which individuals tested positive. Records showing compliance for the crew members employed on the vessel were not available.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation (COD) currently in effect for that vessel. Vessel did not have valid COD on board. Provide proof of valid COD to attending Marine Inspector.

Employers must ensure that they and their crew-members meet the requirements of 46 CFR Part 16. Vessel could not provide proof of Crew's enrollment in a drug and alcohol program required by 46 CFR Part 16. Provide proof of crew enrollment in required drug and alcohol program as required by 46 CFR Part 16 to attending Marine Inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel's charts were not updated. Provide current/corrected charts for operational areas.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Retro-reflective material was wasted on all ring life buoys. Repair/replace retro-reflective tape to satisfaction of attending Marine Inspector.

Strong and effective rudder stops must be provided to prevent jamming and damage to the rudder and its fittings. These stops may be structural or internal to the main steering gear. Rudder stops were not effective and left gaps between rudder stop and rudder fittings (ram). Make rudder stops effective to satisfaction of attending Marine Inspector.

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Public address system was not adequate in accommodation spaces for crew. Make public address system adequate to attending Marine Inspector.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. The emergency instructions must be written/detailed to address the current procedures of the vessel and to the satisfaction of the attending Marine Inspector.

The master of the vessel shall ensure that each EPIRB,must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; and that the EPIRB test shall be logged or otherwise documented, as applicable. Vessel was not conducting EPIRB test or recording testing. Begin recording and testing EPIRB to the satisfaction of attending Marine Inspector.

Provide weak link for portside life floats.

Provide alcohol swabs.

Remove extendion cords and power strip from main deck utility and lifejacket lockers.

Provide stern light showing proper arc of visibility.

Repair minor exhaust leak on port center MDE.

Prove proper operation of lighting in forward hallway and 2 forward state rooms.

Renew 28 lifejacket lights.

Properly weld hull puncture in fwd peak.

After 12JUN17, complete COI inspection in the water prior to carrying passengers or cargo.

Repair weeping hydraulic steering piping port side.

Repair electrical junction box port side.

Replace broken window on aft lounge door.

Replace gasket on water pump for stbd genset.

Renew insulation on port genset exhaust piping.

Prove proper operation of re-built fire pump after installation.
