The Rules concerning shapes shall be complied with by day. Vessel does not have dayshapes on board. Acquire dayshapes and carry onboard.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16). Retroreflective material on lifejackets was faded. Replace retroreflective material on lifejackets to satisfaction of Marine Inspector.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: (1 ) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. (2) Where the station power exceeds 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a general radiotelephone operator license or higher. Vessel Master did not have available Federal Communications Commission Marine Radio Operator Permit. Provide valid Federal Communications Commission Marine Radio Operator Permit.

The master of the vessel shall ensure that each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. Vessel master was unfamiliar with EPIRB testing and operating procedures. Provide copy of recorded training, identifying all vessel masters demonstrating monthly EPIRB test, using the integrated test circuit and output indicator, to determine that it is operative.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Numerous dead-end wires exposed under helm. Remove/make wires in compliance with regulations to satisfaction of attending Marine Inspector.

Vessel log missing EPIRB tests and fire extinguisher checks for 2021. Provide completed log.

Each vessel that operates on the high seas, or that operates beyond three miles form the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHZ EPIRB, installed to automatically float free and activate. The installed EPIRB was found to have an expired hydrostatic release. Provide current hydrostatic release as part of the float free arrangement.

Prove proper operation of E/R submersible bilge pump prior to carrying passengers.

Repair delaminated FRP on port bow as marked.

Make E/R forward bulkhead portside vapor tight.

Remove dead end wire in #3 compartment.

Remove out of service bilge section line in #3 compartment.

Remove anchor chain for further inspection.

Hard wire domestic water pump in laundry room.

Remove hydraulic oil in lazarette.

Provide 6 red hand flares & 6 orange smoke flares (or equivalent).

Provide portable bilge pump with enough hose to pump deepest bilge.

Connect genset to Fireboy engine shutdown control prior to using genset with passengers aboard.

Provide plans/documentation for new generator

Install rail courses on upper deck so as not to exceed 12" between courses.

Replace port MDE cooling hose.

Prove proper operation of stbd navigation light

Secure wet exhaust leak to stbd genset.

Refasten retaining device to port side deck hatch cover.

Secure portside water feed line to stringer in engineroom.

Stabilize and secure engine room deck plating in way of the ladder landing

Prove proper operation of fixed bilge system

Replace/repair port genset exhaust insulation.

Service hatches on aft weather deck.

Prep & coat corroded cooling water inlet on port engine.

Adjust packing on both shafts due to excessive leaking.

Repair leaking raw water strainer.

Stanch hydraulic fluid leak from out-of-service hydraulic pump in E/R.

Remove oily rags/sorbent pads from E/R bilge.

Hydro was expired on the day of the inspection.

Provide updated chart of operating area as per 46 CFR 184.420

Prove the proper operation of the MF radio, limited to 20 mile to shore

Supervisor EAP training not documented.

Drug testing background checks not maintained in accordance with 49 CFR 40.25

Failure to keep positive drug test results for 5 years in accordance with 49 CFR40.333.

Marine Employer failed to use scientifically valid method for random selections. Names were drawn from a hat.

Provide evidence newly-installed flexible fuel line (hose) on port engine meets SAE J 1942-1. Present hose is cloth-covered w/ no fire sleeve.
