Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here:Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Portable fire extinguishers are not being inspected monthly. Conduct inspections and log appropriately on tag or separate record.

For each marine casualty required to be reported by § 4.05-10, the marine employer shall determine whether there is any evidence of alcohol or drug use by individuals directly involved in the casualty. Written report must include: Specifies the method used to obtain such evidence, such as personal observation of the individual, or by chemical testing of the individual. Alcohol and chemical tests found expired. Replace tests.

THE MASTER AND CREW OF A VESSEL WILL BE FAMILIAR WITH THE CONTENT OF AND HAVE MOUNTED AT THE OPERATING STATION, EMERGENCY INSTRUCTIONS CONTAING THE ACTIONS TO BE TAKEN IN THE EVENT OF FIRE, HEAVY WEATHER, OR MAN OVERBOARD CONDITIONS.

PROVIDE DOCUMENTATION THAT ALL MEMBERS SERVING IN SAFETY SENSITIVE POSITIONS ARE SUBJECT TO TESTING FOR DANGEROUS DRUGS ON A RANDOM BASIS.

A VESSEL MUST CARRY EITHER A FIRST AID KIS APPROVED UNDER APPROVAL SERIED 160.041 OR A KIT WITH EQUIVALENT CONTENTS AND INSTRUCTIONS. THE KIT WAS MISSING ITEMS REQUIRED AND DID NOT CONTAIN THE INSTRUCTIONS LISTED IN 160.041

EACH VESSEL MUST BE EQUIPPED WITH AT LEAST 2 OPERABLE PORTABLE BATTERY LIGHTS. ONE OF THESE MUST BE LOCATED AT THE OPERATING STATION AND THE OTHER AT THE ACCESS TO THE PROPULSION MACHINERY SPACE.

AN ADULT LIFE JACKET MUST BE PROVIDED FOR EACH PERSON CARRIED ON BOARD A VESSEL. 2 KAPOK STYLE LIFEJACKETS WERE IN UNSERVICABLE CONDITION AND PLACED OUT OF SERVICE. REPLACE AND ENSURE PROPER NUMBER OF LIFEJACKETS ARE STOWED IN EACH POSITION.

PLACARDS CONTAINING INSTRUCTIONS FOR THE DONNING AND USE OF THE LIFE JACKETS ABOARD THE VESSEL MUST BE POSTED IN CONSPICUOUS PLACES THAT ARE REGULARLY ACCESSIBLE AND VISSIBLE TO CREW AND PASSENGERS.

EACH RING LIFE BUOY WITH A FLOATING WATERLIGHT MUST HAVE A LANYARD OF AT LEAST 3' BUT NOT MORE THAN 6' SECURING THE WATERLIGHT AROUND THE BODY OF THE RING LIFE BUOY

THE MASTER SHALL ENSURE THAT ONE OR MORE PLACARDS MEETING THE REQUIREMENTS OF THIS SECTION ARE DISPLAYED IN PROMINENT LOCATIONS AND IN SUFFICIENT NUMBERS SO THEY CAN BE READ BY THE CREW AND PASSENGERS. PROVISION MUST BE MADE FOR CLOSING ALL SUPPLY DUCT COWLS OR SCHOOPS AND EXHAUST DUCT DISCHARGE OPENINGS FOR A SPACE PROTECTED BY A FIXED GAS EXTINGUISHING SYSTEM. ALL CLOSURE DEVICES MUST BE READILY AVAILABLE AND MOUNTED IN THE VICINITY OF THE VENT.
