The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Deckhand was not listed on DAPI 46CFR16.230

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. Master did not have medical certificate. 46CFR10.301(a)

A vessel on a lakes, bays, and sounds or rivers route must carry: (1) Three hand red flare distress signals... and (2) three hand orange smoke distress signals. Vessel had four red hand flares. Provide proof of purchase prior to carrying passengers. 46 CFR 180.68(b)(1)(2)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel had one operable portable light. 46 CFR 183.430

At each initial and subsequent inspection for certification of a vessel the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of life saving equipment and systems. Operator could not demonstrate successful recovery of "man overboard" drill as a single operator. Vessel SHALL NOT operate as a single operator. 46CFR176.808

Each survival craft must be: stowed in a position that is readily accessible to crew members for launching child life jackets were stowed under various items. 46CFR180.130(a)(3)

In addition to meeting 180.130 each life float must be stowed as required by this section certified to meet 160.073 in subchapter Q of this chapter. Life float is not serviceable. Vessel is restricted to operating only during warm water months. 46CFR180.137(a)

Placards containing instructions for the donning and use of the lifejackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel did not have placards for all types of PFDs onboard. 46CFR185.516

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Vessel does not have emergency procedures mounted at operating station. 46CFR185.510(a)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the [name withheld] extinguisher bracket mount missing base and not properly supporting at the operating station. 46CFR181.520

When the flexible section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. Fuel tank did not have a conductor. 46CFR182.445

A vessel not required to have a power driven fire pump by 46CFR181.300 must have at least three (2.5 gallon) buckets, with an attached lanyard, placed so as to be easily available during an emergency. The words "Fire Bucket" must be stenciled in a contrasting color on each bucket. Buckets did not say Fire Bucket and did not have lanyards. 46CFR181.610

Each vessel constructed of wood certificated to operate on a lake/bays/sounds route in cold water must be provided with life floats that will accomodate 100% of the total number of persons on board. Vessels lifefloat was unserviceable, vessel is restricted to operating in warm water until serviceable lifefloat is inspected.

Sidelights must show minimum required intensities. Vessels sidelights did not illuminate. Vessel is restricted to high visibility and daylight operations until replaced.

Vessels must carry up to date and adequate charts, US Coast Pilot, Coast Guard light list, and tide tables for intended voyage. Vessels charts and publications were all out of date.

The master shall conduct sufficient drills. Master was unable to complete man overboard drill as a single operator.

Found stowage marking of life jackets incorrect. Need to annotate "adult life jackets".

Found lifefloat light lanyard not secure to the body of the lifefloat and not 18' standard.

Found paddle in lifefloat not labeled with vessels name, relabel paddle.

Found name of vessel on lifefloat faded. Relabel lifefloat with vessel name.

Found ring buoy at operating station deteriorating; needs replacement.

Found vessel did not have updated charts for Area of Operation. Replace charts to the satisfaction of the MI.

Found portable diesel tanks stored throughout the vessel. Portable tanks prohibited on the vessel.

Found significant rot on second support beam on the port side below the top of the bulwark. Submit repair proposal.

Found no emergency placard for CO2 system by the engine space. Provide placard with operating instructions.

Donning procedures for life jackets not posted and not onboard. Vessel has two types of life jackets. Procedures for both need to be posted. Correct to the satisfaction of Coast Guard Marine Inspector.

Emergency radio call procedure plaque is not legible. Replace to the satisfaction of Caost Gaurd Marine Inspector.

Renew First Aid/CPR certification at earliest convience in accordance with local policy. CID Note #14

Vessel is missing alcohol testing strips.

Vessel's U.S. Coast Pilot was 10 years old with no updates.

found that vessel’s servicing tags had holes punched for 2017 and 2018; informed vessel representative that we would follow up with the servicing company to verify procedures/intent. Received notification from the fire extinguisher servicing company that there is only one size hole punch that they use. The smaller hole punch was for 2017. There was a larger hole punched over 2018 to show that the servicing was conducted in 2018. According to the servicing company, the larger hole is fraudulent and the company would not issue/keep and service tag with two different dates/years.

Alcohol Testing kit (M-PAK) had expired year (2014).
