07106 - Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. The interconnected fire detection in the forward space did was inoperable. 46 CFR 181.405(c)

02107 - Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. When tested, the fuel shutoff valves did not operate correctly. 46 CFR 182.20-40(b)(3) "Old T"

11118 - Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter. The child life jackets do not have lights attached. 46 CFR 180.25-20(a) "Old T"

05112 - Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. There were not EPIRB tests logged. 46 CFR 185.25-20(a) "Old T"

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. Vessels was found making repairs in the local OCMI zone without notification to loacal [name withheld] are to be witnessed and tested before vessel is placed back in the water.

The owner or managing operator shall make a vessel available for drydock examinations, internal structural examinations at the required intervals. A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel was due a drydock/ISE/Fastener Pull by 11/30/2022. Vessel failed to complete inspection and did not get a DD Extension.

46 CFR 177.405(d) Paint and flammable liquid lockers must be constructed of steel or equivalent material, or wholly lined with steel or equivalent material. Vessel is using its lazarette as a paint locker. Install paint locker or remove paint and flammable liquids from vessel. Vessel must complete prior to completion of drydock.

46 CFR 184.202(b) Fireplaces or other space heating equipment with open flames are prohibited from being used on all vessels. Vessel has wood stove located in accommodation space and is resticted from using while passenger are onboard.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. Fuel shutoff valves on STBD day tank and STBD main tank did not positively seat closed when stops were pulled during the inspection. 46 CFR 182.455(b)(4)

Vessel did not have means to test blood alcohol level.

Vessel is missing two red distress flares.
