The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessels alcohol test strips are expired. 46 CFR 4.06-15 (a)

The VSO must ensure that at least one security drill is conducted at least every 3 months, except when a vessel is out of service due to repairs or seasonal suspension of operation provided that in such cases a drill must be conducted within one week of the vessel's reactivation. Security drills may be held in conjunction with non-security drills where appropriate. Vessel has not conducted security drills as required by applicable regulations. Conduct security drill.

All personnel must complete the training specified in paragraphs (d)(1)(ii) through (v) of this section by January 12, 2026, and annually thereafter. Crew did not complete cyber security training. Complete cyber security training. 33 CFR 101.650(d)(4)

Found 02 cracked welds on longitudinal framing inboard of the clete support framing on port and starboard sides. Complete repairs prior to credit drydock/within 14 days.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flares... and six hand orange smoke distress signals. Identified 04 missing flares. Limit route to lakes, bays, and sounds until flares onboard. 46CFR117.68 (a)(1) & (2)

Vessels described by 46 CFR 114.110(e) must have an interconnected fire detection system in compliance with 46 CFR 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. MI Identified no fire detection in the accommodation space. Submit repair plan within 30 days. 46 CFR 118.400(d)

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of—(1) The location of emergency exits, survival craft embarkation areas, and ring life buoys;(2) The stowage location(s) of life jackets;(3) Either:(i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or(ii) That passengers may contact a crew member for a demonstration, as appropriate, prior to beginning an oceans or coastwise voyage;(4) The location of the instruction placards for life jackets and other lifesaving devices;(5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master; and(6) If the vessel is operating with reduced manning or equipment requirements in § 115.114 of this chapter. A fire hose with a nozzle must be attached to each fire hydrant at all times. Port side fire station did not have the fire hose connected. Crew connected hose to the hydrant after being informed of requirement. 46 CFR 118.320(a)
