All other vessels must undergo an inspection for certification as specified in 176.404 and annual inspection as specified in paragraph (b)(1) of this section. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel lapsed 1st Annual Inspection, due 24OCT2025 (COI issued 24JUL2024). No scheduled or requested inspection date. Submit CG-3752 to schedule COI renewal inspection.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel was involved in several collisions, allisions, and groundings during transit from Jacksonville, FL. OCMI to conduct onboard Damage Survey prior to movement. 46 CFR 115.840

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Conduct dry dock or provide dive report of props, shafts, rudders, and underwater appendages due to vessel running aground. 46 CFR 115.840

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Vessel does not have current charts, navigation publications, or tides/current tables. 46 CFR 121.420 (a)

A vessel on a lakes, bays, and sounds, or rivers route must carry: (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel only had two of the three orange smoke distress signals onboard. 46 CFR 117.68(b)

Each vessel without overnight accommodations certificated to operate on a lakes, bays, and sounds route must: (2) If operated in warm water, be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. Vessel has no ready survival craft. All three of vessel’s life floats are in an unserviceable condition. Additionally, their stowage does not comply with 46 CFR 117.130 and 46 CFR 117.137; and their outfitting does not include the required equipment detailed in 46 CFR 117.175. Rectify prior to operating more than one mile from land. 46 CFR 117.207(b)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Window on starboard side, amidships is shattered. 46 CFR 116.100(a)

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in … normally unmanned spaces. Visual alarm for engine room bilge alarm was inoperable. 46 CFR 119.530(a)

46CFR115.100(a): A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessels COI authorizes operation within a Lakes, Bays, and Sounds route. Vessel is not authorized to transit beyond the boundary line. Prior to transiting beyond the boundary line, the vessel must stop and coordinate attendance of local marine inspectors from Sector Delaware Bay.

All self-propelled vessels must be fitted with a Federal Communications Commission (FCC) type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station. Radar became inoperable during required tests. Fix or replace with operable radar array. 46 CFR 121.404

All vessels must have navigation lights that are in compliance with Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must UL1104. Navlights were not of an approved type. Replace all Nav lights with UL1104 approved fixtures.

In addition to the radiotelephone required by paragraph (b) of this section, each vessel described in paragraph (a) of this section while transiting any waters within a Vessel Traffic Service Area, must have on board a radiotelephone capable of transmitting and receiving on the VTS designated frequency. Vessel only has one marine VHF radio installed. Install additional VHF radio.

At each inspection for certification all observed unsafe practices, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Vessel's bow gangway support cabling, turnbuckles, shackles, in severely deteriorated and wasted condition. Replace all support cabling, turnbuckles, shackles, u-bolt clamps. Vessel is restricted from using forward gangway to embark/ disembark passengers. 46 CFR 115.830(a) 16Sept update: Wire replaced but saddle clamps installed wrong.

Required chemical testing: Employers; must ensure all agreements an arrangements concerning the implementation of the Drug and Alcohol testing requirements are in compliance with these provisions. Vessel's DAPI was missing valid enrollment letter, listing of employees enrolled. Ensure required information is included in plan and properly implemented. 46 CFR 16.203(a)

All vessels shall be provided with satisfactory arrangement for draining any watertight compartment. Vessel was unable demonstrate successful dewatering through Void #3 bilge suction line. Repair line and demonstrate proper operation of bilge suction line. 46 CFR 182.25-1(a)

A-Class bulkheads or decks must be composed of steel. In addition, they must be so insulated with approved structural insulation. Two areas identified: 1.)(RE-VERIFY insullation installed within false Overhead) Boundaries between High Risk Accommodation(7) areas and High Risk Accommodation(7) Areas shall be A-60... e.g. Deck 1 and Deck 2, insulation in the false overhead attached to the above deck steel was found to have fallen from originally installed areas in multiple locations. 2.)(CLEARED 28AUG2024) Boundaries between Machinery Spaces(10) and High Risk Accommodation(7) Areas shall be A-60... e.g. Engine Room and Deck 1, insulation on the overhead was found to have fallen from originally installed areas in multiple locations. Re-install approved structural fire protection insulation in the above areas. 46 CFR 116.415(a)(2)(i)

A fire hose must be attached to each fire hydrant at all times. Engine room fire hose brass fitting was found split rendering the hose ineffective against manufacture's design. Replace engine room fire hose with approved UL 19 compliant fire hose. 46 CFR 118.320

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Vessel's engine room bilges was observed to have an excessive accumulation of oil around all running machinery. Oil absorbent mats were observed all through out engine room bilges, soaked through, with pooled oil all around. Remove excessive oil and clean engine room bilges.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Engine room was observed to have Exhaust leak in exhaust piping on machinery. 46 CFR 115.800(a)

Battery chargers must have an ammeter connected in the charging circuit. Battery charger under the bar on the second deck that powers the forward gangway winch, was identified to not have an approved ammeter in the circuit. 46 CFR 120.350(f)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency the operator must secure each Type III device acceptable methods include: (1) Closing each valve leading to an overboard discharge and removing the handle;(2) Padlocking each valve leading to an overboard discharge in the closed position; or(3) Using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position. Port side discharge valve was observed open. secure valves to one of the three standards above. 33 CFR 159.7(c)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. While conducting engineering checks of operating machinery. Vessel's both main engines and both auxiliary generators were observed actively leaking oil out of valve covers. 46 CFR 115.800

"The area of operation for each vessel and any necessary operational limits are determined by the cognizant OCMI..." 1)Do not enter Sector New York COTP Zone until you have met the requirements outlined below or have been otherwise authorized by the Captain of the Port via the Vessel Traffic Service. 2) All movements and transfer of control within the COTP Zone of New York and New Jersey must be in accordance with a tug assist plan reviewed by Vessel Traffic Service. This tug assist plan must be submitted in writing to this office for review prior to entering the COTP Zone. 3) Coordinate and report movements and towing evolutions within the COTP Zone through Vessel Traffic Service. 46 CFR 115.110(a)

Newly- hired vessel employees may be granted the access provided for in the paragraph (a) of this section only if: (1) the new hire has applied for a TWIC in accordance with 49 CFR part 1572... Newly hired engineer has not applied nor hold a VALID TWIC. New engineer/crew must be escorted into areas with restricted access. 33 cfr 104.267

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:Charts.... Inspectors identified that the charts on board were not up to date. Update charts to recent edition. 46 CFR 121.420

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Inspector identified that the radio equipment on board is without MMSI registration. 46 CFR 121.502

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level... Operators informed inspectors that their high water alarm lights were not working as well as not labeled. 46 CFR 119.530

Windows and other openings at the operating station must be of sufficient size and properly located to provide an adequate view for safe navigation in all operating conditions... Inspectors found motors for wipers removed with exposed wiring on the pilothouse windows. 46 cfr 116.1030

Glass and other glazing material used in windows must be of material that will not break into dangerous fragments if fractured... Inspectors found cracked windows and plexiglass. outfit vessel with proper windows 46 CFR 116.1010

Each lifeline on a ring life buoy must:(1) Be buoyant;(2) Be at least 18.3 meters (60 feet) in length;(3) Be non-kinking;(4) Have a diameter of at least 7.9 millimeters (5/16-inch);(5) Have a breaking strength of at least 5 kilonewtons (1,124 pounds); and(6) Be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Inspectors identified lifebuoys with lines that were not in conformity with regulations as well as not labeled with vessel's name. Outfit lifebuoys with proper arrangements. 46 cfr 117.70

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Identified lifesaving equipment not labeled with vessel's name. Outfit vessel with lifesaving equipment that is labeled. 46 cfr 122.604

A life preserver shall be of a highly visible color, such as Indian Orange, International Orange, or Scarlet Munsell Red. Identified multiple life preservers that have lost its highly visible color. Outfit vessel with proper life perservers. \ 46 cfr 160.001

panelboard must have:(1) A circuit directory cardholder; and(2) A circuit directory that has:(i) The circuit designation of each circuit;(ii) A description of the load of each circuit; and(iii) The rating or setting of the overcurrent protective device for each circuit. Identified panel in pilothouse to be mislabeled and outdated. Outfit vessel with updated circuit directories for all panels. UPDATE 28AUG24: Install proper breakr blanks or spare braker in fwd galley pannel. Switch off Spare breakers. 46 CFR 111.40-11

a weathertight door with permanent watertight coamings that comply with the height requirements in table 171.124(d) must be provided for each opening located in a deck house or companionway that—(1) Gives access into the hull. Identified multiple doors that were not weatherproof. Outfit vessel with weather proof doors. 46 cfr 171.124

All bulkheads and decks shall be classed as A-60, A-30, A-15, A-0, B-15, B-0, or C, depending upon the type of space on each side of the bulkhead or above and below the deck. Inspectors identified spray foam used to seal deck/bulkhead penetrations in numerous cases on the vessel. Maintain the structural fire protection of decks as required. 46 cfr 72.01-10

The Certificate of Inspection issued to a vessel describes the vessel, the route(s) that it may travel, the minimum manning requirements, the survival and rescue craft carried, the minimum fire extinguishing equipment and lifejackets required to be carried, the maximum number of passengers and total persons that may be carried, the number of passengers the vessel may carry in overnight accommodation spaces, the name of the owner and managing operator, any equivalencies accepted or authorized by the Commandant or any Officer in Charge, Marine Inspection (OCMI) in accordance with § 175.540 or § 175.550 of this chapter, and such other conditions of operations as may be determined by the cognizant OCMI. Prior to the carriage of passengers, have the cognizant OCMI conduct a new to zone inspection.

Each vessel must have a stability letter issued by the Coast Guard before the vessel is placed into service which sets forth conditions of operation. This vessel is permitted to operate on Protected Waters. Prior to departing Port Canaveral for an intended voyage to New York, provide voyage plan to MSU Port Canaveral for approval in accordance with COTP Order 24-038 issued on 24 May 2024.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. The bilge pump and secondary bilge pump (fire) do not operate as designed. Prove proper operation of both bilge pumps.

A self priming, power driven fire pump must be installed on each vessel. Prove proper operation of the fire pump.

The owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Demonstrate the steering system operates as designed.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The port shaft seal leaks excessively.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. The port engine shaft seal leaks excessively. The bilge pumps do not operate as designed. Proper operation of the steering system is in question. Prior to movement, submit plan for rectifying these conditions to the OCMI.

Vessel unable to conduct required drills at inspection. Vessel must conduct & document required training and drills for fire, man overboard, abandon ship. 46CFR122.520 46CFR122.524

ALL VESSELS SHALL BE PROVIDED WITH NOT LESS THAN TWO AVENUES OF ESCAPE FROM ALL GENERAL AREAS ACCESSIBLE TO THE PASSENGERS OR WHERE THE CREW MAY BE QUARTERED OR NORMALLY EMPLOYED. MAIN DECK FORWARD EGRESS ROUTE DOOR IS BLOCKED BY A BROW AND EQUIPMENT. MAKE EGRESS ROUTE CLEAR. 46 CFR 177.15-1

VESSELS SHALL BE INSPECTED FOR COMPLIANCE WITH STANDARDS REQUIRED BY THIS SUBCHAPTER. FOR ITEMS NOT COVERED, INSPECT IN ACCORDANCE WITH STANDARDS ACCEPTABLE TO [name withheld] MUST BE CAPABLE OF WITHSTANDING THE MAXIMUM LOAD FROM CONDITIONS EXPECTED DUE TO ITS LOCATION ON THE VESSEL AND AUTHORIZED ROUTE. REPLACE BROKEN WINDOW ON PORT SIDE MAIN DECK. 46 CFR 176.25-1 46 CFR 116.1020

RAILS SHALL BE INSTALLED NEAR THE PERIPHERY OF ALL WEATHER DECKS ACCESSIBLE TO PASSENGERS AND CREW. FOC’SLE HAND RAIL HAS BEEN BROKEN OFF OF ITS MOUNTS BY THE FORWARD “DECORATIVE” BROW. 46 CFR 177.35-1

MARINE INSPECTOR SHALL VIEW AND CONDUCT THE FOLLOWING TEST AND INSPECTION: TEST BY OPERATION ALL WATERTIGHT CLOSURES IN THE HULL, DECKS AND BULKHEADS. MAKE ENG ROOM WATERTIGHT DOOR CLOSE PROPERLY. 46 CFR 176.25-5

THE TWO REQUIRED FIXED POWER BILGE PUMPS SHALL BE SELF-PRIMING AND SHALL BE DRIVEN BY DIFFERENT SOURCES OF POWER. MAIN DIESEL ENGINE DRIVEN PUMP WOULD NOT PRIME. ENSURE PUMP IS OPERATIONAL AS DESIGNED. 46 CFR 182.25-10(C)

VESSELS NOT EQUIPPED WITH A SINGLE SOURCE EMERGENCY LIGHTING SYSTEM SHALL HAVE INDIVIDUAL STORAGE BATTERY POWERED AUTOMATICALLY OPERATED LIGHTS IN STRATEGIC LOCATIONS. 2ND DECK EMERGENCY LIGHTING INSTALLATION WAS NOT OPERABLE. 46 CFR 184.30-5(B)

Vessel not in operation/nor carrying any passengers. Vessel under contract for sale. Vessel did not provide crew to conduct MOB, Fire, Security drills. Vessel must conduct drills in the presence of a marine inspector prior to the carriage of passengers.

Vessel did not have annual testing records identifying the total number of individuals tested and test results. Vessel must provide annual testing records. The MIS report will satisfy these requirements.

182.20-40(B)(3) The shut off valve at the tank shall be operable from outside the tank compartment. The is not operable from outside the compartment, the reach rod and valve handle is not connected to the valve stem.

183.350(f) Battery charges must have an ammeter connected in the charger circuit. There is a battery installation under the main deck bar with a battery charger that is not equipped with an ammeter.

185.30-15 All escape hatches shall be marked on both sides using at least 1 inch letters “Emergency Exit, Keep Clear.” The engine room escape hatch was not labeled on both sides.

180.78(a)(5) Child Life Jackets must be stowed separately from adult life jackets. Throughout the vessel, child and adult life jackets were stored together.

Vessel All around Anchor light inoperable.

Vessel PA System inoperable. Prove proper operation of PA System.

Vessels High Water Alarms in all spaces were inoperable. Prove proper operation of all HW Alarms

STBD Side Air Compressor on vessel not operating in a proper working condition.

Vessel Structural Fire Protection insulation in Engine room and Muffler room needs to be secured in various locations.

Make repairs and prove proper operation of the escape hatch/scuttle in engine room.

Make repairs and prove proper operation of the escape hatch/scuttle in muffler room.

Vessels Emergency lights were inoperable. Prove proper operation of emergency lighting system.

Add one (01) Ring Lifebouy to make a total of three (3) onboard. One Lifebuoy removed due to cracked serfice.

Need to add (01) Floating Lights to Lifebuoy as required.

Remove adhesive residue from deck to avoid tripping/slipping hazards.

Add and secure High Temp lagging/insulation on Port and Starboard Engines and generators in areas missing or not secured.

Provide chaffing gear on Stbd engine fuel pump line to avoid chaffing of line on engine.

Prior to recovering 1st deck floor covering, provide material specifications to the cognizant OCMI to ensure correct fire rating.
