The illustrations and instructions must include information on (1) The fire and emergency signal;(2) Their muster station;(3) The essential actions they must take in an emergency;(4) The location of lifejackets, including child-size lifejackets; and(5) The method of donning lifejackets. The bunker delivery note shall be kept on board the ship in such a place as to be readily available for inspection at all reasonable items. It shall be retained for a period of three years after the fuel oil has been delivered on board. Vessel found with no fuel receipts onboard. MARPOL VI/18.6 Complete prior to DD/ISE credit.

The information on authorities or persons to be contacted in the event of an oil pollution incident as required 33 CFR 151.26(b)(3)(iii). This info must also clearly specify who will be responsible for informing the necessary parties from the coastal State contacts,the port contacts, and the ship interest contacts. This info must include an appendix of port contacts for those ports at which the vessel regularly calls. The port contact list within the SOPEP found onboard was from 2009 and is outdated. Every ship of 400 gross tonnage and above other than an oil tanker shall be provided with an Oil Record Book Part I. The Oil Record Book Pt I shall be completed on each occasion, on a tank-to-tank basis if appropriate, whenever any of the following machinery space operations take place in the ship: .1 ballasting or cleaning of oil fuel tanks; 2. discharge of dirty ballast or cleaning water from oil fuel tanks; .3 collection and disposal of oil residue; 4. discharge overboard or disposal otherwise of bilge water which has accumulated in machinery spaces; and 5. bunkering of fuel or bulk lubricating oil. MARPOL VI/17.2

The master or person in charge of a ship to which this section applies shall ensure that a written record is maintained on the ship of each of garbage discharge or disposal operations, including discharge to a reception facility or another ship. No garbage record book entries or receipts for waste disposal available.

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management methods: Use only water from a U.S. public water system (PWS), as defined in 40 CFR 141.2, that meets the requirements of 40 CFR parts 141 and 143 as ballast water. Vessels using water from a PWS as ballast must maintain a record of which PWS they received the water from as well as a receipt, invoice, or other documentation from the PWS indicating that water came from that system. Furthermore, they must certify that they have met the conditions in paragraphs (a)(2)(i) or (ii) of this section, as applicable, and describe in the BWM plan the procedures to be used to ensure compliance with those conditions, and thereafter document such compliance in the BW record book. The vessel employs the use of PWS for ballast but did not have records onboard from the last time they received ballast from a PWS. 33 CFR 151.2070(a)

The bunker delivery note shall be accompanied by a representative sample of the fuel oil delivered taking into account guidelines developed by the organization (IMO MEPC.182(59)). Where pressure-indicating gages are used, they shall be chosen to be compatible with the pressure to be indicated. The size of the visual display, the fineness of graduations, and the orientation of the display will be considered. In no case shall the upper range of the gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. Gage ranges on all three air receivers were found out of parameters. All three shall be replaced to coincide with relieving device. 46CFR54.10-5(f)

The revised NOx Technical Code 2008 shall be applied in the testing, certification and measurement procedures for the standards set forth in this regulation . Each marine diesel engine specified in 1.2, except as otherwise permitted by this Code, shall be subject to the following surveys: A pre-certification survey which shall be such as to ensure that the engine, as designed and equipped, complies with the NOx emission limit contained in regulation 13. If this survey confirms compliance, the Administration shall issue an Engine International Air Pollution Prevention (EIAPP) Certificate. The EIAPP certificate and associated engine Technical File for the generator were not available for inspection. MARPOL VI/13.8 NOx Code 2.1.1.1

Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection, and shall include, as applicable, a demonstration of - The proper condition of flotation equipment such as lifebuoys, lifejackets, immersion suits, work vests, life floats, buoyant apparatus, and associated equipment; MI observed life ring buoy float light manufacturers requirements for annual battery replacement were not documented. Operator is required to provide documentation for annual float light battery replacement and log future replacements annually. 46CFR199.45(b)(3)

Each lifebuoy stowage position must be marked with either the words “LIFEBUOY” or “LIFE BUOY”, or with the appropriate symbol from IMO Resolution A.760(18). MI observed the lack of marking as required by this cite. Operator to comply with cite and provide picture after rectifying deficiency. 46CFR199.70(a)(1)(iii)

MI observed the current donning instructions affixed to the life jacket locker are not the specific instructions for the model of life jacket carried onboard. Operator is required to post donning instructions applicable to the model of lifejackets carried on board and provide a picture of the instructions installed on the lifejacket locker. 46CFR199.80(c )(5)

Each muster station and embarkation station must be adequately illuminated by lighting with power supplied from the vessel's emergency source of electrical power. MI observed the absence of lighting at the life raft embarkation station. Operator is required to comply with the requirements of this cite and provide MI photos and documentation of the installed light in regards to suitability of service. 46CFR199.110(c )

Structure of the vessel, coamings, closures, and all means of protection provided for openings, such as for ventilators, companionways, machinery casings, fiddleys, funnels, enclosed superstructures on the freeboard deck (and their end bulkheads) or equivalent protective deck houses, openings in the freeboard and superstructure decks, and significant openings at higher levels in the vessel. MI observed all weathertight doors in need of maintenance, significant corrosion has degraded the original material thickness on multiple portions of the doors, lack of adjustment and lubrication on the mechanical portions of the doors has made operation difficult. Operator is required to perform maintenance on the weather tight doors as good marine practice and to the satisfaction of the attending MI. 46CFR42.09-25(b)(2)

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he or she deems necessary to be assured of the safety and seaworthiness of the vessel. MI observed that 02 paint lockers had been installed on the aft deck with no provisions to secure against tip over. Operator is required to secure or remove paint lockers in a manner as to not invalidate the FM certification of the paint lockers. 46CFR91.25-50

Except as provided in paragraph (d)(3) of this section, the number of pounds of carbon dioxide required for each space shall be equal to the gross volume of the space divided by the appropriate factor noted in Table 95.15-5(d)(1). If fuel can drain from the compartment being protected to an adjacent compartment, or if the compartments are not entirely separate, the requirements for both compartments shall be used to determine the amount of carbon dioxide to be provided. The carbon dioxide shall be arranged to discharge into both such compartments simultaneously. MI observed 03 paint lockers situated on the aft deck with no readily available means of fire extinguishing. Operator is required to adhere to this cite and install applicable type and size of extinguisher to the satisfaction of the attending MI. 46CFR95.15-5(d)(1)

The entries into the ORB, which ended in June 2019, did not include all information required, by the standard codes. 33 CFR 151.25(a)

Your vessel must undergo an annual inspection within 3 months before or after each anniversary date, except as specified in paragraph (b) of this section. The annual inspection for the WEEKS 571 (N.O. 1114673) was due to be completed by January 15, 2023, the annual inspection wasn't completed until March 9, 2023.
