Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Stbd fuel oil shutoff valve was inoperable from weather deck and local operation. Upon further investigation by operator, both valves were found inoperable. 46 CFR 182.455(b)(4)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: A space with a non-watertight closure, such as a space with a non-watertight hatch on the main deck. Fwd void bilge high level float did not alarm at operating station upon actuation. 46 CFR 182.530(a)(3)

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. Aft machinery light was extinguished and did not adequately illuminate the space. 46 CFR 183.200(a)

Independent fuel tanks must be located and installed as described in this paragraph (b). All fuel tanks must be electrically bonded to a common ground. Fuel tank was grounded to fuel oil shutoff deck access covers not to a common ground. 46 CFR 182.440(b)(4)

Each survival craft must be: Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. IBA sea painter was not attached to weak link.46 CFR 180.130(a)(2)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel's testing devices were expired. Until such time that valid testing devices are available onboard,vessel must stay within 2 hours of a location to perform the required alcohol tests from the time of occurrence of the SMI. Vessel will need to replace testing devices as required and or show written policy attesting to how the vessel will obtain the devices and conduct the required alcohol tests within 2 hours from the time of occurrence of the SMI.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Retro reflective material was wasted on ring life buoy. Replace/repair ring life buoy to satisfaction of attending Marine Inspector.

The emergency instructions required by 46 CFR 185.510 must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Vessel does not conduct Man-overboard procedures as dictated on posted emergency instructions. Amend emergency instructions to address the particular operation of the vessel and provide to attending Marine Inspector.

A vessel that is exposed to salt water for more than three months in any 12 month period must undergo a drydock examination and internal structural at least every two years. Vessel due for drydock and internal examination on 31JAN2020. Vessel prohibited from carriage of passengers or cargo until drydock and internal structural examinations have been completed to the satisfaction of attending USCG Marine Inspector.

Clean or replace running light lenses.

Provide current coast pilot, light list, tides/currents, and charts.

Incorrect MMSI programmed into DSC VHF radio. Program correct MMSI.

Mark fuel oil shutoff valve locations.

Provide a portable fire extinguisher in the E/R.

Provide updated pubs (coast pilot, light light, tide tables) hard copy or electronic.

Update first aid kit IAW 46 CFR 160.041.

Prior to carrying psgrs >3NM, provide proof of current EPIRB registration.

Establish drug/alcohol policy

Master must complete 1-hr supervisory training.

Replace E/R light switch.

Replace cover on receptacle near galley sink.

Install 2nd hose clamp on fuel shut-off valve.

Laz: clean terminal block & connections.

Post EAP 800 number in a location accessible to all crewmembers.

Provide suitable storage location for combustible liquids or remove from vessel.

Repack rudder shaft seal.

Make the collision bulkhead water tight in way of the access plate.

Double clamp the hose connection to the port through-hull valve.

EPIRB failed to meet the applicable requirements. Replace expired hydrostatic release & expired EPIRB battery.

Make vessel available for hull inspection prior to the carriage of passengers. This item to be completed to the satisfaction of the attending Marine Inspector.

Provide two alcohol test kits.

Conduct MOB and Fire drills with new crewmember and log in vessels logbook.

Provide new hydrostatic release for EPIRB.

Provide updated publications.

Provide two alcohol test kits onboard vessel prior to operating with passengers.

Replace retro-reflective tape on life ring and life float.

Prove proper operation of bilge high level alarms. Crew required to make 30 min. rounds of all spaces until proper operation of bilge alarms can be proven to Coast Guard Marine Inspector. All rounds must be logged in the vessels log until proper operation is proven to Coast Guard. Alarms must be fixed no later than 24 Sept. 2007.

Replace battery in EPIRB. Vessel restricted to less than 3 NM until properly repaired.

Replace/Recharge one B-II portable fire extinguisher prior to operating with passengers.

Replace outdated items in first aid kit.

Prove proper operation of bilge pump prior to carrying passengers.

Provide new EPIRB (new registration required). Provide NOAA registration date, hydrostatic release exp. date, and EPIRB battery exp. date. 46 CFR 180.64 Vessel is ltd to operating NMT 3 miles from shore.

Renew bait box drain hose (old RULE pump hose) in lazarette. Hose is to meet 46 CFR 182.520 (f).

Provide mechanical fastening for outboard (P&S) ends of underdeck strongbacks in way of aft main deck soft patch in lazarette (above the fuel tanks). 46 CFR 177.300

Repair/renew electrical supply terminals for aft compt. sub-bilge pump. 46 CFR 183.340

Replace both fuel tank vent screens with 30 X 30 mesh, corrosion resistant, metallic vent screens. (Presently fitted with 20 X 20 mesh with gaps larger than vent openings) 46 CFR 182.450 (e)

Replace power supply cable (presently orange colored elect. extension cord) for potable water pump, sewage overboard pump, and wash-down pump (and any others not noted) with cable meeting 46 CFR 183.340. (A jumper cable perm. installed to a 110 VAC invertor was also noted to master)

Remove/relocate handle for sewage overboard valve or secure in "closed" position with a tamper-resistant device. 33 CFR 154

Prove proper operation of the automatic shut-down (and reset) devices for both main diesel engines upon discharge of the fixed fire extinguishing system. 46 CFR 181.420 (a) Must be completed prior to the carriage of passengers.

Install guards around exposed flywheel/accessory pullies in front of both main diesel engines. 46 CFR 177.960

Remove petcock drain valves from both Racor fuel filters and replace with either solid plugs or solid-body drain valve fitted with a plug or cap. 46 CFR 182.455(b) (9).

Prove proper operation of radar (presently won't orient proper heads-up display). 46 CFR 184.404 The vessel may not carry passengers on voyages off-shore until corrected.

Demonstrate to attending CG marine inspector, satisfactory fire and man-overboard drills. 46 CFR 185.520

REPLACE LIGHT FIXTURE IN STORAGE COMPARTMENT FORWARD OF STEERING/FUEL TANK COMPARTMENT WITH AN APPROVED MARINE LIGHT FIXTURE.

REPLACE GASKET TO AFT HATCH COVER TO STEERING/FUEL TANK COMPARTMENT.

RENEW THE STERN RUNNING LIGHT.

REPLACE PACKING ON PORT AND STBD PACKING GLANDS OR ADJUST TO ELIMINATE EXCESSIVE LEAKING.

DOUBLE NUT OR DRILL AND PIN STEERING ASSEMBLY PRIOR TO CARRIAGE OF PASSENGERS.

MAKE PORT AND STARBOARD RUDDER QUADRANT AVAILABLE FOR INSPECTION AFTER INSTALLATION.

REPLACE THE PORT EMERGENCY FUEL SHUTDOWN VALVE.

PROVIDE VENTILATION CLOSURES FOR ALL VENTILATION OPENINGS SERVING THE ENGINE COMPARTMENT.

THE OFFICER IN CHARGE, MARINE INSPECTIONS, HONOLULU, HAWAII, SHALL BE CONTACTED BEFORE THE VESSEL CARRIES PASSENGERS FOR HIRE.
